In the How will the hazards and risks be controlled? column, select the hazard or risk and then work through the control levels using the Hierarchy of Control to select the most appropriate. 5. Brief each team member on this SWMS before commencing work. Ensure team knows that work is to immediately stop if the SWMS is not being followed. 6. Observe work being carried out. 8 IMPACTS ON RESIDENTS, OTHER AUTHORITIES AND BUSINESSES 1. Prior to entering resident s property, conduct a visual inspection of the customer premises to identify potential hazards such as dogs, waste material and debris. 3. If a resident or business owner becomes agitated or aggressive, do not confront or provoke. Seek advice from your Supervisor to determine the most appropriate course of action. Where the customer is threatening or becoming physically aggressive, remove yourself from the site immediately. 5. When working in construction zones ensure that an appropriate Vehicle Movement Plan (VMP) is incorporated to ensure all workers are aware of plant, people and vehicle movements. Positive communications must be maintained and a spotter or third party traffic control may be required to direct traffic. All workers onsite need to be informed of construction activities and possible risks that may impact on them. 6. Workers are not permitted to work in contaminated soil areas without the permission of the land owner and all suitable precautions being undertaken. 9 1 Site Planning and setup WORKING IN THE RAIL CORRIDOR Access and egress Being struck by trains 1. Workers are not permitted to enter any Rail Authority property without permission of that Rail Authority. 2. Only designated entry and exit areas authorised by the Rail authority are to be used. 3. All workers in the rail corridor MUST have RSW rail safety card. 4. Positive UHF communications must be maintained at all times. 5. An exclusion zone from the rail line must be maintained. 6. A qualified Rail Protection Officer (PO) WMP15 must be onsite. 7. Workers are to abide by the instructions of the PO including any Vehicle Movement Plans (VMP) and Rail Management Plan (RMP), Protection Officer 8. Any incidents in the Rail corridor must be reported to MTA and the Rail Authority immediately. 1. Only Accredited Locators are to use electronic location devices 2. Dial Before You Dig Plans (DBYD) must be obtained prior to any soil penetration. This can be either , print or iphone App. 3. Verify that the DBYD Plan is in-date and matches the work site. 2 Electronic locating of Services USE OF ELECTRONIC LOCATOR Unable to locate services 4. Undertake a visual inspection of the work site to identify potential underground asset locations, depths & alignment (e.g. pot-holing, use of plant location devices, Visual assessment of site and possible routes of services, where is gas/power/water/sewer/telco) 5. Using the MTA Service Locations Checklist (MTA-SERV-001), mark out services using marker paint. Mark the depth and what service has been highlighted. Cable locator 6. If required, adjust the Traffic and Pedestrian Management setup to cater for the proposed excavation works. 7. If the proposed digging technique is mechanical plant, ensure that all underground services are carefully pot-holed to visually identify their depth & alignment. 8. 10 3 Transport TRANSPORTING EXCAVATOR / DRILL RIG AND MATERIALS Excavator / Drill and materials may fall off truck 1. Ramps must be secured to truck tray. 2. Truck needs to be parked correctly to avoid excavator rolling into traffic. 3. ONLY the competent operator can unload, load and operate excavators. 4. Secure excavator on truck or trailer when transporting using rated slings and chains. (Only rated chains, slings and ratchet dogs are to be used for securing loads, driver MUST inspect load prior to transportation as per RMS Load Restraints guide (second edition). Heavy Vehicle driver 5. All plant over 700kg MUST have ROPS). 4 Excavate and Drilling OPERATING EXCAVATOR AND DRILL Manual Handling Hearing impairment due to engine noise 1. Workers should conduct light stretching before work and rotate the work as much as possible with short rest breaks. Utilise two man lift if required, workers should not lift more than 25 kg each. Use correct manual handling techniques, knees bent back straight etc. * Refer to section 10 of the MTA safety folder. 2. Workers MUST wear appropriate PPE including hearing protection if operating near machinery over 85 decibels, safety footwear, gloves and long sleeve high visibility shirts and trousers. Incorrect use of machinery 3. 11 1. Dial Before You Dig (DBYD) plans must be investigated before work commences. 4 Excavate and Drilling OPERATING EXCAVATOR / DRILL Damage to underground & overhead services. Injury / Electrocution to Workers 2. Conduct electronic survey ( See section 2 of this SWMS) 3. Workers MUST wear long sleeve / long leg high visibility work wear, safety footwear and gloves including high voltage gloves when digging near live power. 4. Use Non-destructive Vacuum extraction or hand dig pot holes to visually verify services. Workers must hand dig with insulated tools within 300mm of low pressure / voltage assets and 1000mm for high pressure / Voltage assets. A minimum separation of 300mm will be maintained when crossing assets and 500mm when parallel. Excavation around Pressure / Voltage assets will require an onsite authority. 5. Workers must remain 3 metres away from 132kv overhead and 8 metres from 330kv and above. (Refer to Excavation COP NOHSC: 1016). Look at types of assets and the approach distances for powered excavators as per underground asset Guide (Work cover 2007). If assets cannot be located the asset owner shall be notified. 6. Excavators have clearance guide notes inside the operator s cabin. 7. Operators must be competent. 8. Directional Drill strike alert MUST be activated and tested before drilling. 