2 QUALITY ASSURANCE INSPECTOR The Gloucester County Improvement Authority (GCIA) invites qualified firms to submit a proposal to provide the services of a Quality Assurance (QA) Inspector and soils laboratory testing services for the above referenced project in accordance with NJDEP regulations. The firm selected by this RFP shall serve as the independent quality assurance required by N.J.A.C. 7:26-2A.7(a)(7) and shall be responsible for the performance of all functions of the independent quality assurance inspector under N.J.A.C. 7:26-2A et seq., whether or not specifically enumerated in the RFP. The construction of Cell 2, which was permitted as part of the Phase VII Development lateral expansion of the Gloucester County Solid Waste Complex (GCSWC) located in South Harrison Township, will provide an additional 9.2 acres of landfill capacity for the disposal of approximately,326,500 tons of permitted solid waste materials. This state of the art designed landfill cell will consist of a double-composite bottom liner, an independent slope riser leachate collection system, and all related excavation, backfill, piping and electrical work. The contract documents, Quality Assurance/Quality Control (QA/QC) plan and other related documents are available for review at the office of T&M Associates, 256 N. Church Street, Moorestown, New Jersey. The QA Inspector s Requirements (QAIR) follow as a guide in preparing your response. Your attention is directed to the job description with the Frequency of Inspections section of the QAIR. Any correspondence you consider confidential should be clearly identified, as the NJDEP may require resumes and additional qualification statements be submitted to them. Your proposal should include (a) resume(s) of the personnel proposed for this work as well as a listing of similar or related type projects with references. The capabilities of both your soils and geotechnical laboratories should also be provided. The named personnel shall be the individuals who will actually perform the services on this project. The contract time as stipulated in the contract documents is 80 calendar days. It should be noted that a winter shut down may be necessary, which would extend the project completion date. We request that your use the enclosed Attachments for presentation of man hours/costs, other direct costs, and soils laboratory test costs. The hourly rate for personnel indicated on Attachment Nos. and 2, will be used, should the project period be extended beyond the estimated schedule. A laboratory test fee schedule should be attached should additional or alternate testing be required during the course of the project. The cost of transporting all lab samples shall be included and the laboratory s current certification shall be indicated.

3 The proposal cost will be based upon a not-to-exceed amount. Overhead and profit should be included in the hourly labor rates and the laboratory test fees. Premium costs due to overtime work, Travel costs (rate/mile), and daily meal expenses shall not be billed nor reimbursed. Costs incurred due to an extended project schedule or additional soils lab tests will be at the rate included in Attachments, 2 and 3. Award of this contract shall be subject to approval of NJDEP under N.J.A.C. 7:26-2A. 7(a)(7) and through a fair and open process pursuant to NJSA 9:44A-20.4 et seq. Gloucester County Improvement Authority Fair & Open Process Requirements A. Any professional service entity or other business entity providing services to the Gloucester County Improvement Authority shall be subject to a fair and open process. A fair and open process shall be set forth in Subsection B. below, which shall establish criteria for the fair and open process. B. The fair and open process established by the Gloucester County Improvement Authority shall require the following: () Any professional position or service contract not subject to bidding that shall expire as of the date prior to reorganization of that calendar year, shall be advertised by the Gloucester County Improvement Authority in the official newspaper of the Gloucester County Improvement Authority for publication of notices and/or published on the Gloucester County Improvement Authority s website, not later than 30 days prior to the reorganization of the Gloucester County Improvement Authority. The published notice shall set forth the name of the position or contract which is to become open, and set forth the criteria to be considered and materials required to be submitted by the applicant. (2) Should the applicant be a professional requiring licensure in the State of New Jersey, said applicant shall be licensed for a period of not less than five years. (3) The applicant shall submit a certificate of good standing, affidavit or other similar document evidencing that the professional s license is not presently suspended or revoked. (4) The applicant shall submit a resume which shall set forth information, including but not limited to the following: (as applicable to a business entity or individual or professional) A. Full name, business. B. A listing of all post high school education of the applicant. C. Dates of licensure in the State of New Jersey and any other state.

