Your quotation, to be accepted, must reach us by April 12 at 12:00 pm with the following attachments: Quotation Request (this page), Your Bid Form, Cost Price Analysis, Section 3 Forms, and MBE Participation. We are exempt from both Federal Excise and Ohio Sales Tax. This RFQ requires Davis-Bacon Wages. Return one signed copy to GDPM, Procurement Department, 400 Wayne Avenue, Dayton, Ohio , and retain one copy for your files. Todd J Daniel Buyer/Facilities Manager DESCRIPTION GDPM is requesting quotes for Flooring Installation at various locations (See Scope). Please see attached scope/specifications. Contractor will supply all material, tools and labor for this RFQ. This RFQ requires Davis-Bacon Wages, see attached Wage Decision. Contractor will be required to submit Payroll Forms. A new wage decision will be utilized when RFQ is awarded. Any suspected discrepancies should be brought to the attention of GDPM prior to submitting a proposal. Any specification or scope related questions or to view jobsite should be brought to the attention of Darryl Miles at or via dmiles@gdpm.org Monday through Friday, between the hours of 8:30 a.m. and 5:00 p.m. Please fax quotations to Todd Daniel at or by to compliance@gdpm.org by 12:00 pm on or before the day outlined above. No bid opening will be conducted. GDPM will award the contract on the best quote received, but reserves the right to waive any informalities in the bidding. This includes past performance, references and other factors. Contractor shall be responsible for obtaining and paying for all permits and inspections necessary to complete all work related to the specifications. All work shall comply with Federal, State and Local codes. Contractor shall repair any damage done by their employees in the performance of this work at no expense to GDPM. SCOPE OF SERVICES: 714 Plymouth # 307 remove all existing carpet, and tile and installing 18x18 premium vinyl tile existing cove base will not be replaced 59 Central # M-208,308 remove all existing carpet, and tile installing 18x18 premium vinyl tile existing cove base will not be replaced 241 Niagara # B remove all existing carpet, and tile installing 18x18 premium vinyl tile existing cove base will not be replaced 465 W. The contractor shall provide all materials and equipment to complete the job as required by all applicable state, city and local code and regulations. Additionally, the contractor is responsible for cutting, patching, work needed to accomplish the installation and shall clean up the work area at the end of each business day. It shall be the responsibility of the contractor to remove all construction debris daily; the contractor shall not dispose of any debris into dumpsters or containers under the ownership or operation of GDPM. 23 rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate). Union Rate Identifiers A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than "SU" or "UAVG" denotes that the union classification and rate were prevailing for that classification in the survey. Example: PLUM /01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers indicates the local union number or district council number where applicable, i.e., Plumbers Local The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate. Survey Rate Identifiers Classifications listed under the "SU" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA /13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier. Survey wage rates are not updated and remain in effect until a new survey is conducted. Union Average Rate Identifiers Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH /29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier. A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/cba rate of the union locals from which the rate is based. WAGE DETERMINATION APPEALS PROCESS 1.) Has there been an initial decision in the matter? This can be: * an existing published wage determination * a survey underlying a wage determination * a Wage and Hour Division letter setting forth a position on a wage determination matter * a conformance (additional classification and rate) ruling On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. 24 contact is not satisfactory, then the process described in 2.) and 3.) should be followed. With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations.