9. 12 4 Excavate and Drilling OPERATING EXCAVATOR / DRILL Environmental damage due to water / sediment run-off, fuel spills and machine breakdown. Exposure to fuels, oils 1. Fuel and chemical spills are to be cleaned up immediately with the use of spill kits; hydro carbon sock and siltation mesh where required. 2. Material safety data sheets must be available for all chemicals onsite and be issued within the last 5 years. 3. All spills must be reported ASAP. 4. Machinery, vehicles and plant are to be serviced regularly as per manufacturer s recommendation. 5. Plant start-up checklists MUST be completed before starting machinery. 6. Lock out tags are to be used to isolate any plant or machinery that has been deemed unsafe. 7. WARNING Fuels are highly flammable and should be kept away from all ignition sources in a ventilated area. Refuelling should be done on level ground in a well-ventilated area away from any ignitions sources, water ways and drains. No smoking is allowed in any MTA vehicle. OPERATING EXCAVATOR / DRILL Workers and pedestrians being struck by excavator or Drill 1. Worksite MUST be delineated with barricades, signs (refer to section 1 of this SWMS) 2. Only competent operators to use excavators & directional drill degree flashing lights and reverse squawker must be operational on all excavators 4. A 3 metre exclusion zone around slewing excavator and drill must be maintained. 5. A spotter may be required for heavy trafficable or high risk areas. 13 OPERATING EXCAVATOR / DRILL Falling into open excavations, excavation collapse. Machine roll over 1. No excavation is to exceed 1.5 metres in depth and exceed 0.5 metres in width without shoring or battering.( Refer to National Code of Practice NOHSC:1016) 2. All excavations are to be reinstated with clean fill; any excavations that are to be left open overnight will require full barricades and MTA signage. MTA will assess the need for flashing warning lights. 3. Workers must continue to inspect ground conditions for stability. 4. All excavators and ride on directional drills are to be fitted with a Roll Over Protection System, ROPS. 5. Plant start-up sheets include the checking of ROPS and FOPS. 4 Excavate and Drilling OPERATING EXCAVATOR / DRILL 1. Conduct area assessment before excavating or trenching under trees and avoid excavations under trees if possible. Supervisor 2. Engage tree trimmer if necessary. Falling tree branches and other objects 3. Do not excavate close to large trees and avoid cutting roots where possible. 4. Excavators and ride on Directional drills working under trees or anywhere where there is a potential for falling objects must be fitted with a Falling Object Protection System FOPS. 14 4 Excavate and drilling OPERATING EXCAVATOR / DRILL Workers being crushed or injured by moving parts 1. A 3 metre exclusion zone is to be set up around drill. 2. Delineate work area. 3. Ensure that 360 degree warning lights are operational 4. Ensure that rod rack guard is in place. 5. Warning labels are to be displayed on drill as per plant risk assessment. 6. Workers are to wear long sleeved, long leg high visibility work wear and gloves. 7. A spotter may be required for high traffic areas. 8. Drill must be shut down before any maintenance or adjustments can be made. 9. Positive communication with operator and tracker must be maintained. OPERATING EXCAVATOR / DRILL Workers being injured by high pressure hose failure 1. Ensure that pressure is released before working on rods, hoses and fittings. 2. Ensure that high visibility work wear, footwear, gloves and eye protection worn. 3. Daily mechanical equipment hazard assessments (pre start checks) MUST be completed along with SWMA prior to work commencing. 4. pressure hydraulics within 500mm of operator must be guarded or wrapped with hose guard tape. 5. 15 OPERATING DRILL Exposure to dry Bore Gel 1. Bore Gel must be stored in a dry area. 2. A P2 type dust mask MUST be used when mixing dry Bore Gel. 3. MSDS for bore gel must be available onsite 1. Ensure that traffic control is in place where necessary. 5 Directional drilling OPERATING DRILL Tracking Drill sonde across trafficable areas 2. An observer may be required in high traffic areas. 3. Tracker must be aware of surroundings and plan route along with stops for recordings. 4. Positive communication with operator and tracker must be maintained. OPERATING DRILL Environmental hazards of waste water discharge 1. Utilise Vacuum (suck) truck to remove slurry from exit and entry holes. 2. Ensure that filtration fencing or hay bales are used in run off areas. 3. Hay Bales will be used to contain overflows and a spotter will monitor area whilst drilling. 4. Refer to site OHS&E Plan for special requirements. 5. MSDS required for all chemicals onsite. 6. Ensure spill kits and Hydro carbon socks are accessible and onsite. 7. 16 BUTT WELDING HDPE PIPE Electrocution from saw and welder 1. All electrical equipment must be electrically tested and tagged as per AS It is Mandatory to use RCD s when using electrical equipment. 3. Welders are NOT to be used in the rain or on wet flooded surfaces. 4. Do not use extension leads over 20 metres. 5. Worker must wear leather gloves when sawing HDPE Pipe. 6 Directional Drilling BUTT WELDING HDPE PIPE Uncoiled pipe striking workers. Manual handling of coiled pipe 1. Pipe must be secured and unwound to avoid springing, avoid cutting straps from outside the coil. 2. The use of pipe trailers is recommended for pipe transport. 3. Use mechanical aids to move pipe where possible. 4. Ensure that Welding unit is positioned for easy pipe entry. 5. Workers MUST wear leather glove when handling welder. BUTT WELDING HDPE PIPE Tripping or causing vehicle collision with Unwound HDPE pipe 1. Ensure that HDPE does not block driveways or vehicle access. 2. Delineate entire length of pipe if required. 3.
