1 SUMMARY OF HAZARDOUS MATERIALS WORK 1.01 GENERAL A. The San Francisco Unified School District (SFUSD) is soliciting proposals for conducting asbestos, lead and mold related remediation work to support Destructive Testing at James Denman Middle School: B. The hazardous materials abatement portion of the project includes removal, clean-up, decontamination, and proper disposal of asbestos and lead containing materials and mold impacted surfaces as necessary to support the Destructive Testing Project. Asbestos, lead and mold related work will take place in areas of work indicated Destructive Testing Request Form and associated floor plans HAZARDOUS COMMUNICATION A. Hazardous materials present in the building(s) and structures at this site include: asbestos containing materials (ACMs); assumed asbestos containing materials; asbestos containing construction materials (ACCMs); lead-based paint (LBP); lead-containing coatings and materials; mold growth and mercury containing fluorescent lighting tubes, switches, and thermostats. Refer to Section Existing Hazardous Materials Conditions for a list of known and assumed hazards at the site SCOPE OF WORK A. The Contractor(s) work includes the removal of asbestos-containing materials, finishes coated with lead-containing paint coatings and surfaces impacted with mold growth to the extent specified to support the Destructive Testing Project. The Contractor is responsible for coordination with the General Contractor for the completion of the work of the project. All removal shall be to the extent necessary to properly complete the work of the project. All asbestos and mold related work shall be conducted within regulated areas under negative pressure with a minimum of a two-stage decontamination unit. All lead related work shall be conducted within a negative pressure enclosure for dust control purposes. The Contractor s hazardous materials scope of work includes but is not limited to the following: 1. Areas 3, 4 & 10 For Bidding Purposes, Contractor shall assume that mold is present behind all plaster surfaces scheduled for removal and that mold remediation is required by manual or mechanical scrubbing, sanding or abrasive means at all exposed concrete surfaces and furring scheduled for demolition. Refer to the Destructive Testing Request Form for locations and details. 2. Area 6 & 9 Remove, clean-up and dispose of specified section of wall plaster and asbestos-containing acoustical ceiling plaster. All cuts shall have a clean edge and be encapsulated with a bridging encapsulant upon completion of inspection by the District s Environmental Consultant. If mechanical methods are used, all powered equipment must be equipped with a shroud and attached to a functioning HEPA vacuum during operation. Refer to the Destructive Testing Request Form for locations and details. 3. Areas (1, 2, 5, 7, 8 & 13) There are no known hazards at these proposed destructive testing locations. 2 containing paint and require good work practices, prompt clean-up with HEPA vacuums and wet wiping. 4. All Areas Contractor shall be aware that asbestos-containing pipe insulation concealed in walls, ceilings, chases, etc. If encountered during the opening of walls and ceilings, notify the District immediately for inspection. For bidding purposes, Contractor shall assume that 10 linear feet of asbestos-containing pipe insulation will be exposed and require removal over the course of all destructive testing locations WORK SCHEDULE A. The following is the anticipated work schedule as defined by the Destructive Testing Project: 1. Five (5) eight-hour shifts after 6:00pm on a weekday, weekend, or holiday when students and staff are not present. Contractor shall comply with local noise ordinance requirements RELATED DOCUMENTS A. Pre-start Submittals. Submit a minimum of one (1) copy of the following hazardous materials submittals to the Environmental Consultant at the site prior to the start of hazardous materials work. Additionally, the Contractor shall maintain one (1) copy at the site at all times during hazardous materials related work. 1. Licensing and Registration: Submit copies of current and valid certificates for the following: a. Contractor s license and Contractor s asbestos certificate issued by the California State Licensing Board (CSLB); b. Contractor s certification to conduct lead-based paint renovation, repair and paint activities pursuant to 40 CFR (i.e. EPA RRP). c. Registration for asbestos-related work from OSHA in accordance with 8 CCR, Article 2.5 (asbestos abatement contractors only). 