2 March 9, 2016 ADDENDUM NO. 2 Solicitation HH-89 Design/Build for Rehabilitation of Skewbacks, Viaduct Piers and Lower Level North Abutment at the Henry Hudson Bridge To all Prospective Proposers: The due date to submit Expressions of Interest remains unchanged: March 29, 2016 Attached is the following additional or clarified information for the Project: 1. The presentation from the March 4, 2016 Pre-EOI Informational Meeting is attached. This presentation is being shared for general information and reference only. 2. A copy of the March 4, 2016 Pre-EOI Informational Meeting sign-in sheet is attached. 3. Closing date for Questions is revised to 3/11/16. Please submit your questions in writing to the Point of Contact by 3/11/16 so there is ample time for a complete response. Yes Will there be an Alternative Technical Concept (ATC) process in the RFP stage? Will be answered in a future addendum. Is it acceptable for a contractor to self-perform the geo technical construction provided that it can demonstrate the requisite experience and resources to perform the work? Yes. Does the Geotechnical Subcontractor as identified in the EOI have to be exclusive to a particular team? No.

Can the Contractor name alternate Geotechnical Subcontractors in order to retain a competitive field of bidders? Yes. Would the TBTA consider removing the naming of the Geotechnical Subcontractor until the RFP process when the scope of work is more clearly defined? No. Please confirm if subcontractors, sub designers or sub consultants of the Design Professional and the Construction Inspection Professional Engineering Firm are required to submit Standard Forms RFQ-4 (rev ) and SF-330. Confirmed except as noted below in A16 Is there a limit to the number of Design-Build firms that will be shortlisted from the evaluation of these Expressions Of Interest? If so, how many? No. May the documents that do not count towards the 60 page total, the RFQ-4 Form and Financial Statements, be submitted on a disk only or shall they be printed out as well? All submittals must be made as hard copies as well as in electronic format. Can the firm performing the Construction Inspection /QC services also provide the Structural Health Monitoring System? No. Can the letter from surety not be included in the page count as they can be multiple pages and vary in page length due to the surety attachments? The only exclusions from the 60-page limitation are the RFQ-4 forms and financial statements. It was stated at the 3/4 pre-eoi meeting that any sub consultants to the Design Professional or Construction Inspection Professional Engineering Firm that are included in the EOI would need to submit the appropriate forms as well (RFQ-4 and SF330). Would the SF330 be an individual SF330 for each subconsultant(s) or would a combined SF330 for the Professional firm and its sub(s) be acceptable? A combined SF-330 is acceptable as long as the company affiliations are clearly noted. Form CU-078 Page 2 (Rev. 1/4/99)

Is this limitation for the customized SOQ (Item 3.b.ii)? Is the Designer also able to cite the 10 project examples as per the requirements of the SF330 Part I Section F? Please refer to the instruction under Part I Section F which states: Present as many projects as requested by the agency, or 10 projects, if not specified. If subconsultant(s) are being included, are their project examples part of the Professional firm s limitations or can they submit five (5) as well. Additional sub firms may list their own most relevant project examples. Please remain within the stated page limitations. Please be governed accordingly in submitting your Expressions of Interest for this solicitation. 16 STRUCTURAL HEALTH MONITORING SYSTEM Design, install, and operate a structural health monitoring (SHM) system including sensors for: Strain measurement Displacement measurement Vibration measurement Load cell measurement Tilt monitoring Weather Station System shall monitor the performance of the new construction and existing bridge elements. System shall be web-based and modular to allow for future expansion. Design-Build Team will be required to maintain and operate a SHM System for up to 2 years following substantial completion of Construction. Monitoring reports shall be provided 12

25 QUALITY CONTROL / QUALITY ASSURANCE (QC/QA) QC/QA is Design-Build Team s Responsibility Design-Build Team shall appoint a QC/QA Manager responsible for Project QC/QA Construction Inspection shall be by an Independent Engineering Firm (Retained by the Design-Build Team) Materials Testing shall be by an Independent Testing Firm or Laboratory (Retained by the Design-Build Team) The Authority will perform audit and review QA on Design-Build Team s Design and Construction Activities. Additionally, the Authority will perform in-depth QA inspections of any possible environmental work. 21
