40 April 30 An employee applying insulation in an attic received an electric shock and then fell. The incident occurred on April 24 th. May 2 An employee at a livestock sales business was crushed by a front end loader on April 26 th and passed on May 2 nd in the hospital.

43 July 5 A pilot car driver was electrocuted while attempting to raise a high voltage line with a hot stick in order to clear a load under the line during transport. July 16 An HVAC worker was electrocuted when he contacted an energized part while repairing an air conditioner at a retail business
44 July 28 An employee cleaning a frac tank at a company yard received a fatal electrical shock from portable lighting fixture. Aug 17 A line came into contact with a 7200 volt power line while installing new service to a building on private property.

84 Confined spaces, such as sewers, manholes, crawl spaces and tanks, are not intended for continuous occupancy. They can be very difficult to exit and they can be deadly. Effective date of new rule is August 3, CFR 1926 Subpart AA
85 Here are a few of the requirements in the new rule that differ from those in the existing general industry rule: A "competent person" must conduct the initial jobsite evaluation in the final rule. Information exchange requirements in the final rule inform employers what discussions must be conducted, and when, during confined space entry.
86 . Air contaminant and engulfment hazard monitoring must be done continuously, as the technology is readily available for most hazards. For substances where continuous monitoring technology is not available, periodic monitoring is required. The construction rule explicitly requires employers to coordinate emergency services before workers enter the confined space. During "controlled atmosphere" entry, employers may isolate physical hazards rather than eliminate all of them (such as using lock-out/tag-out, blocking off access to struck-by hazards, etc.).
