1 Individual Employment Plan/Individual Service Strategy (IEP/ISS) Overview The Individual Employment Plan (IEP) /Individual Service Strategy (ISS) is required by the Workforce Innovation and Opportunities Act (WIOA) for every individual registered into WIOA. The IEP/ISS is an ongoing strategy jointly developed by the participant and the career coach that identifies the participant's employment and educational goals, the appropriate achievement objectives, and the appropriate combination of services for the participant to achieve the employment and/or educational goals. The IEP/ISS is the central document that outlines the action steps needed to obtain the employment or educational goal. This document needs to be tailored to the individual customer and should not use a cookie cutter approach. The primary goal for adults/dislocated customers is unsubsidized employment. All adult/dislocated customers must have an Employment Goal. Any ITA or remedial training necessary to meet the employment goals should be included as objectives under the Employment Goal. An adult/dislocated worker may have an Educational Goal that is in addition to the Employment Goal. For example an adult drop-out who wants to become a truck driver (which does not require a diploma) and complete her GED. Her Employment Goal would be to gain employment as a truck drive. CDL Training and obtaining a CDL license would be objectives under the Employment Goal. She would also have an Educational Goal to obtain a GED. The goals for youth are unsubsidized employment and/or post-secondary education (which includes advanced training). Actions that should be addressed in the IEP/ISS include, but are not limited to: addressing barriers, remediation of barriers and skills deficiencies (as determined by the TABE scores under 9.0 in either reading or math and English Language proficiency), ex- offender status, etc. Note: ALL CUSTOMERS CARRYING OVER FROM IWDS THAT WILL BE ACTIVE INTO PY 17 MUST HAVE THEIR IEP/ISS CREATED IN CAREER CONNECT. PRINT THE IEP/ISS AND ATTACH TO THE ORIGINAL DOCUMENT. NO NEW SIGNATURES WILL BE REQUIRED UNLESS THERE IS A GOAL CHANGE. IF YOU ARE ENTERING THE IEP/ISS IN CAREER CONNECT, AND THE ORIGINAL IEP/ISS IS OVER 90 DAYS, BACKDATE THE CAREER CONNECT IEP/ISS AND ADD A CASE NOTE REGARDING THE DATE OF THE ORIGINAL (PAPER) IEP/ISS. AN INDIVIDUAL CAN ONLY HAVE ONE IEP/ISS OPEN AT A TIME. Requirements Before creating the IEP/ISS there are five steps that need to be completed. 1. The WIOA Application must be completed (and the customer must be eligible in at least one Title). 2. The WIOA Participation must be completed and the career coach must open the first service provided to the customer. 3. Customer should have completed career exploration and identified a clear and specific goal. 4. The customer s current skills should have been assessed through Basic Skills testing, review of credentials and an analysis of work history. IEP/ISS Procedure Page 1 of 10 Updated 12/13/2017

2 5. Open and close the IEP/ISS Same Day service (Staff Profile > Case Management Profile > Programs > WIOA > WIOA Activities/Enrollments/Services). The service is opened once you start the Plan. The service start and end date should match the IEP/ISS creation date. Navigation The IEP/ISS can be found under Staff profiles > Case Management profile > Plan > Create Individual Employment Plan/Service Strategy. Procedures When you click the Create Individual Employment Plan/Individual Service Strategy button, four tabs will appear: Plan, Goals, Objectives and Services. Completion of each tab is required to complete the IEP/ISS. 1. The Plan Tab contains general information about the participant, program they are enrolled in and plan dates. Identifying Information: This section contains display-only information, including Plan ID Number, State ID, User ID, Name, Created By, Create On, Last Edited By, and Edited On fields. Currently Participating: This section displays the programs in which this individual is currently participating, along with application and participation dates. 3 Plan Information: From this screen, staff can enter or select information as follows: Complete required fields marked with a red asterisk (*). Enter the Plan Start Date in the field provided, using a MM/DD/YYYY format (i.e. 12/10/2003). You can also click on the calendar icon to select a date, or click the Today link to enter today's date. Select the LWIA- Chicago Cook Workforce Partnership and Office Location from the drop-down menu. If you are viewing the information for a previously-created IEP/ISS, you will not be able to edit the "Plan started" date, LWIA, or office location Leave plan closed date blank Click the When printing plan do you want to print services? check box if you want to print the list of services provided with the IEP/ISS plan. Click the Next button to save and continue. 2. The Goal Tab allows you to establish or edit goals for an IEP/ISS. The primary goal for Adult/Dislocated Worker customers is unsubsidized employment. ALL ADULT/DISLOCATED WORKERS MUST HAVE AN EMPLOYMENT GOAL. The goals for Youth are unsubsidized employment and/or post-secondary education (which includes advanced training). 4 Goal Information - This section allows you to enter information about the goal. Complete required fields marked with a red asterisk (*). Select a LWIA-Chicago Cook Workforce Partnership and Office that is managing the IEP/ISS. If the customer is co-enrolled in a non-wioa program (e.g. Opportunity Works) staff should also select the co-enrolled program. Select a Type of Goal: Employment for Adult/Dislocated Workers and Employment and/or Educational (Post- Secondary Education including Advanced Training) for Youth Enter the Term of the Goal: Long term, short term or intermediate Enter a Description of the Goal: The description of the goal would include details of the type of employment or education. Enter the Date Established using the MM/DD/YYYY format Enter the Estimated Completion Date using the MM/DD/YYYY format This date must be equal to or greater than the Date Established field entry. Make sure the Completion Status is Open (because you are creating the goal) Enter additional information in the Goal Details (Comments) textbox provided. (e.g. Client is interested in the food service industry. Client likes cooking and really wants to work in the food industry) Click the Save button IEP/ISS Procedure Page 4 of 10 Updated 12/13/2017

