Calls for nominations are made by the Vice-Chancellor s Office. All Staff currently on Multimedia University payroll will be invited to submit nominations. Retired staff are not eligible for nomination. 2. Individuals or teams may not self-nominate except for the Innovation Award. 3. An individual or team may only be nominated once, and for one award category only, in any particular year. 4. Nominees must have completed their period of probation at the time of nomination to be eligible for consideration for an award. 5. The Selection Committee reserves the right to not make an award in all/any particular category in any given year. 6. Nominations will not be considered if: a) received after the closing date; b) the prescribed Nomination Form is not used or is incomplete; c) the prescribed letters of support are not submitted with the nomination; and/or d) the prescribed word count is exceeded in any part of the Form/Nomination letter 7. A nominator is required to provide relevant information on why he/she believes the individual or team nominee should be considered for the selected award category. The nominator should ensure that the supporting documentation is as complete as possible. Nominators are asked to assess the contribution of a nominee against the criteria for the relevant award category. Nominators must focus the information provided on the nomination form specifically on the relevant selection criteria. 8. The nomination form and prescribed supporting documentation should make the full case for the nominee. The nominator will not have an additional opportunity to submit further information nor make the case for a nominee before the Selection Committee. 9. Two letters of support, from other colleagues and/or students are required as part of the nomination. One of the two letters of support may be from a person(s) external to MMU. Letters of support should address the criteria for the relevant award category and should not exceed 200 words. 10. Nominators will be required to submit the nomination form together with references and letters of support. 11. Deadline for receipt of nomination form with supporting documentation will be Tuesday, 31 st October, 2017, midnight (Any nominations received after this date will not be accepted). 1

3 University Staff Recognition Awards documentation and guidelines Read the following University Staff Recognition Awards guidelines before completing this nomination form. i. Nominees can only be considered for one award category. Please ensure this category is clearly identified on the nomination form. ii. Names will not be considered by the selection committee without a completed nomination form and supporting documentation. iii. Nominations will not be considered if the nomination form is received after the closing date for receipt of nominations. iv. Nominations will not be considered if the word count is exceeded in any part of the nomination form. Total word count for each section must be included. v. Nominators must focus the information provided on the nomination form specifically on the relevant selection criteria. vi. Employee of the Year Award This Award may be presented to individuals who demonstrate with exemplary performance in some or all of the following: i. Consistently and clearly surpass performance requirements of his/her position ii. Make significant contributions toward reaching the University s goals and objectives iii. iv. Demonstrate dedication to the University, and Exhibit a high level of leadership and cooperation in working with students, other employees, and the general public Examples might include, but are not limited to: a) Teaching Staff i. has made important and significant contributions in their area and is an asset to the staff of his/her department; ii. Consistently excelled in their positions and demonstrated integrity and a strong commitment to the mission and values of Multimedia University. iii. exhibits exemplary service in daily work of the individual and notable contribution to a Faculty/Department/University iv. willingness to take initiative and consistently and substantially exceeds the expectations of the position, performing at a level above and beyond normal job requirements and expectations; v. supporting the involvement of MMU students in University life beyond the classroom such that students feel like valued members of the University community; vi. demonstrating outstanding progress in developing flexibility in MMU academic/life-long learning programmes or other services; vii. outstanding contribution to student learning, demonstrating exemplary initiative in offering MMU students a personally &/or intellectually rewarding curricular &/or extracurricular learning experience; viii. developing innovative systems and processes that enhance the MMU student experience in a significant way. ix. consistently dependable and punctual in reporting for duty, completing 6

7 assignments on time, and participating in additional responsibilities. x. portrays a positive attitude toward work responsibilities, co-workers, and customers and serves as a role model for others. xi. xii. excels in knowledge and has expertise in his/her department. exhibits a willingness to work in a team setting within and/or outside of his/her assigned department. b) Non-Teaching Staff i. Consistently excelled in their positions, demonstrated integrity and a strong commitment to University s mission and core values; ii. willingness to take initiative and consistently and substantially exceeds the expectations of the position, performing at a level above and beyond normal job requirements and expectations; iii. has made important and significant contributions in their area and is an asset to the staff of his/her department. iv. Productive and exhibits commitment to quality in carrying out job responsibilities and to the University's mission and core values v. consistently dependable and punctual in reporting for duty, completing assignments on time, and participating in additional responsibilities. vi. Nominee portrays a positive attitude toward work responsibilities, co-workers, and customers and serves as a role model for others. vii. exhibits exemplary service in daily work of the individual and notable contribution to a Faculty/Department/University viii. excels in knowledge and has expertise in his/her department. ix. exhibits a willingness to work in a team setting within and/or outside of his/her assigned department. 2. Innovation Award The Innovation Award may be presented to an individual or a team who demonstrates the most outstanding achievements in some or all of the following: Building relationships /collaborations; Optimization of opportunities; Creativity and innovation; Self-starter capability. Examples might include, but are not limited to: the identification and optimization of opportunities that contribute to the successful delivery of one or more of the University s strategic goals; the introduction of an initiative that brings about a positive change in support of the University s strategic goals; improvements to policies/practices and/or programme design; the development of relationships/collaborations to enable MMU to meet one or more of its strategic goals; evidence of imagination, creativity and innovation which supports the achievement of the University's strategic goals. 3. Honesty and Respect Award 7

8 This Award may be presented to individuals who demonstrate the most outstanding contribution towards honesty and Respect at MMU. Examples might include, but are not limited to: Exhibiting particular tenacity or energy in pursuing honesty and respect initiatives; Raising the profile of MMU as an organization that exhibits best practice in relation to honesty and respect; Demonstrating creative initiative in progressing honesty and respect issues; Creating an inclusive environment at MMU that embraces diversity, honest and respect; Developing awareness of honesty/respect in others (e.g., through positive role modelling, through teaching & learning initiatives or curriculum development); Identifying opportunities to enhance the welfare of vulnerable individual (s); Collaborating, communicating or cooperating to enable or enhance honesty and respect provisions. Staff Recognition Committee The Staff Recognition Committee consists of representatives from both Academic and non-academic staff of the University appointed by the Vice-Chancellor. The Staff Recognition committee will evaluate and rank the nominees and forward the awardees to the University Management Board for approval. 8
