1 WORK AND PROJECT REQUIREMENTS BID PACKAGE 3A SITE CONCRETE & CONCRETE TURNKEY CRABAPPLE MIDDLE SCHOOL REPLACEMENT April 26, 2018 EXHIBIT B CONSTRUCTION DOCUMENTS AND SPECIFICATIONS shall be acknowledged by subcontractor. Subcontractor has reviewed 100% Construction Document GADOE Final drawings dated 04/13/2018 and Specifications dated 04/13/2018. Subcontractor has reviewed the following specification sections including, but not limited to: Cast-In-Place Concrete, Rigid Paving and Site Concrete SUBCONTRACTOR shall prosecute and complete the Site Concrete & Concrete Turnkey scope of work: The following items are included as part of this subcontract and are listed for clarification only. They are not intended to limit the scope of work nor do they diminish the subcontractor obligation to provide a complete and fully functional installation. The scope of work included but is not limited to, the following: A. General Requirements 1. Subcontractor agrees to comply with photo badging system for all workers and to supply all workers with a reflective safety vest with Subcontractor s name or logo in a prominent location on the vest. Vests are to be worn at all times while on site. 2. There is no on-site parking. WINTER cannot guarantee access to a remote parking area. Costs associated with transportation to the jobsite will be at the responsibility of the Subcontractor. Subcontractor will be limited to no more than one (1) crew vehicle on the project. 3. The campus and jobsite are 100% tobacco and smoke-free. This includes but is not limited to all forms of smoked and smokeless tobacco, as well as vaping and e-cigs. 4. Subcontractor employees are not permitted to enter campus buildings or dining halls. Workers are confined within the limits of the construction fence. 5. Subcontractor employees shall not fraternize or socialize with campus employees, students, and parents. Infractions will be subject to immediate and permanent removal from the project. 6. Deliveries to will not be permitted during the timeframe of student drop-off and pick-up. Actual times will be posted at the project office and are subject to change based on school s activities. 7. Due to logistical and space constraints, Subcontractor will be required to provide Just-in-Time deliveries. Storage containers (Conex) may not be allowed on site. No Subcontractor office trailers will be allowed. 8. Due to construction occurring on an occupied school campus, Subcontractor will be required to refrain from certain noise-making or disruptive activities because of school activities. 9. Walking through the campus from off-site parking area to the jobsite will not be permitted. 10. Compliance with BIM Coordination Execution Plan as applicable. 11. Work onsite is subject to compliance with the City of Roswell Noise Ordinance. Outside work is permitted between the hours of 7:00 AM to 7:00 PM, Monday through Saturday. 12. Where work is required within a specially controlled access environment, or where students are or may be present, work shall be performed by personnel who have passed, or can pass, a security screening. B. Trade Specific Requirements 1. Subcontractor includes the following: Site Concrete:

Concrete Walks i. Including reinforcing c. Heavy Duty Concrete Paving i. Including GAB ii. Including reinforcing d. Concrete Steps on grade and reinforcing e. Cast-in-place retaining walls and reinforcing i. Walls B, C, & D f. Flared Concrete Ramps g. Raised Vehicular Concrete crosswalk h. Handicap ramps, including tactile pavers i. Brick Pavers, including entry plaza and exterior courtyards. i. Includes granular base j. Concrete walls and footings at knee walls/benches (See A1.11) k. Installation of bollards. Bollards provided by others. l. Installation of flag pole base m. Monument sign footing n. Fine grading to achieve subgrade for concrete flatwork o. All exterior equipment pads and supports. i. Includes but is not limited to dumpster pad and curb, cooling tower, transformer, and generator p. Header curb at track to be by others. Concrete: a. Spread Footings b. Pier or concrete cover at footings as required (details 1 and 7 on S3.01) c. Continuous Footings d. Slab on Grade a. Concrete mix for slabs on grade shall have maximum Water Cement Ratio of e. Gravel placement under Lower Level slab-on-grade (per detail below) a. Includes furnishing and installing of 57 stone at lower level slab on grade to complete underslab drainage system. Site contractor shall provide 57 stone to 24 below finished floor, +/ f. Sloping & Depressions within Slab on Grade as required g. Concrete conveyance h. Pan stair fill i. Topping slab at elevator pit j. All concrete accessories k. Steel pipe bollard (furnished by others) fill and installation (see sheet A1.11) l. Turndowns at Slab Edge m. Unit Prices: In accordance with Article 3.2, the following unit prices shall be used in connection with the following items of Work. Unit Prices includes all costs of Subcontractor's performance of this Work, including, by way of example and not limitation, the costs of labor, supervision, materials, equipment, insurance, taxes, jobsite overhead, home office overhead, profit and any other costs or expenses in connection with, or incidental to, the performance of that portion of the Work to which such unit prices apply D. Allowances: The Subcontract Price specified in the Subcontract includes the following allowances, if any, to be furnished by Subcontractor in accordance with the Contract Documents. Each allowance and costs associated with each allowance must be reconciled by change order. If work associated with an allowance is performed on a unit price basis, Subcontractor shall provide a record of time and material no later than the following business day. Subcontractor shall not perform work associated with an allowance without prior written approval from WINTER. Subcontractor shall not exceed the amount of the allowance without prior written approval from WINTER. Such Allowances are considered complete and include all materials, taxes, equipment, labor, delivery, installation, overhead, and profit and any other costs or expenses in connection with or incidental to, the performance of that portion of the Work to which such Allowances apply. Note that "days" as indicated in the attached Schedule indicate work Days in lieu of Calendar Days. Work days shall comply with work hours specific to the project.
