1 The Office of Radiological Protection How to apply for a Licence online in EDEN 1. You will need to register with the EDEN Portal before applying for a licence. (a) Login at the following (b) If you are a new user you must Sign Up within EDEN: The EDEN Help Portal section provides additional information on getting started as illustrated below.

2 (c) During the sign up process you will be asked to select an application that you wish to access for access to your radiological licence please select the Authorisation Module (AM) Apply for a Licence or Authorisation option: 2. How do I apply for my radiological licence? The Apply For menu brings the user into the screen which allows you to Click on New Licence. In order to apply for a licence, you will need to submit information about the following: the type of business you are engaged in information relating to the radioactive sources and/or X-ray units you intend to acquire details of the locations where the sources/x-ray units will be used. You will be invited to submit this information as you work through the following tabs: Licence details Premise Personnel Inventory Documents Complete

3 You will also have to upload documents during the applicant process in support of your applications. At any stage of the application process you can SAVE your progress and return to complete your application at a later time. Tab 1. Licence Details: Please select the business area you are involved in: Dental Medical Veterinary Education and Research Industrial Once you have made your selection you will be asked to identify the activities you plan to carry out such as dental radiography, security screening or veterinary radiology. If you know the name of your dosimetry service provider, please enter it in this tab. Tab 2. Premise: Details of the address you entered when setting up your EDEN account will be displayed here. If this is not your primary contact address you will be able to note this and enter details of the address you want to use for correspondence. If you have additional premises where sources will be located, other than your primary or correspondence address, you should enter the details of these premises by using the + Add Premises option. Tab 3. Personnel: Please provide details of the following: Senior Management Contact: The individual with primary responsibility for the licence, e.g. dental surgeon, hospital manager, general manager or CEO. The licence will be sent to this person. Contact(s) for correspondence: The individual(s) who will look after maintaining the licence information. This person will be copied on all EPA licence correspondence, including new and amended licences. Radiation Protection Officer (RPO) 1. This person must be an employee within your organisation, assigned the day to day to day responsibility for radiation protection. Any amendments to the licence will be sent to the this person In the case of a dental or veterinary practice, the senior management contact, correspondence Contact and RPO, may often be the same individual. 1 please refer to guidance documentation on the personnel tab

4 Finally, you will also need to provide the name of your Radiation Protection Advisor (RPA) 2. A list of approved RPAs is provided at the hyperlink on this tab. This person will advise you on radiation safety issues and how to comply with the conditions of your licence and relevant radiation protection legislation. Note: currently veterinary licensees are not required to appoint an RPA Tab 4. Inventory: Please provide details of all radioactive sources and/or X-ray equipment you wish to obtain, including the reason for which you require this inventory. If you are not sure of all of the details as yet, please fill in as much as you can. Tab 5. Documents: For all types of practices the following documents are required: Plans of the facility indicating the location of all licensable items and room features (doors, windows, walls, adjacent rooms). Risk Assessment; see guidance notes link. In most cases this is provided by your RPA. Radiation Safety Procedures 3 If you are applying for a licence for either a sealed or unsealed radioactive source, the Guidance Document for New Applications on this tab provides details of further documentation you need to submit in support of your application. Tab 6. Complete: Finally click the submit button in order to make a declaration that you wish to apply for a licence. Your application will be reviewed by an inspector who may request further information or documentation.
