WELLNESS COACHES OUTCOMES REPORT SUMMARY Wellness Coaches, the largest national provider of onsite wellness coaches, is pleased to release our 2016 Book of Business Client Outcomes Report. Our report is unique in the workplace wellness space. Unlike many providers, we did not cherry pick our best results. We are not showing you the results only for those who participated or engaged. Our report reflects the aggregate outcomes our workplace delivery model has achieved for every employee working at every customer location we have ever serviced. Defining Workplace Wellness Success Employee Engagement Employee engagement is important, but it s not enough. While 86% of employees did work with our coaches, and we did coach them a lot, engagement is just a means to an end. We are just getting started! Our Report Covers 1200 employer locations in 35 states 295,000 eligible employee lives 86% or 254,000 employees coached 20 coaching interactions per employee Over five million total onsite coaching interactions High Quality Coaching It s also not enough to present outcomes relative to only those who actually participated or engaged. This is a common statistic touted by most providers. It s misleading and only part of the measure of success, as these stats only illustrate how effective the coaching has been for the employees that were coached. As you can see from the chart, our coaches are really good at helping people reduce risks. But it begs the question, what does this mean relative to my entire employee population? Before Coaching After Coaching 68% 66% 44% 34% 36% 29% 29% Stress Exercise 7% BMI Blood Pressure The Bottom Line The bottom line for us, and the real measure of workplace wellness success, is large-scale improvement in employee population health risks. Sure, high levels of engagement and high quality coaching are critical, but the fundamental issue is the number and percentage of people (in the facilities we serve) that our coaches helped improve health risks like lowering blood pressure, improving BMI, increasing exercise, or reducing stress. How Many People We Helped 103,000 EEs lowered BP 88,000 EEs reduced BMI 48,000 EEs increased exercise 18,000 EEs reduced stress 155,000 EEs, or 52% of the total population, improved one of just these four risks. No other provider comes close! Wellness Coaches wcusa.com 3

WELLNESS COACHES OUTCOMES REPORT SUMMARY Other Key Takeaways Connecting Everything One consistent theme we hear from employers when we are just getting started is that employee utilization of the other health and wellness resources they provide their employees is disappointingly low, and that these resources rarely produce their intended results. As part of our wellness coaching efforts, however, we utilize these resources as valuable tools in the health risk improvement process, bringing them to life and enhancing their value connecting, integrating, promoting and referring them all as part of our coaching process. More Than Onsite Coaching At no added cost, and as important components of our strategy and infrastructure, our coaches design, implement and coordinate a customized and regular array of other wellness programs and activities to further support and enhance both the employer s culture of well-being, and the employee population health risk improvement process. We also equip our coaches to provide Skype, phone and/or coaching when access to our onsite coaches is impractical. This opportunity is especially useful for remote employees and their families. Our Coaches Have Also Delivered: 877,000 blood pressure tests 655,000 BMI/weight measurements 10,000 s of programs and activities 59,000 remote coaching sessions 80,000 family coaching sessions Wellness Coaches wcusa.com 4

WELLNESS COACHES USA OUTCOMES REPORT SUMMARY Linking Wellness and Injury Prevention There is a profound link between employee healthfulness and injury prevention. It is well established that healthy, fit employees are safer workers, and vice-versa. In fact, research has demonstrated that workplace wellness is perhaps the most potent injury prevention strategy. Losing weight, exercising more, or simply being more mindful, can have a meaningful impact upon injury rates, and costs. That s why, for many of our customers, particularly in arduous work settings, we deliver a wellness and injury prevention service model which blends and integrates both a wellness and injury prevention focus. Injury Prevention We coached 102,000 employees on safety and injury prevention. Our model has produced an average 30% reduction in injuries and associated costs. Success Without Incentives There has been much debate over the years about how to effectively use incentives and in some cases, whether incentives work at all. Our outstanding outcomes demonstrate that truly impressive, largescale improvement in employee population health risks can successfully be achieved without the use of incentives. We believe we have proven that it is intrinsic motivation created from the genuine, caring and trusted personal connections our coaches have with employees that achieve lasting health risk improvements. No Incentives Wellness Coaches wcusa.com 5