2. Notifications, Communications and Postings. Provide copies of all required notifications including the following: a. Division of Occupational Safety and Health Local Office (Temporary work site notification-asbestos and lead) b. 3 3. Personnel Qualifications: Personnel documents required by this section shall be organized by individual employees and must be current and valid. All workers who will be performing work at the site will be required to show photo documentation prior to approval of their personnel documents. Workers who do not have all the required documentation present at the site, including photo documentation, will be denied access to the type of hazardous material Work Areas for which they are lacking full valid documentation. a. Training Certificates for Asbestos: Submit proper documentation that Competent Person(s) and Workers scheduled for this project have successfully completed Cal/OSHA approved courses for asbestos abatement. b. Medical Examination: Submit proper documentation, in the form of the physician s written opinion, showing that all hazardous materials abatement personnel scheduled for this project have had the appropriate medical examinations applicable to their assignments. Exams must be in accordance with 8 CCR for lead and 8 CCR 5144 for respiratory protection. All exams must have been conducted within the last 12 months. Respiratory use evaluation exams alone do not suffice for asbestos and lead related work. Do not submit actual medical exam results. The written physician s opinion should indicate what exam(s) were provided and limitations on the worker. c. Respirator Fit Tests: Submit proper documentation that personnel who will be entering Regulated Areas have had a qualitative respirator fit test performed within the last 12 months for all face fitting respirators. d. Provide a signed copy of Certificate of Worker s Acknowledgment (Appendix C) for each worker conducting hazardous materials related removal work. 4. Calibration Data: Submit calibration data for the secondary standard (rotometer) that will be used on this project to calibrate personal air sampling pumps. The secondary standard must be calibrated to a primary standard within the last (6) six months. 5. HEPA Filtration Certifications: a. Provide third party test certificates for all Differential Pressure Equipment and HEPA Vacuums to be used on this project. Such Certificates shall document that each item of equipment has been tested on-site prior to start-up and that the results have demonstrated that each HEPA equipment assembly meets the efficiency requirement for HEPA filtration as an installed system or unit of equipment. b. All HEPA filtration testing must be conducted by challenging the installed filter system with 0.3 micrometer diameter particles using a dioctyl phthalate (DOP) particle generator & appropriate aerosol measurement test equipment designed for this purpose. Alternate test methods may be accepted if demonstrated to be equivalent and approved by the Environmental Consultant. c. Test certificate stickers shall be placed on each machine tested and a copy of the testing certification shall be provided to the Environmental Consultant. The test result, date and time of testing, testing firm, and signature of qualified test technician shall be included on each certification along with equipment identification information. 4 SECTION EXISTING HAZARDOUS MATERIALS CONDITIONS PART 1 GENERAL 1.01 SUMMARY A. This section provides a list of known and assumed hazardous materials that may be impacted during renovation, demolition, repair, custodial and/or maintenance activities. The hazardous materials information has been provided through existing surveys conducted by the San Francisco Unified School District (District) and the District s environmental consultants. B. Some materials and items found at the Site either contain or may contain materials known to the State of California to be either hazardous, carcinogenic or reproductive toxins. These include but are not limited to asbestos, lead, PCB s, silica, and other materials. C. The Contractor shall hold the District and its consultants harmless for claims, damages, losses, and expenses, including attorney s fees arising out of the Contractor s hazardous materials related work including releases from any incidental disturbance of existing hazardous materials, on-site or off-site spills of hazardous materials, or from non-compliance with the Contract Documents and regulatory requirements HAZARD COMMUNICATION A. The District may have conducted previous hazardous materials abatement projects at the site. The hazardous materials abatement oversight information is available for review by appointment only through the District s Asbestos Control Program at (415) B. Copies of previous hazardous materials report(s) and the AHERA Management Plan for the site are available for review by appointment only through the District s Asbestos Control Program at (415) C. Asbestos Hazards at James Denman Middle School 1. Asbestos has been identified or shall be treated as containing asbestos at concentrations greater than one percent (>1%) in the following materials: a. 5 1) Glazing compounds (black) (3% Chrysotile asbestos) at exterior metal windows (small with random patterns) and metal storefront assembly at north elevation. 2) Acoustical wall plaster (1-2% Chrysotile asbestos) at Auditorium 005, Sound Room 005A and Projection Room 005B. 3) Pipe insulation (35% Chrysotile asbestos) on straight pipe runs, pipe elbows and tees at Auditorium Crawlspace (Asbestos Restricted Space), Stage 006, below Stage 006 and throughout at wall cavities and ceiling plenums. 