2 Congratulations! Your medical plan benefits includes a wellness program called, Your Healthy Reward$. In this program, you can earn rewards for participating in healthy activities and health coaching. Why a Your Healthy Reward$ program? Health care claim costs are continually rising each year, in an effort to control and lower health care costs for our members and Spartan, we needed a program that will positively impact the health of our employees (and spouses) enrolled in our medical plan. Your Healthy Reward$ is a wellness program designed to: reduce YOUR out-of-pocket medical costs provide YOU with resources to help YOU remain healthy or to improve a chronic health condition reward YOU for ongoing healthy behaviors and managing YOUR health As a participant, you will have the resources, tools and incentives to empower you to manage your health, adopt and maintain healthy lifestyles, and become better healthcare consumers. Your Healthy Reward$ The Program and Benefits The Your Healthy Reward$ program provides you an opportunity to EARN an Enhanced Benefit Level, reducing your annual deductible and out-of-pocket maximum amounts by 20%. Active participation will also reduce your co-pay for Primary Care Physician office visits to $10. There are TWO levels of benefits, 1. Basic Level standard annual renewal rates, including employee contributions and out-of-pocket deductibles and out-of-pocket maximums. 2. Enhanced Level - same employee contributions as the Basic Level, but, out-of-pocket deductibles and out-of-pocket maximums are reduced by 20% and Primary Care Physician office visits are only a $10 co-pay. All members (employee and spouse) are enrolled in the Basic Level until they EARN enrollment in the Enhanced Level. Dependent children will always be enrolled in the Enhanced Level. You can enroll in the Your Healthy Reward$ program to earn the Enhanced Level by completing 2 easy steps! 1

3 HOW TO ENROLL After completing the following two steps, you will be enrolled in the Your Healthy Reward$ Enhanced Level! Step 1: Participate in a Know Your Numbers preventive care screening. Earn a $25 Gift Card! Step 2: Complete a Health Assessment (HA) questionnaire. Earn a $50 Gift Card! You will remain enrolled in the Enhanced Level until the end of the year. LET S GET STARTED STEP 1: Participate in Know Your Numbers preventive care screening EARN A $25 GIFT CARD! Know Your Numbers screening, also known as Biometric Screening, can be done in your Primary Care Physician s office, at an on-site health fair, or at a convenient care clinic. These screenings include four key health tests: Blood Pressure Cholesterol. Blood Sugar Body Mass Index (BMI Your numbers in these areas provides a snapshot of your health and alerts you to areas of concern. These tests are part of our FREE preventive care benefits. After completing these tests, you will receive a wellness Health Report which will have your numbers on these screenings. To receive your $25 Gift Card, you will have to SELF-REPORT these numbers to HealthSCOPE Benefits (HealthSCOPE). Self-reporting these numbers is easy only two steps: 1. Obtain and complete a Know Your Numbers Data Form. You can obtain a copy from your Human Resources Team or from our HR Intranet. 2. Submit both the Know Your Numbers Data Form and a copy of your Health Report (from your screening) to HealthSCOPE by ing to Spartan@healthscopebenefits.com or faxing to Be sure to fill in your Member ID number (from your ID card) or Social Security number on the Know Your Numbers Data Form. HealthSCOPE cannot accept any forms without this information. Your location s HR Team can provide assistance in submitting your information to HealthSCOPE. You will receive your $25 Gift Card from your HR Team within 4-6 weeks from the date HealthSCOPE receives the information stated above. You will need these numbers to complete Step 2. STEP 2: Complete a Health Assessment questionnaire EARN A $50 GIFT CARD! A Health Assessment is a questionnaire that gathers important facts about you, such as your weight, height, age, preventive care screening numbers, smoking status, and lifestyle behaviors. There are NO family history questions. Typically, it takes minutes to complete. 2

4 YOUR INFORMATION IS CONFIDENTIAL. Your personal health information will remain confidential with HealthSCOPE and American Health in compliance with privacy regulations required for personal health information. Complete on-line or a paper copy! The Health Assessment can be completed on-line by logging onto our Plan s Wellness website provided by American Health, our disease and health management provider, at: You will receive a welcome letter from American Health which will include your confidential login information. Once you receive this letter, log in and complete your Health Assessment questionnaire right away! Now that you have completed Steps 1 & 2, you will be enrolled in the Enhanced Level until the end of the year. EARNING ENROLLMENT FOR THE NEXT YEAR To remain in the program and receive benefits at the Enhanced Level, you will have to complete four (4) Healthy Activities before November 15 of the current year to earn enrollment for the next year (beginning January 1). A list of Healthy Activities is located on page 4. Each completed activity will only count once. For example, if you have two dental cleanings during the year, only one will count as a Healthy Activity. If you enroll into the program after January 1, the required number of annual Healthy Activities to complete by November 15 are prorated as follows: If you enroll in the program by: March 31 - complete 3 activities June 30 - complete 2 activities September 30 - complete 1 activity We encourage you to complete the questionnaire on-line, however, you can request a paper Health Assessment from your local HR Team. Fax your completed paper copy to HealthSCOPE at or to Spartan@healthscopebenefits.com. Your location s HR Team can provide assistance in submitting your information to HealthSCOPE. You will receive your $50 Gift Card from your HR Team within 4-6 weeks from the date you completed the online assessment. If you submitted a paper copy of your Health Assessment, this time frame may be extended. The Your Healthy Reward$ program is designed to promote healthy activities and behaviors throughout each year. The Healthy Activity list on the next page includes over 20 activities to choose from. RE-ENROLLING If you were previously enrolled in the program, but did not remain enrolled. You can re-enroll by only completing STEP 1- Know Your Numbers preventive care screening. Submit your STEP 1 paperwork to your HR Team. They will submit to HealthSCOPE and notify them of your re-enrollment. 3