2016 WELLNESS COACHES Unlike other service providers who publish select client outcomes, or in many cases virtually no meaningful outcomes at all, our 2016 Book of Business Outcomes Report includes the aggregate outcomes of every employee at every client location we have serviced since our inception. We believe this Outcomes Report clearly details our outstanding and industry leading employee population Engagement and Health Risk Improvement outcomes. These outstanding results demonstrate the unique value our onsite wellness coaching delivery methodology, when integrated with our client s existing wellness resources, can bring to workplace wellness helping our clients achieve a much higher level of workplace wellness success than otherwise possible. SECTION ONE Founded in 2002, Wellness Coaches is the largest and most experienced national provider of onsite wellness coaching. Our services are provided and implemented by the industry s largest staff of professional, highly trained onsite coaches. We have serviced over 1,000 client locations in 35 states throughout the U.S. and Puerto Rico. Our clients range from smaller, local and regional employers, to the largest employers in the world (from 50 lives to tens of thousands), and span a broad range of business classifications including distribution, oil and gas, healthcare, construction, hospitality, manufacturing, education, transportation, chemical, pharmaceutical, municipalities and financial services. We implement our highly effective onsite wellness coaching services through the deployment and placement of our wellness coaches directly in the workplace. In the workplace, our coaches engage and coach entire employee populations, one-on-one, face-to-face, regularly and frequently. For us, however, onsite coaching is about much more than simply placing coaches in the workplace. That s because you can t just drop someone in the workplace, call them a coach, and expect them to build the very high levels of engagement we stimulate, nor the exceptional health risk reduction outcomes we produce. That s why we not only place our coaches in the workplace, but also equip them with the wellness industry s most comprehensive onsite wellness coaching infrastructure, which includes: (i) our time tested management and operational protocols and processes, (ii) CoachWell, our coach support, training and development infrastructure and (iii) StageCoach, our proprietary coaching software and reporting system. All of these components have been developed by us to specifically satisfy the highly specialized requirements of onsite coaching. It is this finely tuned infrastructure that allows us to deliver the high quality service and unmatched outcomes our customers are accustomed to, and which truly distinguish us from other vendors who merely say they do onsite coaching. 7

2016 CONNECTING COMPLEMENTING INTEGRATING Our Wellness Coaches help make everything an employer offers better! SECTION ONE COACHING REFERRALS During our coaching sessions, our coaches made 187,000 employee referrals to 3 rd party programs, tools and resources made available to employees by our clients. Some examples include web portals, HRAs, blood testing, incentive programs, clinics, DM, Weight Watchers, EAPs, etc. Our onsite Wellness Coaches visit all client locations on a systematically planned, regularly scheduled and routine basis to meet with and help employees improve their well-being. Our coaches work with employees, each according to their own needs and interests, and at their own pace, to pave the path towards a healthier tomorrow. As part of our coaching process, coaches also integrate, regularly promote and refer employees, when appropriate, to all of the other health management resources and programs which comprise our client s workplace wellness strategy. In addition, during their time onsite, our coaches will design, implement and coordinate a customized, regular and continuous array of wellness/safety programs and activities to further support and enhance a culture of well-being. In essence, our coaches become our client s boots on the ground for all things wellness. 8

SECTION ONE 2016 MUCH MORE THAN ONSITE COACHING AT NO ADDITIONAL COST 1,500,000 BIOMETRIC TESTS PERFORMED Our coaches have delivered 877,000 blood pressure tests, and 655,000 BMI-weight measurements. 59,000 REMOTE COACHING SESSIONS While most of our coaching interactions are performed oneon-one and face-to-face, our coaches supplemented their onsite coaching interactions with nearly 59,000 phone, and/or Skype coaching sessions. 10,000 s OF CUSTOMIZABLE PROGRAMS As part of our strategy and infrastructure, our coaches have designed, implemented and coordinated 10,000 s of customized wellness programs and activities to further support and enhance our clients cultures of well-being. 80,000 FAMILY COACHING SESSIONS In addition to coaching employees onsite, our coaches also had over 80,000 coaching interactions with spouses and dependents. This clearly demonstrates that not only do we coach a lot of employees, but we are also equally as adept and successful at helping family members. 9

