On behalf of the senior leadership team, I am excited to share a few thoughts with you about a new and exciting Stryker program. As you know, People is one of our company s four core values.

Because of this, we re excited to announce that Stryker has a new wellbeing program, Stryker Strive, which will begin on July 1.

With Stryker Strive, you can improve your physical, emotional and financial wellbeing and more.

One of the ways we are committed to supporting this value is by providing you with the very best resources to help with your overall wellbeing. Because of this, we re excited to announce that Stryker has a new wellbeing program, Stryker Strive, which will begin on July 1. Improving our wellbeing and staying healthy helps us all to be more productive and happy both at work and at home. With Stryker Strive, you can improve your physical, emotional and financial wellbeing and more. Along the way, we want you to have fun, connect with other Stryker employees and meet your personal goals. In this newsletter, you ll learn all about the different programs, opportunities and incentives that Stryker Strive provides. We hope that what you find here will inspire you to live your best life. 3 What s different about Stryker Strive We know you re committed to being your best and our new program aims to help you do just that. You can now: > Join activities on demand > Track your fitness on the go by syncing a fitness app or device > See personalized activities and recommendations after taking your Stryker Strive Wellbeing Assessment > Achieve your wellbeing goals with IGNITE YOUR LIFE coaching > Compete against other departments and divisions (team activities coming soon) > Earn points and enhanced rewards as you move through the program Previous Wellness credits and assessment Don t worry your Wellness Assessment completion and credits from Live Well with Stryker automatically carry over to Stryker Strive. When you login to the new platform, you ll see points for your completed Live Well with Stryker activities. Please Note: The date displayed in My Points > Points History will show that you completed the Live Well with Stryker Wellness Assessment on June 30, 2015, even if you took it on a different date. Stryker has recorded the actual date for your 2016 Wellness Credit! No action required on your part. We recommend you print your Live Well with Stryker Wellness Assessment results before June 30, 2015, if you would like to keep them for your records. We will credit you for your completed Wellness Assessment, but we won t transfer your results or keep them on file once the new program launches. Action: Print Live Well with Stryker Wellness Assessment results before June 30, We encourage you to take the new Stryker Strive Wellbeing Assessment, even if you ve already completed the previous Live Well with Stryker Wellness Assessment. Your results will help you understand your overall health and wellbeing, and the platform will highlight your top strengths and opportunities for improvement so you can choose activities that make the most sense for you. As always, your data is completely confidential. Your individual results are never shared with others, including Stryker. Wellbeing Assessment deadlines You will receive a $600** wellness credit in 2016 if you elect 2016 medical coverage and you and your covered spouse/declared domestic partner complete the wellbeing assessment by your deadline. You will receive $50* if you do not participate in the 2016 medical plan. If you were hired or added as a new spouse/declared domestic partner: Your deadline to complete the Wellbeing Assessment is: Before January 1, 2015 March 31, 2015 Between January 1, 2015 and July 31, 2015 August 31, 2015 On or after August 1, 2015 You will automatically receive the 2016 Wellness Credit *All cash rewards must be treated as taxable income. You must be employed on the incentive date in order to receive the incentive. **The Wellness Credit for Hawaii employees with Employee Only coverage is $25 per month in 2016.

4 Stryker Strive coaching program IGNITE YOUR LIFE IGNITE YOUR LIFE coaches challenge you to get real, get rid of your excuses and take action! Coaching programs include Change Your Habits/Change Your Life, Address the Stress, Weight Loss and Mission Nutrition. Nip it in the Butt The Quit For Life Program has been replaced with Nip It In the Butt starting July 1. This program will help tobacco users become tobacco-free by reshaping habits. Your confidential sessions give you the tools to discover your strengths, break down your barriers and move toward lifelong change. Starting July 1, you can reach out to a wellbeing coach at or participate in free informational webinars. Stryker Strive Program Levels: Aim (100 points) = $600** wellness credit or $50* cash payout Aspire (200 points) = Entry for $1,000* drawing (deadline 8/31) Accelerate (300 points) = $150* cash (deadline 12/3) Achieve (400 points) = Two entries for $1,000* drawing (deadlines 10/31 and 11/30) You get credit for points you have already earned!