5 HOW HEALTHY ACTIVITIES ARE TRACKED HealthSCOPE will be tracking all completed Healthy Activities. HealthSCOPE will be receiving activity information from Spartan s claims system (examples mammogram, colonoscopy, wellness visits) and from other vendors, such as Delta Dental, and UnitedHealthcare Vision. American Health will be providing member participation information and reports to HealthSCOPE. Members can also self-report activities. HOW DO YOU SELF-REPORT? If you participate in a 5K walk/run, have a gym membership, join a weight loss program, or successfully complete a smoking cessation program, you will need to self-report these items to HealthSCOPE. Instructions on how to self-report are located on the Activity Participation Form. You can obtain a copy from your HR Team or the HR Intranet. You can view your completed activities and your current benefit level on HealthSCOPE s website. If you haven t already registered on HealthSCOPE s website, you can still do so. You can obtain a HealthSCOPE website login guide from your HR Team or on the HR Intranet, or just log onto click on Member, enter the Group ID, SPRTN, or our company name. On the next screen, click New Member Registration, and follow the directions. Healthy Activities Annual Wellness Exam and Know Your Numbers biometric screening tests; Annual Health Assessment; Mammogram; Colonoscopy; Prostate Exam; Annual Vision Exam; Dental Exam & Cleaning; Vaccinations, including Flu Shot; Skin Cancer Screening; Health Screenings; Cardiac Stress Tests; Participate in our Disease Management Program work with Health Coach; Participate in a local exercise, strength training, Martial Arts, Tai Chi, etc. class(s); Join a gym annual membership; Participate in local wellness-related events, such as Runs, Walks, Biking, or join a local sport team (softball, volleyball, etc.); Successful completion of a Smoking Cessation Program; Attainment of Primary Care Physician s recommendations for lowering Cholesterol, High Blood Pressure, and weight loss, as measured by your Physician or Wellness Coach; Participation in weight loss program as prescribed by Primary Care Physician, or join Weight Watchers, Jenny Craig, etc. Active participation is measured by your Physician or Wellness Health Coach. Complete a Lifestyle Wellness on-line or telephone coaching program on: Losing Weight Healthy Eating Managing Stress Quit Smoking Getting into Shape Exercise Completion of one coaching program counts as one activity. 4

6 HEALTH COACHING Our nurse health coaches from American Health can help you reach your health goals to stay healthy, get healthy or manage a chronic health condition. As a participant in our Disease Management program or our Lifestyle Wellness coaching program, you will have an opportunity to work with a knowledgeable health expert one-on-one to assist you in making incremental and manageable lifestyle changes that will improve your health. Disease Management Members with a chronic condition (as identified through analysis of claims or self-referral) may be contacted by a Nurse Health Coach from American Health. Chronic conditions include: Asthma Heart Disease Diabetes High Cholesterol Stroke Kidney Disease Cancer Participation in Disease Management coaching will count as a Healthy Activity. If your Nurse Coach has been unable to reach you by phone, you can call American Health at to begin your participation. Lifestyle & Wellness Coaching Most of us probably know areas where we could improve our health and happiness, but, we lack the motivation or simply do not know the best way to be successful in making changes to improve our situation. YOU now have a coach available to assist you! Our confidential Lifestyle Wellness Coaching program provides tools and resources to help you reach your health and weight goals. Lifestyle Wellness Coaching can help you with: Losing Weight Getting into Shape Managing Depression Managing Stress Quit Smoking Eating Better It s easy to get started! On-line or by phone, a Lifestyle Wellness Coach can help you develop personalized goals and provide the support you need to achieve them. Completion of one program listed on the Healthy Activities list counts as one activity. To work with a Coach by phone, call American Health at To use the on-line coaching and tools, log onto American Health s Wellness website at You should have or will be receiving a welcome letter in the mail from American Health. In this letter includes your unique login ID. If you do not have your unique login ID, contact your location s HR Team. PLEASE NOTE There may be times where you and your spouse (if applicable) are enrolled at different benefit levels because eligibility for the Enhanced Level is based on you and your spouse individually completing the requirements. You can enroll in the Your Healthy Reward$ program at any time during the year by completing Steps 1 & 2 as stated on page 2; or re-enroll by only completing Step 1 (page 3). PLEASE NOTE: Your Healthy Rewards benefit level and completed activities are located under the Benefits and Coverage tab. Click on View Wellness Activity. There may be a 30-day lag between the time you completed your Healthy Reward Activity and when you will see it listed on this website.
