1 (Enter name of prospect or project) VENDOR CHECKLIST I. SECURITY (enter name of rig and list security provisions for going on board) The RIG NAME security plan and procedures require positive identification, and prior notification to board the facility. Vendors are advised to notify the OIM on the rig by FAX at least 24 hours prior to their personnel going offshore. The notice must be on company letterhead and provide the following information for each person going aboard the rig/vessel: Name and job title Expected duration of time aboard the rig/vessel Date and expected time of arrival aboard the rig/vessel Personnel are required to have a picture ID, (valid driver s license, or passport, or other form of positive identification). II. PERSONNEL A. Drugs, alcohol and contraband are prohibited onboard Apache leases and contracted facilities. Random testing may be conducted during the course of the job. Refusal to participate in these tests will result in the immediate removal from location. B. All personnel reporting for transportation to the offshore location will be fit for duty and have, as a minimum, the following Personal Protective Equipment (PPE): (1) An ANSI- approved hard hat (non-metallic). (2) Steel-toed safety shoes or boots meeting the ANSI Z41.1 Specifications. (Employees going offshore should also bring along appropriate casual footwear to be worn in the drilling rig galley and quarters). (3) Safety glasses with side shields. (4) Appropriate work clothes. All personnel will be required to wear Fire Resistant Clothing (FRC) on a drilling location after the spud date and at all times thereafter until the well is completed and shut in or plugged and abandoned. (5) Hair guards (nets or caps) will be worn by persons with long hair to avoid being caught in machinery. Beards are not allowed where the job requires wearing respirators. (6) Jewelry that presents safety hazards will not be worn while working. Examples of such jewelry include finger rings, earrings, earring studs, chains, medallions, metal watch straps, or any other item that conducts electricity or that could be caught in rotating equipment, or snagged and endanger the worker. Personnel whose job description involves well control or production safety systems operation, testing, maintenance, or repair are required to have documentation (certificates) in their possession to establish that they are current on required training. D. All personnel will report at the designated transit point at the specified times. The Apache dispatcher should be notified of any delays as soon as possible. 3 E. All personnel onboard the rig/vessel will abide by the safety procedures, emergency response plan, and specific practices as required by bridging agreements with Apache. Personnel will participate in all safety meetings and drills unless excused by the OIM, or the Apache Supervisor. F. All personnel shall report to the rig/vessel Safety Officer immediately upon arrival onboard to register onboard and receive a safety induction. Upon completion of the induction, each person will sign the induction form acknowledging their understanding of its contents. G. (enter name of rig/vessel owner) safety awareness program is used onboard to recognize and reinforce safe behavior as well as to correct unsafe behavior. All personnel are expected to learn about and participate in the program. Information can be reviewed onboard. H. Any questions regarding the above personnel requirements should be referred to the (enter name of rig/vessel owner) Safety Representative, or the Apache Supervisor. III. MATERIALS The shorebase dispatcher must be provided a list of incoming materials/equipment complete with number of lifts, type of container (basket, box, etc.), and weights and dimensions (attach to shipping ticket). All materials delivered to the shorebase for transportation offshore will comply with the following requirements: A. All materials shall be properly packaged. This packaging should provide adequate protection against the hazards expected during offshore transit. B. All materials will be properly marked where fragile, hazardous, or requiring special handling. C. All materials packaged shall be marked as either property of Apache or (enter name of rig/vessel owner). D. Material Safety Data Sheets (MSDS) must accompany all chemical products and hazardous materials. E. ALL MATERIAL OR MATERIAL PACKAGES SHALL HAVE THE WEIGHT CLEARLY STENCILED ON THE TOP OR SIDE OF THE PACKAGE. F. Certified slings shall be used. Certification means that a proof test was performed on the sling. All certified slings must comply with API RP 2D and have a manufacturer s label that includes working load limits, proof test certification number, length, diameter, and date of proof test. G. The rating on shackles must equal or exceed the rating on the slings. Shackles shall be Crosby type or equivalent and use a secondary securement (i.e., bolt & pin, safety wire, tie wrap). Loads that shall be pre-slung include the following: (1) Equipment over 5,000 lbs., or equipment longer than 6 feet, or equipment needing a four point hookup (2) All tubulars (casing, drill pipe, tubing, etc.) (3) Subs, stabilizers, and mills greater than 4 feet long (Subs, stabilizers, bits and mills shorter than 4 feet will be transported in a container, bolster, or basket provided by the supplier). I. All regulatory requirements pertaining to the safe handling of supplied materials will be complied with at all times. Vendors shall notify (enter name of rig/vessel owner) or Apache of any special handling requirements before shipment to the shorebase. Any questions regarding regulatory requirements should be discussed prior to shipment. J. All manifests shall be addressed (enter appropriate information as follows): Manifest to: Apache c/o Martin Terminal North th Street, Bldg. 2 Golden Meadows, LA Well Name: Deliver to: Apache c/o Martin Terminal North th Street, Building 2 Golden Meadows, LA IV. TRANSPORTATION A. Apache has contracted United Vision trucking company to provide a 24-hour logistics service to effectively coordinate and consolidate all trucking to and from the shore base. This transportation policy applies to all vendors, service, and contract companies. If Apache is to be billed by a vendor for transportation, we require that the vendor coordinate shipment through United Vision s 24-hour logistics department. Apache will not accept any third party trucking charges from vendors. B. Vendors, service and contract companies, who own and operate their own trucks and provide their own transportation are not obligated to use Venture. The policy applies only to third party transportation services where outside transportation companies are used. C. When shipment is required, call United Vision 24-hour Central Dispatching to arrange transportation. To produce cost savings, it is important that you give as much advance notice as possible to allow time to consolidate and combine loads. Page D-29-C-4