4 D. A listing of all professional affiliations or memberships in any professional societies or organizations with an indication as to offices held therein. E. The number of licensed professionals employed by/affiliated with the business entity or the business entity which employs the applicant. F. A listing of all special accreditations held by the individual licensed professional or entity. G. A listing of all previous public entities served by the business entity or licensed professional indication the dates of service and positions held. July 0, 204 Role of the Quality Assurance (QA) Inspector The QA Inspector is required by N.J.A.C. 7:26-2A.7 and the duties of this representative are set forth at N.J.A.C. 7:26-2A. et seq. Specifically, the duties of the QA Inspector will be to perform all required system audits of the Quality Control (QC) inspections to insure proper implementation of the design and permit requirements.

5 Qualitification Requirements of the QA Inspector Five (5) years construction experience with specific experience in earthwork projects, including soils, the installation of geomembrane liners and the installation of geosynthetic clay liners (GCLs); Familiarity with the use of nuclear density soil testing equipment; Basic understanding of surveying methods; Ability to comprehend results of laboratory and field test analysis for soils, and also mechanical properties of geomembrane liners, GCLs and other geosynthetics. The QA Inspector must be a New Jersey Licensed Professional Engineer (P.E.) if the weekly status reports are certified by the QA Inspector. Alternatively, the reports can be reviewed and certified by a second individual who is a licensed P.E. (The QA Inspector shall report directly to this licensed P.E.) QA s Inspector s Contact with the New Jersey Department of Environmental Protection (NJDEP) Prior to the start of construction, the QA Inspector shall attend a meeting with the NJDEP to establish reporting procedures. The certifying P.E. will also attend this meeting if the QA Inspector is not a licensed P.E. The QA Inspector shall forward weekly construction status reports with back-up documents, signed and sealed by a licensed P.E., certifying that to the best his/her knowledge the construction requirements of the NJDEP are being met. In addition, the certifying P.E. will inspect the project work site at the midpoint of construction and at the end point, and certify that these inspections were made if the QA Inspector is not a licensed P.E. The QA Inspector shall attend a Project Summary meeting at the end of the construction project. The certifying P.E. will also attend this meeting if the QA Inspector is not a licensed P.E. The QA Inspector shall perform additional duties as required and documented by the NJDEP and/or N.J.A.C. 7:26:26-2A. et seq., provided approval is first granted by the Owner prior to performing any additional tasks. QA Inspector s required Tasks The QA Inspector shall audit the QC Inspector s reports, findings, and surveys to insure that the QC Inspector has inspected the following:

6 Subgrade ) excavation and preparation to required elevations; 2) backfill and compaction of disturbed areas to required density and lift thickness; and 3) inspection for acceptability prior to installation of geosynthetic materials Geomembrane Liner ) placement of liner panels are in accordance with approved installation plan; 2) permanent and temporary anchoring procedures are followed; 3) overlap and seam width are in compliance with the design; 4) area to be seamed is clean and supported; 5) check the uniformity and continuity of seams and welds; 6) check for imperfections in seams, wrinkles at seams and confirm that fishmouths are repaired as required; 7) installation is performed during permitted weather conditions; 8) non-destructive field testing of seams is verified using vacuum test methods (for double fusion seams only); 9) verify that required destructive field seam strength tests, including in-field testing for peel and shear are performed; 0) confirm that all destructive test areas are repaired and tested; ) check that laboratory conformance testing is performed on all roll samples in accordance with specification requirements. Geosynthetic Clay Liner (GCL) ) placement of liner panels are in accordance with approved installation plan; 2) permanent and temporary anchoring procedures are followed; 3) overlap and seam width are in compliance with the design; 4) area to be seamed is clean and supported; 5) check the uniformity and continuity of seams; 6) check for imperfections in seams and confirm repairs as required; 7) installation is performed during permitted weather conditions; 8) check that laboratory conformance testing is performed on all roll samples in accordance with specification requirements. Leachate Drainage Layer ) check that thickness is in conformance with the design; 2) verify that drainage pipes are installed as shown on the design drawings; 3) check that permeability of sand is verified by laboratory testing; 4) check that particle size is in conformance with the design requirements;

7 Additional duties as required by N.J.A.C. 7:26-2A. et seq. Frequency of Inspections for QA Inspectors: This project has an estimated construction period of 80 calendar days. The QA Inspector shall be at the site during the initial construction phase of the subgrade preparation, containment system and leachate facilities The QA Inspector shall be onsite observing the construction for a period on one nine (9) hour day per week for the 8 week construction period (or longer if conditions warrant, i.e., the project completion period is extended). During this observation period, the presentation of all documents shall be made to the QA Inspector by the QC Inspector, and copies of all documents shall be furnished for this person s review. The total hours expended for all tasks are estimated to be as listed below.