4) Fire curtain (assumed) located at Stage ) Chalkboards and associated mastics (15% Chrysotile) at Classrooms 001, 002 and 003 and Storage Room ) Heat shields (assumed) at lights at Practice Rooms 001A and 001B, Office 001C, Storage Rooms 004A and 004B. 7) Stage lighting fixtures with insulated wiring (assumed) at Storage 004 (loose) and Stage 006. c. Main Classroom Building 1) Caulking (1 2% Chrysotile asbestos) located between the existing wood window frame and sills concealed by new metal framing and panning at windows located at the First, Second and Third Floors. 2) Window glazing compounds, caulkings and putty material (1 2% Chrysotile asbestos) at metal windows, metal window frames, metal security screens / frames and rough opening at the north and south elevations at the Ground Floor and Stairwells S80 and S82. 3) Window glazing compounds and caulkings (1 2% Chrysotile asbestos) at wood window and frame at Girls Toilet T-66. 4) Acoustical ceiling plaster and strip of acoustical wall plaster at upper walls (1-2% Chrysotile asbestos) at Office 022 and Corridor / Stair C97 at the Ground Floor; Classrooms 100, 102, 104, 106, 108, Principal Office 110, Administration Office 112, Administration Hall 114, VP Office 114A, Office 114B, Office 114C, Counselors Office 114D, Faculty Lounge 115, Storage 115B, Corridor 116, Offices 116H, 116J, 116K and 116M, Corridor 118, Office 118A, Office 120, Storage 120A, Teacher Work Room 127, Copy Room 127C and Corridor C95 at the First Floor; Science Room 201 (concealed by 12 x12 ceiling tile), Classrooms 200, 202, 204, 206, 207, 208, 210, 211, 212, 214, 216, 217, 218 and 223, Corridor C95 and Storage C95A at the Second Floor; and Classrooms 300, 301, 302, 304, 306, 307, 309, 310, 312, 314, 316, 317, 318, 319, 321 and 323, Storage 301A and 301B, Corridor C-95 and Stair S-81 at the Third Floor. 5) Acoustical wall plaster (1-2% Chrysotile asbestos) at Office 120 and Storage 120A at the First Floor and Classrooms 301, 306, 307 and ) Taping compounds (2% Chrysotile asbestos) on gypsum board walls at Corridor 116, Offices 116H, 116J, 116K and 116M, Corridor 118, Office 120 and Storage 120A at the First Floor (Note: for disposal purposes, the overall gypsum board/taping compound composite is less than one percent October 4, EXISTING HAZARDOUS MATERIALS CONDITIONS

6 asbestos and shall be disposed of as a non-hazardous asbestos waste). 7) Pipe insulation (35% Chrysotile asbestos) on straight pipe runs, pipe elbows and tees at Storage 002, Fan Room 002A, Storage Room 003, Service Closet 003A, Mechanical Room 004, Boiler Room 005, Storage/Book Room 005A, Office 005B, Toilet T-05, Storage 010 and Storage S-81A at the Ground Floor; Corridor C-95 at the First Floor; and throughout at wall cavities and ceiling plenums. 8) Paper insulation (assumed) located behind counter tops at Classrooms 207, 211 and 223, Storage 211A and Classroom ) Thermal insulation (assumed) located behind recessed convectors located at all Corridors and Stairs. 10) Chalkboards and associated mastics (3% Chrysotile / 3% amosite) at Classroom 223 at the Second Floor. 11) Sinks with black undercoating (3% Chrysotile asbestos) at Classrooms 300, 302 and ) Boiler brick, mortar and caulkings/sealants (15% Chrysotile asbestos) at interior boilers at Fan Room 002A and Boiler Room ) Metal sliding fire doors (assumed) at Boiler Room 005 and Transformer ) Boiler insulation located on boilers (assumed) at Boiler Room ) Hot water tank insulation (assumed) at Boiler Room ) HVAC duct / breeching insulation (assumed) at Service Closet 003A (in ceiling) and Boiler Room 005 and throughout at ceiling plenums and wall cavities. d. Gym Building 1) Acoustical ceiling plaster and strip of acoustical wall plaster at upper walls (1-2% Chrysotile asbestos) at Offices 051, 052 and 057, Classroom 058 and Corridor C98 at the Ground Floor. 2) Floor tile mastic (black) (5% Chrysotile asbestos) located below nonasbestos containing 12 x12 tan/white/gold floor tile at the entrance to Gymnasium ) Pipe insulation (35% Chrysotile asbestos) on straight pipe runs, pipe elbows and tees at Girls Locker Room 050, Storage 050A, Office 051, Office 052, Mechanical 053, Entry to Boys Locker 055, Storage 055B and 055C, Classroom 058 and at the Ground Floor and throughout at wall cavities and ceiling plenums. 4) Thermal insulation pads (assumed) located behind recessed convectors at CorridorC98. 5) Wood parquet flooring glues and underlayments (assumed) at Gymnasium 150, Storage 150C, Gymnasium 155 and Storage 155A. e. Shop Building 1) Window glazing compounds (white / gray / light blue cementitious material) (1-2% Chrysotile asbestos) at all steel windows and curtain walls. 2) Caulking/putty material (1-2% Chrysotile asbestos) located between metal window framing and concrete rough openings at building exterior. 7 3) Acoustical ceiling plaster and strip of acoustical wall plaster at upper walls (1-2% Chrysotile asbestos) at After School Program 081 and Corridor C-99 at the Ground Floor; and Computer Room 181, Corridor C99 and Stair S85 at the First Floor. 