Springboard Senior Education and Employment Case Worker is located at YSAS Frankston. Period: Full time (1 FTE) 38 hours per week fixed term to 30 June Remuneration: Annual Salary $60, $63, 546 (SCHADS Award level 6 ) dependent on qualifications and experience, plus 9.5% super, attractive salary packaging and full private use vehicle. Responsible to: Direct reports: Key responsibilities: Manager, YSAS Frankston. NIL Springboard is funded by the Victorian Department of Human Services (DHS), Child, Youth and Families Branch. The Southern Metropolitan Region Springboard Consortium (SMRSC) is delivered across the entire SMR by a consortium led by Mission Australia in partnership with Monash Health, Springvale Community Aid and Advice Bureau, YSAS and Berry Street. The aim of the Springboard initiative is to provide an intensive youth-focused program to explicitly assist young people aged 16 to 18 in residential out-of-home care, or those up to 21 years who have left residential care, to gain secure, long-term employment by re-engaging with appropriate education, training and/or supported employment opportunities. 1

2 Position Objective: The primary aim of the Springboard program is to assist young people leaving care in making a successful transition to further their education, training or employment pursuits. The Senior Education and Employment Case Worker provides direct service provision to the client group involving individual intensive case management and support, group work programs, assertive outreach and the provision of information and supported referrals. KEY RESPONSIBILITES Service Delivery Responsibility A goal of the Senior Education & Employment Case Worker is to form co-operative and collaborative relationships with other service providers. The person in the role will work closely with the Adolescent Placement and Support Service System. Participation in inter-agency collaborations and partnerships, including networking and presentations to the wider welfare community is also part of the role of the Senior Case Worker. Assist in the management and triage of referrals into Springboard, including client allocation; Maintain an average case load of young people; Undertake client registration and assessment process; Undertake and record client and staff risk assessments Develop & monitor Individual Pathway Plans (IPP) for all Springboard clients that set realistic goals and plan strategies, to overcome identified barriers to engagement in employment and/or education; Provide regular support to clients via case management/case work practices and review progression against Individual Pathway Plans; Participate in care team meetings as required; Support clients to achieve Progressive and Final Outcomes as per the Springboard contract; Provide assertive outreach services to engage with severely disengaged young people and encourage them to participate in the program; Work in a culturally sensitive, confidential and supportive manner; Conduct group workshops, recreational sessions for clients where necessary and appropriate and/or develop other innovative service responses; Assessments, including safety plans and risk management undertaken for all clients and recorded appropriately; Ensure all clients have a comprehensive Individual Pathways Plans (IPP). Ensure accurate recording of reports within organisational and SMRSC timelines and procedures; Support clients to improve links with communities and other services. Ensure confidentiality and legislative requirements are adhered to and in line with policies and procedures; Work collaboratively with team members to determine care and service needs to assist with coordination of care/service needs as required. Client review forums and supervision undertaken on a consistent basis; 2

3 Program coordination and staff support Work collaboratively with all relevant government and community service organisations to maintain effective referral pathways and opportunities for clients; Work collaboratively and co-operatively with residential, lead tenant and out-of-home care providers along with Adolescent Placement Teams (APT) within DHS; Actively participate in and contribute to the planning and delivery of one to one and group engagement activities; Provide secondary consultation and support to staff in conjunction with consortium line managers; Assist in maintaining a high performing team by role modeling clear communication, active involvement in decision making, early management of conflict and maintaining a positive team culture. Communicate with team regarding allocations of clients workloads and tasks; Represent the Springboard team at the SMRSC Operations Management group. Facilitate and actively participate in team meetings, peer supervision, strategic and program planning and professional development in relation to Springboard. Administration Create and update individualised case management files for all clients in line with Springboard protocols and ensure that all Springboard clients are registered and recorded on YATMIS according to contract Complete a range of administrative duties as required to operate an efficient service including development of Individual Pathway plans for clients, statistics, reports, referral letters, promotional material, etc. Access brokerage funding and maintain auditable records for individual clients to support achievement of outcomes and administer these funds in accordance with Springboard protocols Communicate directly and provide feedback to the SMRSC Operations Management, including monthly reporting of outcomes. Develop collaborative service/program responses; Maintain formal/informal networks in order to preserve positive working relationships; Provision of advice and support to team and relevant organisations; Facilitate fortnightly team meetings, in house professional development forums and case review/reflective practice; Develop an understanding of YSAS practice frameworks. Maintain all client data is recorded accurately and up to date; Provide relevant data as specified by the SMRSC as to program development and delivery; Record and report accurate brokerage expenditure; 3

4 Stakeholder Relationships Provide reports and regular feedback to Service Managers, Quality and Compliance Officer and the SMRSC Operations Management group; Work with all Consortium partners and their staff in accordance with the contract; Build and maintain community and stakeholder relationships; Work consistently and represent the SMRSC in a professional manner at all times; Develop strong supportive relationships with Springboard staff state-wide; Develop strong, effective and consultative working relationships with Springboard staff to contribute to the effective functioning of the consortium; Develop strong relationships with key external stakeholders including education providers, other service providers, community service workers, government departments to assist in the receipt of information and referral of clients; Continued enhancement of service coordination with key stakeholders. Attendance at relevant community networks/service presentations/seminars as required or directed; Maintain a continual presence with identified key stakeholders by the SMRSC. Compliance and Professional Conduct Work within the bounds of relevant legislation/regulations (e.g. Privacy, FairWork, OHS, etc.) and relevant professional codes; Adhere to all YSAS policies and procedures including the Code of Conduct, Confidentiality Agreement, EEO policy; Contribute to continuous quality improvement of the organisation. Demonstrated commitment to YSAS organisational values; and Provide highest ethical standards at YSAS: not only to young people in the communities we serve, but to our co-workers and ourselves; Ensure all work complies of the relevant legislation/ regulations, YSAS policies and procedures and other relevant compliance requirements; On every occasion YSAS is represented ethically and professionally; Ensure that the confidentiality of documentation and information is maintained. 4

5 Qualifications, skills and other requirements Tertiary qualifications in a human services discipline (e.g. Social Work, Youth Work, Family Studies etc); A current Victorian driver s license is essential; The successful applicant will be required to undergo a National Police check & hold a current Working With Children Check; Comprehensive knowledge of the Placement and Support Service system, DHS; Availability to work some weekends and after hours as required A current First Aid (Level 2) certificate is desirable Training in Mental Health First Aid is desirable Key Selection Criteria 1. Demonstrate a comprehensive understanding of issues affecting young people who are, or have been, in residential out-ofhome care. 2. Advanced skills in engaging and supporting the client group to overcome vocational and non-vocational barriers to employment and education and comprehensive knowledge of this service system. 3. Highly developed engagement and communication skills with young people from a range of backgrounds (including cultural awareness) and age groups, including the ability to model appropriate behaviours and motivate young people. 4. Demonstrated experience of participation in co-operative and collaborative work practices with other service providers and community groups, including well developed liaison and networking skills and. 5. Demonstrated skills in written communication, including report writing, case notes and data recording. 6. Ability to work autonomously in an outreach capacity and in relative isolation whilst maintaining resilience and accountability along with experience in the peer support, provision of secondary consultation, mentoring and maintaining a positive team environment.
