1 TOWN OF HATFIELD REQUEST FOR PRICE QUOTATIONS GENERAL OVERVIEW The Town of Hatfield invites price quotations from qualified contractors for the preparation of technical plans, specifications and installation for energy retrofits at the Town Hall. See Attachment A for background information and a detailed scope of work. The Town may cancel this solicitation, in whole or in part, at any time that such an act is deemed in the Town s best interest. The Town reserves the right to waive any informalities or to reject any and all proposals in total or in part as may be deemed to serve the Town s best interest. The Town will not be responsible for any costs incurred by a proposer in preparing and submitting a response to this solicitation. The Town is the Awarding and Contracting Authority. PRICE QUOTE CONTENTS All submissions must include: A fully completed Attachment B (Price Quotation Form). All prices entered on Attachment B are to include all costs for labor and materials as well as any other costs associated with carrying out the work as described in the Scope of Services (Attachment A).

2 A fully completed Certificate of Non-Collusion (Attachment C) signed by an authorized official of the firm. A fully completed Non-Discrimination and Affirmative Action Certification (Attachment D) signed by an authorized official of the firm. A fully completed Compliance Certification (Attachment E) signed by an authorized official of the firm. SUBMISSION PROCEDURE The submission and review of price quotations, as well as contract documents, must comply with all applicable local and state regulations and Massachusetts Department of Energy policies regarding the use of funds for such projects. Acceptance of any price quote is subject to the continued availability of funding.

3 ATTACHMENT A TOWN OF HATFIELD BACKGROUND The Town of Hatfield has been awarded funding from the Massachusetts Department of Energy GCA Grant for the purpose of upgrading some of its municipal facilities with energy efficient heating systems and insulation. Hatfield is seeking price quotes for energy retrofit projects at several town buildings including the Town Hall. An evaluation was performed by an energy consultant, who provided recommendations that are expected to save the town substantial energy resources. Among the recommendations are provisions to upgrade the windows at Town Hall. The town will consider other energy upgrades that are determined to be within the allocated budget and adhere to the goals of the program. The contractor hired will be expected to provide detailed plans and specifications that incorporate the goals of the Town of Hatfield s Energy Plan and GCA Grant Application, which will be made available by the town. The contractor will be expected to employ the services necessary to comply with requirements stipulated by the Town Building Inspector in the process of acquiring building permit(s). Contractor will provide pricing for each Second Floor of the East Portion of the building Replacement Window and the Air Sealing of Casement with double glazed low E/argon filled window with thermal break sash with a u-value of.33 BTU/hr-s. The window must appear to replicate the original in design and style. Contractor will provide pricing for each Attic Floor of the East Portion of the building Replacement Window and the Air Sealing of Casement with double glazed low E/argon filled window with thermal break sash with a u-value of.33 BTU/hr-s. 7 ATTACHMENT E COMPLIANCE CERTIFICATIONS Qualifications and Taxes: The Contractor represents that it is qualified to perform the services required under this contract and possesses or shall obtain all requisite licenses and permits. Pursuant to M.G.L. c. 62C, 49A, I certify under the penalties of perjury that, to the best of my knowledge and belief, I am in compliance with all laws of the Commonwealth relating to taxes, reporting of employees and contractors, and withholding and remitting child support. Employment Security Contributions and Compulsory Workers' Compensation Insurance: Pursuant to MGL C.151A, S.19 and MGL C.152, the Contractor certifies with all laws of the Commonwealth relating to payments to the Employment Security System and all Commonwealth laws relating to required worker's compensation insurance policies. For Base Window proposal Finished is baked enamel White for Alternate #1 & Alternate #2 Sash lift furnished for field installation. Two lifts on units with make width 37" and greater. Finish to match sash locks. Optional Products Grilles Integral Light Technology grilles for Base and Alternate #2 Windows Grilles are solid 7/8" regular profile. Patterns are to Match Existing Exterior grilles are extruded aluminum painted to match window cladding. Clear pine interior grilles are Factory Stained to Match for Base Bid and Factory Painted white to match alternate #2. Insulating glass contains non-glare spacer grid between two panes of glass. Non-glare spacer is adhered to the glass. Grilles are adhered to both sides of the insulating glass with VHB acrylic adhesive tape and aligned with the non-glare spacer. Grilles-Between-the-Glass for Alternate #1 Vinyl Basement Windows Insulating glass contains 3/4" contoured aluminum grilles permanently installed between two panes of glass. Patterns are Traditional to Match existing light patterns Interior color is White.

10 Exterior color is White Screens InView Screens Half-Size black vinyl-coated 18/18 mesh fiberglass screen cloth complying with SMA 1201, set in aluminum frame fitted to outside of window, supplied complete with all necessary hardware or metal mesh screen as above. Screen frame finish is baked enamel, color to match Exterior window color Bid to include Removal and Disposal of all old windows and window materials according to Town of Hatfield bylaws, and Commonwealth of Massachusetts guidelines for the handling and disposal of such materials.
