9 Outcomes Improved understanding of Service Coordination by agency leaders and staff Understanding of potential for Service Coordination to assist agencies to meet the needs of community members Assistance in meeting accreditation and HACC National Service Standards
10 Outcomes Ongoing support and resources for Service Coordination through local Primary Care Partnerships Plans developed for stage 5 linking workshops
11 Aboriginal agencies invited staff from other local agencies to build connections Local cultural and access issues for Aboriginal community members were explored together Plans made for future working together to provide coordinated support for Aboriginal Elders and people with disabilities
12 Outcomes 22 Linking workshops were held and one more is planned for next week 385 staff from a broad range of agencies participated, met each other and shared information Referral and care coordination pathways were established and/or strengthened

14 16 community information sessions were held with a total of 580 people attending
15 Community members gathered information about services, asked questions, gave feedback and referred themselves
16 Agency staff met and learned from community members
