3 Introduction We are happy to take a step ahead in extending you the next level of Health and Wellness services through our new INTEGRATED HEALTH PORTAL (IHP). On the portal You can... - Document your medical reports and prescriptions online and access them from anywhere, anytime! - Chat with your Health Coach, who will guide you and provide personalised assistance for your health queries. - Get access to a treasure house of articles on health and wellness and set your favourites according to what s relevant for you. Keep yourself updated with the latest. - Your Health Tracker will help you track and review your diet, exercise plan, and health parameters over a period of time, once you upload them on the IHP. - For every medication you have to take, or any appointment you have with your doctor, the IHP alert will never let you forget any of them by prompting you regularly before time. - Avail exclusive offers & discounts on products & services. This manual guides you through the various sections of the IHP to help you understand the unique benefits designed for you to stay healthy...

4 Registration process Enter your member id (as shown on policy schedule) and any other two fields from Name/ id/policy no./date of Birth. Choose a nickname for yourself which will be your username.click Register Step 3 Confirm Contact Details Step 3 Confirm Contact Details Step 2 Click on Sign up and select Individual Tab Step 1 Login to Confirm and/or update your contact details. Click ok

5 Step 5 Check your id registered with Us for the one time password. Step 4 Registration Confirmation Login to access the IHP with the Username and password received on your registered id. Change your password and select 3 security questions. The answers to the security questions are case sensitive. Your new password and the replies to the selected security questions will be mailed on your registered id for your records

6 Home Page Welcome to your personalised Home Page. The Home Page displays the various activities that can be performed on the IHP such as the Health Risk Assessment, Design Wellness Plan etc. Click here to take the Health Risk Assessment as the first step in understanding your health status

7 Features on the Integrated Health Portal Health profile Health & Lifestyle Information Click to take the HRA to know your current health score and steps to improve your health. View Health Assessment Click to see your previous HRA Score Update Health Monitor- Click to update your diagnostic values of BP, Blood Sugar, Cholesterol, Weight & Water Intake Track Performance- Track your values updated in the health monitor. Wellness Plan Create Wellness Plan Post completing the HRA; create a wellness plan by setting your health goals for diet and exercise. View Wellness Plan View your health goals Track Daily Diet/Exercise-Graphical Tracking of your calorie intake and consumption Contact Health Coach- Write to our health coach for any health related query or assistance.

8 Click set medication alerts to set reminders for medication through s or sms. Click set appointment alerts to set reminders for appointment through or sms. Manage Records Set Reminders Click View Test Reports to see the reports uploaded till date. Your Pre Policy Check, Wellness Test 1& 2 reports will automatically get uploaded. Click upload test reports to upload your personal diagnostic reports

9 Click My favourites to make a repository of your favourite articles Click Content Center to read articles on health& lifestyle Click Health Calculators to compute your BMI etc Click useful links for shortcuts to Apollo Pharmacy, Apollo Clinic and Apollo Munich Click on Wellness DC list to check the closest diagnostic centre to your locality Market Place Knowledge Center On taskbar click on Market Place, select offer details This page displays the various offers available to you from our channel partners across product categories. To avail these offers, select the product category,state and City. All the three fields are compulsory. Press Search. The current offers in your city for the selected product category, will get displayed on the screen. Click on avail offer. The Offer coupon will be displayed with the merchant offering. Print the coupon and submit at the merchant location to avail the offer
