1 RFP No Vending Machine Services ADDENDUM No. 1 April 10, 2018 Any and all changes to the Request for Proposal are valid only if they are included by written addendum to all potential respondents, which will be ed prior to the proposal due date. Each respondent must acknowledge receipt of any addenda by indicating in its proposal. Each respondent, by acknowledging receipt of addenda, is responsible for the contents of the addenda and any changes to the bid therein. Failure to acknowledge receipt of ad addendum may cause the proposal to be rejected. If any language or figures contained in this addendum are in conflict with the original document, this addendum shall prevail. This addendum consists of the following: 1. Addendum No. 1 is attached and consists of a total of Eighty-Six (86) pages including this cover sheet. 2 RFP No Vending Machine Services Addendum No. 1 April 10, 2018 This addendum forms a part of the RFP Documents for RFP # and modifies these documents. This addendum consists of the following: Questions: 1. Question: Do you have sales for the last year and this year. Answer: Attached is the only vending sales information available for year No information is available for year Question: Are they broken down by location and machine? If not, can we receive the data. Answer: Attached is the only information on file regarding a breakdown of vending sales for year No information is available for Question: Are you planning to have site walk-throughs. Answer: There is not a planned walk through of the site locations. 4. Question: Do you have the day to day populations for all locations. Answer: The city does not track daily attendance at the locations. 5. Question: Are you open for equal type of equipment. Answer: Yes, the city would be open for equal type of equipment. 6. Question: Who is the current service provider. Answer: The Mark Vend Company is the current service provider for all locations. 7. Question: What are your revenue share expectations. Answer: The City is looking to maximize its profits from the best proposal. Page 2

3 8. Question: What are the expected weighted values for the criteria listed below. The evaluation criteria listed below will be used in the selection of the successful Proposer. A.
