2 Why are we asking you to report near misses? You re probably concerned with penalties Where do these numbers go? Somewhere in Shell Global Sr. HSE Specialist Canada Logistics 2

3 Here s what it s really all about: Shell senior management needs to know your company is safe We re looking to see your safety culture We need to know you re identifying hazards We want to know everyone is thinking safety, and doing something to make sure no one gets hurt Eventually trending, identifying repeat issues What it s not about: Meeting minimum numbers, penalties, trying to cut your business Sr. HSE Specialist Canada Logistics 3

4 But what does that really mean? How can we know you re safe without having someone there every day watching? Reporting of interventions or stop work events Reporting of near misses Reporting of unsafe acts or conditions We need to know you re safe that you re looking out for each other Sr. HSE Specialist Canada Logistics 4

5 What is a near miss? Any time an incident happens, but there are no consequences You slip on the steps, but catch yourself with the handrail Someone cuts you off, but the collision is avoided Improper lifting technique used You see someone walk behind a truck backing up, but they run out of the way and there is no contact Sr. HSE Specialist Canada Logistics 5

6 What is an intervention or stop work? Any time you stop someone doing something unsafe, and get them doing it safely Stopping someone as they climb into the cab when they are not holding a handrail, and asking them to have 3 point contact Stopping a truck from backing up when you see they could hit something A stop work would be a situation where you totally stop the work going on because it s unsafe, until the situation is made safe You smell an abnormally strong odour of natural gas at the location where you are about to deliver fuel Sr. HSE Specialist Canada Logistics 6

7 What is Unsafe Act or Condition? Unsafe Act you see it but it s too late or not possible to intervene, no near miss occurred J-walking with no traffic around Not wearing required steel toe boots in the yard when required, even though there is no work going on or traffic in the yard A Potential dropped object is found on top of a Container that is being secured for travel Unsafe Condition Slippery walkway Truck driving with a headlight burnt out Load not secured properly identified before departure / driving Sr. HSE Specialist Canada Logistics 7

8 Discussion: What hazards are you not seeing? What hazards are you used to and don t notice any more? Vehicle Maintenance? Seatbelt usage? Fatigued driving? Worn straps? Sr. HSE Specialist Canada Logistics 8

9 What hazards are you used to and don t notice any more? Just because you re used to it it doesn t make it okay! Intervene it s better than watching the person get hurt and knowing you could have made a difference Correct the situation if you can and then report it If someone told you in the past or at another location that it can t be fixed report it again Sr. HSE Specialist Canada Logistics 9
