5 Conference Program 2018 The 11 th Annual National Healthcare Supply Chain conference is scheduled for May 14-16, The conference includes two full days for the Conference Program and Trade Show (Tuesday, May 15 and Wednesday, May 16) and one Pre- Conference Professional Development Day on Monday, May 14, Both the Conference and Trade Show and the Pre- Conference Professional Development Day programs are designed specifically for the Canadian professional supply chain community. The Conference and Trade Show program scheduled for Tuesday, May 15 and Wednesday, May 16 is open to delegates and exhibitors and includes: keynote and motivational speakers; plenary and concurrent sessions; and panel discussions. Conference hours are as noted below and include continental breakfast, lunch and two networking coffee breaks and the evening networking reception hosted on Tuesday May The Trade Show is open Tuesday, 7:30 AM to 6:00 PM and Wednesday, 8:15 AM to 3:30 PM. The conference agenda provides dedicated time for delegates to network at the exhibit booths. All exhibitors are included in the Trade Show Passport that is received by all provider delegates to drive traffic to visit all 34 booths over the course of the two- days. All trade show staff are welcomed and encouraged to attend the conference program sessions. 27 Tradeshow Floor Plan Exhibit booths are reserved on a first- come first- served basis following the order of both sponsor and exhibitor confirmations. HSCN reserves the right to revise the trade show floor plan and will advise sponsors and exhibitors if a change is made and reassign booths in discussion with sponsors and exhibitors. Notes about the floor plan: Doors between to booths 7 and 8 are service doors only and not a point of entry or exit for attendees. All doors / areas marked ENTRANCE/EXIT (4) are primary access points for the attendees. The doors between booths 3 and 4 are available to attendees as points of entry and exit and will be in use as delegates attend concurrent sessions. 28

28 Policies, Terms and Conditions: Once you have confirmed your participation as a sponsor or exhibitor you will receive a confirmation from the conference management office (Aston Events). Following the confirmation , you will receive an invoice directly from the HSCN office. Please note that HST at 13% will be charged on all sponsor and exhibitor fees. Payments are to be submitted directly to HSCN. We will also ask you to confirm who will be the lead from your organization, with regard to sponsor/exhibitor logistics for the conference management office (Aston Events) to work. Aston Events will continue to be your primary contact leading up to the conference and on- site at the conference, and will coordinate with you and provide all necessary information and resources for your successful participation as a sponsor and or exhibitor. 1. Cancellation policy: Upon confirmation of participation as a sponsor or exhibitor, you will receive a confirmation from the conference management office. You will also receive the sponsor or exhibitor invoice directly from the HSCN office. This process confirms your company as a sponsor or exhibitor at the conference. Sponsorships are non- refundable. Exhibits cancelled on or before Monday, February 26, 2018 may be refunded at fifty percent (50%) of the total value of the exhibit fee. Exhibits cancelled after Monday, February 26, 2018 are non- refundable. 2. Payment of sponsor and exhibitor fees policy: Payment is due upon receipt of invoice from HSCN. Complimentary and discount registrations codes will be provided only after the HSCN office has received full payment for sponsor and exhibitor fees. Access to the conference and trade show will not be allowed unless full payment of your sponsor or exhibitor commitment has been received in full by HSCN. 3. Sponsor and exhibitor delegate registration process and timelines: Conference registration opens January Upon confirmation of receipt of payment by HSCN, Aston Events will provide sponsors and exhibitors with directions on how to register your complimentary and/or discount registrations for the conference. Completing the on- line registration process for each of your delegates (including complimentary and discounted registration) is your responsibility. Early Bird options are available the deadline for registration for all conference delegates is Friday April 20, Sponsor and exhibitor registration policy: Sponsor and exhibitor registration rates apply to sponsor and exhibitor companies only. Companies recognized as sponsors and exhibitors are those companies that have purchased a sponsorship or an exhibit booth. No other company or affiliate will be recognized as a sponsor or exhibitor or be entitled to the rates received by sponsors and exhibitors. Sponsor and exhibitor complimentary and discounted registrations are a benefit of sponsorship/exhibit participation and cannot be transferred. Should a sponsor / exhibitor registration be found to be used by another company HSCN reserves the right to cancel that registration/s. HSCN Non- Members from sponsoring or exhibiting companies may be eligible to register for the conference at the HSCN Member Not Exhibiting or Sponsoring registration rate. For more details on HSCN membership please visit the HSCN website. 5. Booth sharing policy: Sponsor and exhibitor booths are for the exclusive use of the sponsor or exhibitor company only. Should another company/affiliate be found to be part of the activity and or display at a booth, HSCN reserves the right to ask that company and its representatives to leave the show floor. 29