General Equipment Requirements (1) Equipment arriving at the shorebase will be inspected by the dispatcher for serviceability for the intended task, but inspection and acceptance by the dispatcher shall not relieve the vendor of any of its responsibilities or obligations under its agreements with (enter name or rig/vessel owner) or Apache. All equipment provided to the project will be fit for purpose. (2) Equipment furnished to the project shall be packaged, containerized, or unitized in such a way as to facilitate shipping and protect the equipment from damage. Loads shall be pre-slung if greater than 5,000 lbs., or more than 6 feet in height, or a four point hookup is required. (3) All vendors are encouraged to develop their own equipment checklist for inspection of equipment prior to transport to the rig. These checklists should be included with equipment delivered to the shorebase. B. All powered equipment shall be either diesel, electric, hydraulic, or pneumatically powered. No gasoline powered equipment will be allowed. C. All diesel powered equipment: (1) Shall meet all BSEE/USCG requirements for shutdowns, emissions, pollution containment, and certification. All certifications will accompany the unit to the shorebase and the rig. (2) The diesel tank will be properly secured and capped to prevent spillage. (3) The unit will have a pollution pan installed. The drain plugs on such pollution pan will be properly installed. (4) The unit will have a fire extinguisher installed and affixed to it. (5) The unit should be mounted on a self-contained skid capable of supporting the unit for transportation from the vendor to the drilling unit and return. (6) The skid should have certified lift points. Page D-29-C-5
6 (7) The weight of the unitized equipment and skid shall be clearly stenciled on the top or side of the unit. (8) All safety shutdowns should be inspected and checked by the vendors prior to shipment. (9) All moving parts and hot surfaces should be properly guarded to prevent accidental contact. (10) Where equipment is intended for use in a hazardous area, it shall clearly have an approved hazardous area designation plate on the unit. The designation should be approved by a recognized standards body, such as UL, API, DNV, ABS, B.S.I., or ISO. (11) Operating instructions shall be provided with the unit. D. All electrically powered equipment will meet the following requirements: (1) When intended for use in a hazardous area the equipment shall be certified for hazardous area use with such certification provided with the equipment. The certification will be endorsed by a recognized certification authority such as UL, API, DNV, ABS, B.S.I., or ISO. (2) The equipment will be unitized to allow shipping and transportation to and from the rig. This unit shall have certified lifting arrangements affixed. (3) The arrangements for installation of the electrical supply shall be positioned to allow safe, convenient access by the rig's electricians. (4) Cable requirements must be defined with (enter name of rig/vessel owner)/apache prior to shipment or the unit. (5) All equipment will be properly grounded. (6) No equipment will be commissioned without prior approval of (enter name of rig/vessel owner) onboard electrician. (7) All moving parts will be properly guarded. (8) Operating instructions shall be provided with the equipment. (9) All equipment shall have clearly marked Start/Stop switches. E. Hydraulically powered equipment will comply with the following requirements: (1) Equipment will be inspected to insure that all fluid systems are sealed and fittings and hoses are in serviceable condition without leaks. 7 (2) All hoses and fittings should be free of defects and rated to an acceptable pressure rating above the required operating pressures (e.g., 3000 psi rating for 2500 psi operating system; 5000 psi rating for 4000 psi operating system). (3) Operating instructions with defined operating pressures will accompany the equipment. (4) Hydraulic power units will have pressure relief systems installed. These systems will be plumbed to contain released fluids and return to a reservoir. (5) Pollution pans will be installed on all units that contain hydrocarbon based fluids. (6) Where the hydraulic power source is remotely located from the equipment it operates, due care will be given to routing of the supply lines. The routing will be marked to indicate the lines containing pressured fluid. Areas of high usage by personnel or in the way of crane loads will be avoided if possible. (7) These units will be packaged or unitized in such a manner as to allow the safe, pollution free transport to and from the rig/vessel. (8) No equipment should be commissioned without prior approval of (enter name of rig/vessel owner) onboard Safety Representative. (9) All moving parts will be properly guarded. (10) The weight of the unitized equipment should be stenciled on the top or side. F. All pneumatically powered equipment will meet the following requirements: (1) Before pneumatically powered equipment is sent to the rig/vessel, the vendor should ascertain that there is sufficient air pressure available to power the unit onboard or that an auxiliary air source is available. (2) All pneumatic tools should be inspected, tested, and lubricated prior to transport to shorebase. (3) Units should be packaged such as to facilitate handling and to protect the equipment being transported from damage. (4) Lubricators and air receivers that contain hydrocarbon-based lubricants should be sealed and protected from shipping damage. Spillage of these lubricants shall be avoided. (5) Pneumatic power lines must be secured at the power source and the equipment so as to minimize accidental disconnection or rupture of the lines. Crows-foot connections must have the safety key or pin installed before air is turned on. Page D-29-C-7
8 (6) Operating instructions with defined pressure and volume requirements must accompany the equipment. (7) Systems that require greater than 125 psi supply must provide their own power source. These high pressure systems will require special care to insure that they are safely installed and used. (8) All moving parts will be properly guarded. (9) All personnel operating reciprocating or rotating equipment will wear the required personal protective equipment as specified in the (enter name of rig/vessel owner) Safety Policy Manual. 9 VI. INVOICE / BILLING PROCEDURES Please forward this section to your accounting department. A. All invoices must be invoiced to Apache Deepwater LLC for payment. Note: Invoices addressed to Apache Corp or other Apache business units will be returned to the vendor for correction. B. All invoices must have original Delivery/Time Tickets signed by authorized Apache Deepwater LLC representative attached. Note: No invoice will be accepted for payment without an original signed delivery/time ticket attached. C.