SECTION TWO 2016 REPORT PARAMETERS AND METHODOLOGY The data presented in this study was collected by StageCoach, our proprietary coaching software specifically designed, developed and utilized to meet the data requirements of onsite wellness coaching. Health Risks Measured The health risk improvement data provided in Section Two, and the stratification of such data in Section 5, includes improvement in the five common behavioral and biometric health risks we routinely measure in StageCoach. These risks include Exercise, Body Composition (BMI/Weight/Body Fat), Tobacco Use, Blood Pressure and Stress (collectively, Health Risks). No other 3 rd party data is included. Though our wellness coaches conducted an average of 20 coaching interactions, on multiple wellness issues for each employee coached per year, the study includes results for all employees who had at least two coaching interactions for one or more of the above five Health Risks. Measurement Period The results presented are based upon a comparison of measurements taken at the time we began coaching each employee included in the report ( Baseline ) with their most recent or last recorded coaching session ( Most Recent ). It should be noted that, as most employees will continue coaching into the future (often on a regular basis for years to come), Most Recent or last coaching session does not suggest that employees have completed coaching. That s because it is critically important to not just help people make improvements, but to also help them sustain their improvements over time, and to progress to improving other risks which also impact their well-being. Study Sample This report includes ALL client locations that have utilized our onsite coaching services. There were over 1,200 of such locations (in 35+ states and Puerto Rico), with a total of 295,000 employees, all of whom were eligible for coaching. The average client location had our coaching services in place for 4.5 years. Our results clearly demonstrate that not only does our coaching service consistently generate a very high level of employee engagement, but also that our services drive substantial, lasting and growing population health risk improvements (behavioral and biometric). 10

SECTION THREE 2016 SECTION THREE SUMMARY Employee Engagement Data The charts in Section Three set forth data regarding the: (1) very high number of employees who work with our coaches, (2) large number of coaching interactions each receive, and (3) wide variety of coaching issues addressed. In the aggregate, these three metrics drive our exceptional levels of improvement in Health Risks. That s because, first, workplace wellness success requires that a large majority of any employee population must engage in Wellness Coach-supported efforts to improve well-being. Chart 1 shows our exceptionally high coaching engagement levels. Second, it takes more than just some wellness coaching. Rather, coaching success requires enough coaching to provide the consistent and regular support people need, for the extended period of time necessary, to achieve and sustain improvement. Chart 2 shows the high number of coaching interactions each coached employee receives. Finally, as overall health and well-being is a function of the interaction, interdependence and association between multiple behavioral and biometric Health Risks, our wellness coaching regimes include a focus on addressing as many of such risks and related issues as possible. Chart 3 shows the comprehensive scope of the issues and risks that our coaches routinely address. 11

2016 ELIGIBLE EMPLOYEE POPULATION COACHED The average ratio was 1 coach per 800 employees Since our inception, we ve coached an average of 86% of all employees! We ve coached 254,000 of the 295,000 eligible employees! Employees have had over 5.2 million one-on-one coaching interactions! SECTION THREE 85% 87% 84% 87% 88% Eligible Population represents all employees located in the 1,200 locations for which we provided coaching services. % of Eligible Population Coached refers to the percentage of the eligible population that engaged in coaching each year. % of Eligible Population Coached CHART 1 12

2016 AVERAGE NUMBER OF ONE-ON-ONE COACHING INTERACTIONS We ve averaged 20 interactions per employee coached since our inception! SECTION THREE 46% 11% 25% 18% Interactions per Employee per year Of the approximately 86% of employees coached each year, the above categories represent the percentage of employees who engaged in 1-3, 4-6, 7-9, and 10+ one-on-one and face-to-face wellness coaching interactions on average per year. Research demonstrates that only a few coaching interactions each year is not sufficient to achieve and sustain improvement in Health Risks; and that the more coaching interactions the greater the level of improvement. That s why, in addition to coaching the vast majority of the populations we serve, we coach them often and frequently, averaging approximately 20 interactions per employee coached. CHART 2 13

2016 HEALTH / HEALTH / WELLNESS / SAFETY WELLNESS ISSUES / ADDRESSED SAFETY ISSUES 75% of employees were coached on four or more issues! Top 10 Health Issues: Blood Pressure Body Composition Nutrition Exercise Weight Loss Stress Cholesterol Sleep Tobacco Use Diabetes Over 100,000 employees were coached on safety and injury prevention. Top 5 Safety Issues: Back Knee Shoulder Elbow Wrist SECTION THREE 75% 10% 8% 7% Issues per EE or more The categories above represent the Number of Issues each employee discussed with their coach. The previous two charts demonstrated that we interact frequently with a very high percentage of employees. However, as achieving overall health and well-being is a function of the interdependence and association between multiple behavioral and biometric health risks, our coaching regimes also focus on addressing as many of such risks as possible. That s why 75% of employees coached were coached on 4 or more issues of interest, and relevance to their lives. CHART 3 14

SECTION FOUR 2016 SECTION FOUR SUMMARY Health Risk Improvements Section Four includes data for all employees coached two or more times on the five Health Risks we routinely collect with our StageCoach coaching software: Blood Pressure, Body Composition, Exercise, Stress and Tobacco Use. The results presented (as to each Health Risk for which an employee was coached) are based upon a comparison of measurements taken at the time we began coaching each employee included in the report ( Baseline ) with their most recent or last recorded coaching session ( Most Recent ). It should be noted that, as most employees will continue coaching into the future (often on a regular basis for years to come), Most Recent or last coaching session does not suggest that employees have completed coaching. Charts 4 to 6 demonstrate that coaching the large majority of employee populations on a routine basis year after year leads to substantial improvement in employee population Health Risks. 15

