LIST OF UESTIONS AND METRO RESPONSE(S) The following questions and METRO Responses are hereby made a part of and subject to the Request for Proposals. Item 1 2 A uestions / Answers With regard to the 7 business day turnaround for deliverable after excavation. Does the 7 days begin when assignment is received, when excavation is complete, or some other time? Pothole information needs to be submitted 7 days after completion of the excavation. Section 4.D.2 Scope of Services Will 7 day response time be extended when minimum 7 day notification to inspector is required when prohibiting left turns at signalized intersections? A Same answer as # A A A A Section 9.B of Scope of Services Will Horizontal Survey Control be provided in addition to elevation benchmark information? Also, will pothole requests included coordinates of requested potholes? METRO will provide Horizontal Survey Control Points along with Temporary Bench Marks but the pothole request will not include coordinates, it the responsibility of the Contractor to locate the utility, provide Horizontal and Vertical coordinates based on City of Tempe Datum as referenced in paragraph 9.B. Section 4.G of Scope of Services refers to City of Phoenix Traffic Barricade Manual, should this actually refer to the City of Tempe Traffic Barricade Manual? The City of Tempe Traffic Barricade Manual is incorporated by reference. See website to download said document. See paragraph 5.1 in the City of Tempe Traffic Barricade Manual wherein it states; The city has adopted the policies, standards and guidelines for proper design set forth in the City of Phoenix 2007 Traffic Barricade Manual except those sections that provide information such as fines, websites, specific departments, and phone numbers with the following modifications: This section also states; A copy of the City of Phoenix Traffic Barricade Manual can be obtained from the Right-of-Way Management Section counter located at Phoenix City Hall, 200 W. Washington St, 6th floor. Will plating of potholes be allowed (to allow the CLMS time to dry)? No, plating of potholes will not be acceptable. Bid Item No. 6 is in conflict with Item No. 13 in Attachment A Scope of Services. Please clarify if hot asphalt or concrete will be used for pothole repairs in the roadway. Revised paragraph 13, 4 th paragraph to read: Payment for potholes in the roadway shall require the removal and replacement of pavement and shall be at the unit price specified for Sawcut, remove AC pavement and replace with 6-inch hot asphalt - permanent patch to match existing pavement grade at each potholed Location per Section 11. Revised paragraph 15 to read: METRO and/or Consultant(s) will make requests for locating and potholing existing utilities. At the request of METRO and/or Consultant(s), locating will be conducted in up to two (2) stages (requiring the SUE contractor to mobilize up to two (2) times). Stage I will include: APS 69 kv, 230 kv at various locations and all their 12 kv feeders that cross and/or parallel the tracks, SRP Electric 12 kv feeders, SRP Irrigation crossing plus private irrigation, SWG, Nitrogen lines, CenturyLINK, MCI/Verizon and storm drains built by ADOT along Mill Avenue. SUBSURFACE UTILITY ENGINEERING (SUE) This decision has been updated (12/9/11) by the Dept. of Labor and is incorporated herein and supersedes Attachment D1 included with the RFP and the version included in Addendum No. 1. 4 POTHOLING SERVICES FOR TEMPE STREETCAR Request for Proposal Contract No. LRT TPOT (c) (d) (e) (f) Thickness of existing pavement or concrete, if applicable. Size, type and elevation of other utility lines encountered in the same pothole. Horizontal location of centerline of the utility, using three (3) swing ties to physical structures existing in the field and survey ties to project centerline with offsets. All distances shall be measured to one-tenth of a foot. Specify type of utility and utility owner s name. Note City bench marks, as related to US&CGS NAVD88 B. Dry Hole, Utility Location and Pothole 12. WATER JETTING: The Contractor shall comply with all applicable criteria specified for Utility location and potholes. If the Contractor cannot locate the anticipated underground utility at the general location shown, the pothole shall be classified as a dry hole. If a pothole is classified as a "dry hole" the Contractor shall notify the METRO Inspector. The METRO Inspector shall make the determination if additional potholes shall be attempted in trying to locate the specific utility. The Contractor shall make every attempt to pothole utilities by vacuum extraction. In areas where the Contractor and/or subcontractor(s) are unable to use this method due to hardened subsurface conditions, the Contractor shall immediately notify the Inspector. Where directed by the Inspector, the Contractor shall use water jetting to penetrate the subsurface. 13. PAYMENT Payment for Utility Location and Potholes shall be based on the unit prices specified on the Bid Form, Attachment I Utility Location and Pothole including Excavation and Backfill, from Surface to 8-feet Deep, Utility Location and Pothole including Excavation and Backfill from over 8-feet Deep to 16-feet Deep or Utility Location and Pothole including Excavation and Backfill, from Surface to Required Depth in Excess of 16-feet. Payment for Dry Holes shall be at the unit price specified for the Dry Hole, Excavation and Backfill. Payment for Water Jetting shall be at the unit price specified for the Water jetting per Section 12 and as directed by the Inspector. Measurement for water jetting shall commence at the location where the hardened soil condition is encountered. The Contractor shall be paid a minimum of 8-feet (1 LF) for each pothole where water jetting is approved by the Inspector. Payment for potholes in the roadway shall require the removal and replacement of pavement and shall be at the unit price specified for Sawcut, remove AC pavement and replace with 6-inch hot asphalt - permanent patchconcrete to match existing pavement grade at each potholed Location per Section 11. Direction of utility (E-W, N-S, etc ) All reports for Utility Location and Pothole shall be sealed by a Registered Land Surveyor or Professional Civil Engineer properly licensed in the State of Arizona. As noted above, the Testhole Data Report and the Testhole Data Summary Report shall be prepared in the same format as the enclosed examples. Reports not in accordance with the requirements specified above shall be rejected. Submit CAD files of the potholes, layer each utility (Example: layer for water, sewer, electric, irrigations, storm water, etc ) Test Hole Data Summary Report should be in an Excel Spread Sheet format. 15. SUBSURACE UTILITY ENGINEERING (SUE) METRO and/or Consultant(s) will make requests for locating and potholing existing utilities. At the request of METRO and/or Consultant(s), locating will be conducted in up to two (2) stages (requiring the SUE contractor to mobilize up to two (2) times). The Contractor shall have at least three (3) years experience in utility location and potholing by the vacuum excavation method. Failure to submit the information may result in proposal rejection. In the listing above, the "SU" designation means that rates listed under the identifier do not reflect collectively bargained wage and fringe benefit rates. Other designations indicate unions whose rates have been determined to be prevailing. WAGE DETERMINATION APPEALS PROCESS 1.) Has there been an initial decision in the matter?