29 6. Logo and company profile submissions: Company logos and profiles are due to the conference management office immediately upon confirmation as a sponsor or exhibitor. Logos and company profiles will be posted within the sponsor and exhibitor recognition section of the conference website as well as the conference App. Company profiles are to be a maximum of 50 words. Profiles over 50 words will be returned for editing. For quality purposes, company logos are to be high- resolution jpeg or eps file format. HSCN makes no guarantee that materials received after Friday March 16, 2018 will be included in on- site print materials at the conference or conference app. 7. Delegate bag literature guidelines and deadlines: Literature insert options for the delegate bags are limited to a maximum of one 8 ½ by 11 sheet. The sheet can be full colour and double- sided. Companies submitting insert options are responsible for direct delivery of materials to the Delta Airport Hotel & Conference Centre (655 Dixon Road, Toronto). Delivery date is Friday May 11, The conference management office will provide delivery instructions and shipping label. 8. Booth selection process: Exhibit booths are reserved on a first- come first- served basis following the order of both sponsor and exhibitor confirmations. Once you have confirmed your participation as a sponsor or exhibitor the conference management (Aston Events) will advise which booths are available and will help you select your booth. 9. Insurance: All exhibitors must provide proof of insurance (general liability) in the amount of $2,000, to HSCN no later than Friday May 4, Each exhibitor shall assume responsibility for all damage to the exhibit materials and exhibit area and that they shall indemnify and hold harmless the HSCN their employees, officers and agents and any service providers acting as agents for said company, from all liability which may ensure from any cause whatsoever. Directions will be provided within the Exhibitor Manual 10. Exhibitor logistics and show services: As conference management, Aston Events will assist with the coordination of exhibitor logistics and provide a complete exhibit manual and show services guide with all the necessary details and information you will need. The exhibitor manual will be available in February 2018, and distributed after that date according to receipt of your sponsor or exhibitor payment to HSCN. 11. Exhibit booth details Each exhibitor and exhibiting sponsor receives one 10 wide by 8 deep exhibit booth with complimentary pipe and drape (black). Exhibitors and exhibiting sponsors are responsible to order directly and pay for their booth power, hard- wired/dedicated internet access and booth accessories such as chairs, tables, etc. The conference management office (Aston Events) will provide you with a complete Exhibitor Manual including contact information for these additional services. HSCN and Aston Events are not responsible for any such costs as incurred by exhibitors and exhibiting sponsors. 12. Booth Footprint, Access and Line of Sight: For safety reasons, exhibitors must ensure their exhibits stay within the 10 wide by 8 deep exhibit space. Exhibitors are also asked to respect fellow exhibitors and ensure their display does not block, obscure access to/or view of any another booth/s. Exhibitors not respecting these guidelines will be directed to re- set their display. Exhibits that do not comply will be removed from the show floor at the discretion of HSCN and Aston Events. 13. Exhibit Hours: The trade show is open for the two full days of the conference. For safety reasons, exhibitors are not permitted to remove their booth until the formal close of the conference and ALL delegates have cleared the conference and trade show floor areas on Wednesday May

Booth prizes and draws: Companies providing booth prizes and draws at the show as part of the booth activities are directly responsible to award their prizes and draws and to give the prize items directly to their selected winners. HSCN will not announce or hand out an exhibitor company prize or draw. Nor will HSCN make arrangements to deliver a prize on behalf of the exhibitor. Companies are to make their arrangements to deliver any booth prize or draw items. 15. Nuisance: HSCN reserves the general right to restrict exhibits which, because of noise or other reason become, in show management s opinion, objectionable, and to close without indemnity, any exhibit or his employee or agent who shall refuse after notice, to conform to the regular rules which apply to all exhibitors. Specifically, no amplified sound, except when directly related to product demonstration, is permitted and then only at levels that do not exceed normal conversation levels as adjudged by show management. 16. Acceptance: HSCN reserves the right to accept or reject any application for exhibit space without stating a cause. Exhibitors may not sublet exhibit space. Exhibit space cannot be used to display any items for which the exhibitor is not a representative. HSCN reserves the final decision on acceptability of display materials and exhibitor content. All exhibits must comply with the host property exhibitor rules and regulations that will be provided with the Exhibitor Logistics Manual.