2016 NUMBER OF TOTAL HEALTH RISKS IMPROVED 155,000 employees or 52% of the total population improved one or more health risks. 261,000 Health Risks improved for the population! 73% of Health Risks improved were for Blood Pressure and/or BMI! SECTION FOUR Quit Tobacco Increased Exercise Reduced Stress Quit Tobacco Lowered BP Lowered BMI Chart shows number of employees who have improved each of the 5 health risks tracked in StageCoach. CHART 4 16

SECTION FOUR 2016 HEALTH RISKS IMPROVED PER EMPLOYEE COACHED 50% of employees improved two or more health risks! 18% 32% Employees coached 10+ times improved >2 risks 50% The more coaching, the better the results! Number of Risks Improved per EE Coached Improved 1 risk Improved 2 risks Improved 3+ risks Number of Risks Improved Per EE Coached CHART 5 Number of Coaching Interactions CHART 6 17

SECTION FIVE 2016 SECTION FIVE SUMMARY Population Prevalence Rates The charts in Section Five stratify the health risk improvement data presented in Section Four to provide a more conventional look at the changes in population prevelence rates for BMI, Blood Pressure, Exercise and Stress Risk Levels (for all employees coached). To be included in this analysis, an employee must have had at least two coaching interactions on one or more of the four Health Risk factors listed above. The percentages shown on each chart reflect the percentage of employees who presented in the various risk categories as of their Baseline measurement, and then as of their Most Recent coaching interaction. Baseline represents measurements at the time when employees first engaged in coaching, and Most Recent represents measurements as of their most recent coaching session. It should be noted that even after their most recent coaching session, most employees will continue to be coached for many years to come. The charts in Section Five (charts 7 to 10) demonstrate that the substantial and sustained personal Health Risk improvements shown in Section Four convert to outstanding improvement in the health risk prevalence rates for high risk employees. 18

2016 BMI IMPROVEMENTS 100,000 employees were measured 655,000 times! 88% of employees improved BMI Obese population ( 30 BMI) dropped 17%! Low Risk BMI population improved 29%! 23 SECTION SIX ,000 LIFE CLIENT METABOLIC SYNDROME OUTCOMES The following outcomes were independently measured and provided to us by a third party consultant for a large 15,000 life client. We currently provide 13 coaches to this client. The analysis was designed to measure the impact of onsite coaching on Metabolic Syndrome. The results show that the aggregate employee population prevalence rate for employees with Metabolic Syndrome has declined 30% since onsite coaches were implemented. Coaches started Year 2 Year 3 Year 4 Year % 35.6% 34.3% 26.9% 24.8% Criteria Men Women General Waist >40 inches >35 inches HDL Cholesterol <40 mg/dl <50 mg/dl % of EE Population wth condition Metabolic Syndrome is a cluster of health risk conditions that raise the risk for heart disease, diabetes and stroke. These risks include (1) high blood pressure, (2) high blood sugar levels, (3 high triglyceride levels, (4) low HDL cholesterol levels and (5) abdominal obesity (large waistline). If a person persists with three or more of these conditions, they are considered to have metabolic syndrome. Those with Metabolic Syndrome are twice as likely to develop heart disease, five times more likely to develop diabetes, and generate health costs 1.6 times higher than the costs of people without Metabolic Syndrome. Triglycerides >150 mg/dl Blood pressure Systolic: >130 Diastolic >85 Glucose >100 mg/dl 23

SECTION SIX ,000-LIFE CLIENT: FIVE-YEAR INJURY PREVENTION ANALYSIS The following summary information is based upon a recent five-year analysis of injury rates at a 10,000-life client. We currently deliver 16 coaches to this client. Our coaches service a wide variety of departments in multiple states. We integrated our coaching efforts with our client s existing safety and injury prevention strategies. In addition, we created customized programs and deliverables to target their number one cause of injury, strains and sprains. At the end of five years, in partnership with our client, we were able to achieve the following outcomes (as reported by our client). MANUFACTURING : 75% reduction in recordable strains and sprains LABORATORIES UTILITY OPERATIONS : 40% reduction in recordable strains and sprains SERVICE WORKERS MAINTENANCE : 80% reduction in recordable strains and sprains : 75% reduction in recordable strains and sprains : 69% reduction in recordable strains and sprains 24
