2 MOLD SCREENING REPORT Section 1 General: Subject: Mold Screening Report Date: 5/01/2015 Time: 9:00 AM Location: 269 Water Street, Mt. Washington, KY Inspection Contact: Mark Mitchell (Director of Facilities) Inspector: Tony Ritenour, CMI (Council-certified Microbial Investigator) Inspector s Company Address: 3701 Taylorsville Road, Suite 1 Section 2 Method: A visual inspection of classrooms 214, 218, 308, 306, 305, and 303 was completed in the student occupied areas and above the ceilings. One air sample was collected in each classroom and two outdoor control samples. (note: This building indoor air quality survey was limited to the specific rooms listed above). Air sample medium: Micro 5 spore trap cassettes

3 Section 3 Findings: Surface mold was detected as follows: Location and list of all spore types detected on sample: No surface mold was observed in any student occupied areas in classrooms 214, 218, 308, 306, 305, and 303. Amplified indoor mold spore counts detected as follows: location and list of amplified spore types detected on sample. Amplification means higher than outdoor baseline or background levels (1-6 spores); unless inspector/analyst renders opinion to the contrary. No amplified airborne mold spores were detected in classrooms 214, 218, 308, 306, 305, and 303. Section 4 Discussion: This scope of work included the inspection of classrooms 214, 218, 308, 306, 305, and 303 where students would usually be occupying the space. This inspection excluded each room s small mechanical closet that are usually locked and under negative pressure. Previous mold air testing took place in room 307 and 304. Those results indicate those rooms did not contain amplified mold spore levels in the air. Many rooms contained some sort of water damage, whether it was drywall walls or ceiling tiles. Though water damage or stains were visible, there was no visible suspect mold growth observed on the finished side of drywall or either side of stained ceiling tiles. The roof was undergoing corrective repairs during this inspection. All walls were scanned with a thermal imaging camera (Testo 865) and cross checked with a moisture meter (GE Surveymaster Protimeter). All walls and ceiling tiles in the student occupied areas were dry.

4 Section 5 Conclusions/Recommendations: The following recommendations are made: It is recommended that the water damaged paint on drywall walls be properly prepped and painted. If new paint begins to fail due to water damage, then the roof repair in that area was not sufficient or there is another source of water intrusion. It is recommended that water stained ceiling tiles be replaced. If new ceiling tiles become water stained, then the roof repair in that area was not sufficient or there is another source of water intrusion. Since the roof was undergoing repairs and no visible or amplified mold spores were detected in the occupied portions of classrooms 214, 218, 308, 306, 305, and 303, then no further air testing of those rooms is recommended at this time. If conditions should change or leaks continue, then further testing may be needed.

5 Section 6 References: 1 - The American Society of Heating, Refrigerating, and Air Conditioning Engineers (ASHRAE) has determined that acceptable ranges of temperature and relative humidity for the summer and winter are: Relative Humidity Winter Temperature Summer Temperature 30 % 69.0 o F o F 76.0 o F o F 60 % 68.0 o F o F 74.0 o F o F These standards apply for persons clothed in typical summer and winter clothing, at light, mainly sedentary activity. Source: Adapted from the ANSI/ASHRAE Standard , Thermal Environmental Conditions forhuman Occupancy. 2 - ANSI/ASHRAE Standard , Ventilation for Acceptable Indoor Air Quality, suggests that carbon dioxide levels be maintained at or below 700 ppm above the outside level to keep bioeffluents (body odor) within an acceptable level. The OSHA Permissible Exposure Limit for carbon dioxide is 5000 ppm Threshold Limit Value (TLV) of the American Conference of Governmental Industrial Hygienists for carbon monoxide is 25 ppm. The OSHA Permissible Exposure Limit for carbon monoxide is 50 ppm. 4 - Investigating Fungal Contamination in Buildings, EMLab P & K, 2003, states that in general, indoor concentrations of fungi and bacteria should be lower than outdoor levels. 5 - In the OSHA Technical Manual, OSHA considers indoor surface samples of 1,000,000 CFU/in 2 or indoor air sample results of 1,000 CFU/m 3 as an indication of contamination. OSHA Technical Manual, Chapter 2.

Though some particulate levels between 0.5 um and 10 um were elevated when compared to outdoors, it is important to note that there was some foot traffic in the hallways as workers removed furniture from classroom 307 prior to the inspection. Air testing showed that no amplified mold spores were detected in the classrooms listed in the tables.
