Tenders may be submitted by the following means: Sealed with the words Gravel Crushing to the Rural Municipality of Spiritwood No. 496 Box 340 Spiritwood, Saskatchewan S0J 2M0, or Faxed with a cover page stating Gravel Crushing to , or ed with the subject line stating Gravel Crushing to rm496@sasktel.net Tenders will be opened at the Regular Meeting of Council on February 14 th, 2017 at 2:30 p.m. I, the undersigned, hereinafter called the Contractor hereby offer to the R.M. of Spiritwood No. 496, hereinafter called the R.M., to furnish all necessary tools, equipment, plant services, and labour to execute the work set out in the Contract Documents. Pursuant to and in compliance with the Invitation to Tender and the proposed Contract Documents relating to, the Contractor, having become thoroughly familiar with the terms and conditions of the proposed Contract Documents and with the local conditions affecting the performance and cost of the work at the place where the work is to be completed, and fully inspected the site in all particulars, hereby offers to fully perform the work including furnishing of any and all labour and materials and equipment. The Contractor also undertakes to do all work required to complete in accordance with the Contract Documents at the price tendered as follows in the schedule of items and prices. 1

The Contractor shall crush all the gravel listed in Table 1 by May 31, 2017, and all gravel listed in Table 2 by June 30, An extension of time required to complete the contract may be extended if agreed in writing between the Contractor and the R.M. of Spiritwood No The contractor is responsible to contact the R.M. of Spiritwood Municipal Office at (306) ten (10) business days prior to the estimated date of the completionof crushing a gravel pile, in order thatan engineer can be contacted to take measurements. 3. Payment will be made based upon measurements taken by engineer, within 10 days of the completion of crushing, with no shrinkage. 4. Primary jaw must be capable of crushing rock up to 28 inches in diameter. 5. The quantity and types of crushed gravel and quantities of reject sand will be in accordance with Table 1 and Table 2, and as described in Table The Municipality may contract an engineer to take random sampling of the gravel crush to ensure compliance of gravel quality. 7. The Contractor shall complete the Gravel Crushing Agreement attached to this Request for Proposal and submit it with the tender documents. The R.M. of Spiritwood No. 496 will complete the Gravel Crushing Agreement with the successful contractor. 8. The R.M. of Spiritwood No. 496 reserves the right to reject any and all Request for Proposals. The lowest bid, or any bid, may not necessarily be accepted. 9. The Contactor shall complete all work described in this tender by the dates outlined in Section 1, or a penalty of $500/day may apply at the discretion of the R.M. until the work is completed. 10. The successful contractor will be required to provide proof of liability insurance of no less than $2,000,000.00, a letter of good standing from WCB. 2
