3 What? Paramounts Steps2Health program is designed to encourage you to become healthier by rewarding team members with a discount in payroll contributions for their health care. By completing the program we ve outlined, you will save money throughout the year on your health care costs. How? Meijer is offering team members a simple approach to saving money and improving your well-being. It s as easy as: 1 Complete the HRA and Select a PCP: Within 90 days of your effective date, visit www. paramounthealthcare.com/meijer to complete an online Health Risk Assessment (HRA) and select your primary care physician (PCP). 2 PCP Visit: Schedule an appointment and visit your selected PCP within 90 days. Complete the partnership agreement form with your doctor. 3 Partnership Agreement: As a member, you re responsible for returning the completed agreement to Paramount and following your doctor s directions, within the time frame indicated. The activities your doctor recommends are designed to help you maintain and improve your health. Remember that both you and your spouse (if enrolled) must complete these steps to continue (or begin) receiving the wellness incentive discount. 2

4 1 Step One: Complete the HRA and Select a PCP The Health Risk Assessment (HRA) is a confidential online tool that determines your current health status, as well as your potential risk for other health conditions. Access your HRA at: First time users will need to register. Make sure you have your Paramount ID card handy. This secure online portal will give you access to your HRA, as well as claims data, authorizations and more. The HRA will take about 10 minutes to complete. Detailed instructions can be found at: When all answers are complete, LOCK your HRA to complete the process. Print a copy to take to your PCP visit (Step 2). This gives you and your physician a list of discussion items. If you prefer to enter your blood test results from your PCP visit (Step 2), you may do so by NOT locking your assessment and coming back later to enter the numbers. Be aware! The HRA needs to be locked within 90 days of your effective date to qualify for your incentive. Go online to to select a participating PCP. These PCP s were specially selected for their commitment to help you improve your overall well-being. 3

6 2 Step Two: PCP Visit Upon your enrollment in Paramount Steps2Health, you are required to select a PCP. A list of participating PCPs can be found at: www. paramounthealthcare.com/meijer (click on the appropriate PCP Directory) A visit with your PCP needs to be completed within 90 days of your effective date in the plan. During your PCP visit, your physician will check your basic health indicators, such as blood pressure, weight and tobacco status. You and your physician should also review your HRA results from Step 1. If you are covering your spouse, they will need to complete this activity as well. While at your physician s office, the two of you will complete a Partnership Agreement (PA), which is provided by Paramount or can be found online (see link above). 5

7 3 Step Three: Partnership Agreement (PA) The PA is an outline of your personal action plan for the next 12 months. Your physician will help you determine what goals and course of action make sense for your current health status. A completed PA with your goals may look like this: Lower blood sugar by 5% Exercise and lose 10 lbs. Quit smoking Be sure to work toward your goals as outlined on the PA throughout the year. If your physician recommends a program or a follow up visit, Paramount will monitor your progress and remind you to keep on track. Becoming non-compliant with the program could mean losing some of your payroll contribution discounts on health care premiums. Your physician and Paramount are here to help you achieve your goals. Your completed PA needs to be sent to Paramount by fax * at: * Your PCP can fax the PA for you. Member Services can assist with an alternative method of submission if needed. 6

11 (FAQs) Frequently Asked Questions Q1 When will I receive my Paramount Steps2Health ID card? Your ID card will be issued as soon as Paramount receives your enrollment record. If your card is missing a primary care physician (PCP) name, see Q2 and Q3 to have the name updated. Q2 Is my primary care physician (PCP) a part of your program? Visit our website at and select the Provider Directory to determine if your PCP is one of the select physicians participating in this program. Q3 How do I select my PCP? There are three ways to select your PCP at Paramount: 1. Use the PCP Selection Form provided in your enrollment materials. 2. Call Paramount s Member Services at after you are enrolled. 3. Login to Paramount s website meijer and select your PCP once enrolled. Q4 What if my spouse doesn t complete the incentive activities? If your spouse is enrolled with you, both of you need to complete the steps necessary to receive the full incentive. If either you or your spouse does not complete the incentive activities as outlined, you will receive a partial payroll contribution discount. Q5 How do I get more information on how to complete each Steps2Health activity? Visit our website and select the activity in question for more detail on how to complete. You can also call Paramount s Member Services at for assistance with the program at anytime. Q6 What if I ve missed a deadline, can I still get the incentive? To help you meet deadlines, Paramount will send you a reminder notice before an incomplete activity is due. If you still miss the deadline, you can still earn the premium incentive at a later date but will have missed out on the savings in your paycheck from your contribution reduction. 10