5 After you save the goal, the Goals screen will refresh with the added goal. Proceed to the Objectives tab by clicking Next or by clicking the Objectives tab. 3. The Objectives Tab allows you to add objectives associated with an IEP/ISS. You can select Pre- Defined Objectives or create a New Objective. A. Pre-Defined Objectives Objective Information: This section allows you to select pre-defined objectives from a list. (See Appendix A below for a list of the available pre-defined objectives.) Click the select pre-defined objectives link From this screen, enter or select information as follows: Click the check box for each objective you want to add to this individual s program goal. You can select multiple objectives. Type the Date Established using the MM/DD/YYYY format. Type the review date using the MM/DD/YYYY format. This date must be equal to or later than the Date Established field entry. Click Save when finished. IEP/ISS Procedure Page 5 of 10 Updated 12/13/2017

6 B. Creating a New Objective General Information - This section lists the User Name and Name of the individual you are currently assisting. Objective Information If the customer has objectives that are not included in the pre-defined objectives list, you can create a free-form objective by clicking the Add new objective link. Please note that established date cannot be earlier than today s date. Complete required fields marked with a red asterisk (*). Select the associated Goal: Employment or Educational Enter the Objective Date Established Select a LWIA/Chicago Cook Workforce Partnership and Office that is managing the IEP/ISS. Select Program Affiliation: Workforce Innovation and Opportunity Act (WIOA) Program Type the specific Objective Type the Date the objective was established using the MM/DD/YYYY format. You may also click the calendar icon to select the date. Type the Review Date using the MM/DD/YYYY format. You may also click the calendar icon to select the date. This date must be equal to or greater than the Date Established field entry. Click Save when you are finished. IEP/ISS Procedure Page 6 of 10 Updated 12/13/2017

7 Note: The suitability for ITA Training should be documented in the Case Note section entitled Suitability for Training. The objectives required for ITA: Research LMI & Occupations Research Training Providers & Programs Submit ITA Approval Documents Attend Occupational Training Click next. 4. The Service Tab displays all services provided for the customer, including Universal (WP), WIOA and Generic (e.g. OPW). All services provided should relate back to the Objectives and Goals in the IEP/ISS. General Information - This section displays the User Name and Name of the individual you are currently assisting. IEP/ISS Services - This section lists the services associated with the IEP/ISS. Information displayed includes the App # - program, Service/Activity code, Begin Date, End Date, Provider, and the Staff who recorded the service. Click finish. 8 5. Printing the IEP/ISS: A completed IEP/ISS must be printed and signed by the participant and career coach and a copy must be placed into the participant s folder. The parent signature is not required for this document. Working with Existing Plans If an individual IEP record exists in the system, the screen that displays is shown below. Click the Edit link to review or modify the existing record. Review or modify an existing goal by accessing the Goal Tab. Enter the modification and/or objectives on the screen that displays and click Save to save changes. Review or modify an existing objective. Review services provided to the individual. Click Save to save all changes Click print to print the modified IEP/ISS. Note: The objectives and services should be closed as they occur. Closing the IEP/ISS To close the IEP/ISS, perform the following tasks. Before closing a Plan, you must close all Services, Objectives and Goals associated with the Plan. If you do not close the services, objectives and goals first, you will receive a warning message advising you to close. Enter a date to close the plan in the Plan closed field. Click the calendar icon to select the date or enter a date using the MM/DD/YYYY format. Click the Save button to save entered information. IEP/ISS Procedure Page 8 of 10 Updated 12/13/2017

9 Result A completed IEP/ISS is an agreement between both the participant and the career coach and becomes a living document that can be modified as needed. Goals, Objectives and Services must be updated throughout customer s active participation in the WIOA program. This document becomes the road map to helping the participant reach their goal.