4) Pipe insulation (35% Chrysotile asbestos) on straight pipe runs, pipe elbows and tees at Room 080A, Records 080, 080B and 080C, After School Office 081 at the Ground Floor; and throughout at wall cavities and ceiling plenums. 5) Chalkboards and associated mastics (assumed) at Classroom 180 (concealed by mirrors). 6) Wood parquet flooring glues and underlayments (assumed) at Classroom ) Glazing compounds (assumed) at Skylight at Corridor C99 and Storage Rooms183A and 183B and Office 188 at First Floor. 8) All roofing materials and associated mastics/caulkings (assumed). 2. Asbestos has been identified at concentrations less than one percent (<1%) in the following materials: a. Asphalt slurry (no aggregate) (0.20% Chrysotile asbestos) located between the asphalt paving and concrete sublayer at the Main Play Yards (upper and lower). 3. The following sampled suspect materials had results that reported NO asbestos detected by PLM analysis (Exterior Site and Exterior and Interiors of all Buildings): a. Concrete sub material located below asphalt paving at the Main Play Yards. b. Petromat (black felt) located between the asphalt paving and concrete sublayer at Main Play Yards. c. Exterior concrete retaining walls and curbs. d. Exterior paints. e. Exterior skim coat stucco/plasters and base stucco/plasters on exterior walls and overhangs. f. Exterior green ceramic wall tile, grout and mortar at the front of the campus. g. All roofing materials (field) at the Main Building. h. Silver paint coatings on roof parapets. i. Black and gray tar / pliable caulking / sealants at window glazing at metal windows and curtain walls. j. Red caulking / sealant at window glazing at metal windows and curtain walls. k. Gray caulking / sealant at window glazing at metal windows and curtain walls. l. Gray silicone / rubber caulking at window glazing at metal windows and curtain walls. m. Glazing compounds (white sticky) used at glazing replacement at metal storefront assembly at north elevation of the Auditorium Bldg. n. Interior paints. o. Smooth wall and ceiling plasters. p. Skim coat plasters on concrete walls and ceilings. q. Concrete ceilings, walls and floors. r. Sprayed-on acoustical ceiling material at Auditorium Wing. s. Areas and/or Spaces known or presumed to be contaminated with asbestos containing materials, dust, and debris include: a. The crawlspace areas located below Auditorium 005 (accessed from Service Closet 003A) at the Ground Floor is contaminated with asbestos containing pipe insulation and duct insulation debris (35% Chrysotile asbestos). Access to these crawlspace areas should be isolated and restricted to asbestos trained personnel only. b. The ceiling plenums at Corridor C-99 at the Ground Floor of the Library/Shop Building is contaminated with asbestos containing pipe insulation debris and damaged asbestos containing pipe insulation (35% Chrysotile asbestos). Access to these ceiling plenum areas should be isolated and restricted to asbestos trained personnel only. 5. Areas and/or Spaces where asbestos abatement was conducted include: a. All asbestos containing acoustical ceiling plaster was removed at Boys Locker Room 055A and Corridor C-96 at the Ground Floor and Corridor C-96 and Computer Rooms 105 and 107 at the First Floor and as part of the Leroy Green Project in 1993/1994. b. All asbestos containing 9 x9 floor tile and mastic were removed at Boys Locker Room 055A, After School Office 081 and Corridor C-96 at the Ground Floor October 4, EXISTING HAZARDOUS MATERIALS CONDITIONS

10 and Storage 217A, 217B and 217C at the Second Floor as part of the Leroy Green Project in 1993/1994. c. Asbestos cement ceiling tiles were removed from Service Area 009, Kitchen 013 and Office 015 as part of the Leroy Green Project in 1993/1994. d. All asbestos containing fire doors excluding the two metal sliding doors located at Boiler Room 005 and Transformer 006 were removed from the Gymnasium Wing as part of the Leroy Green Project in 1993/1994. e. All asbestos containing 12 x12 orange floor tile and mastic were removed from Classroom 016 and Cafeteria 018 as part of the Leroy Green Project in 1993/1994. f. All asbestos containing pipe insulation and pipe insulation debris were removed at the Crawlspace areas located below the Academic Wing of the Main Building (Crawlspaces accessed from Cafeteria 018) and the areas were completely cleaned and decontaminated as part of the Leroy Green Project in 1993/1994. g. All asbestos containing thermal pads were removed on the sides of the counter tops at Faculty Lounge 115 as part of the Leroy Green Project in 1993/1994. h. All exposed asbestos-containing pipe insulation was removed from Storage 012 and Toilet T-14A in 2007 as part of the 2003 Bond Program. i. All 9 x9 (multiple colored) and mastic were removed in the following areas as part of the 2003 Bond Program in 2007: 1. Ground Floor at Storage 012, Toilet T14A, Storage 050A, Offices 051 and 052, Classroom 058, Office 057 and Staff Toilet T-57A. 2. First Floor at Classrooms 001, 002 and 003, Practice Rooms 001A and 001B, Office 001C, Storage 004, 004A and 004B and Corridors C-91, C- 92, C-93, C-95 (patched areas at Corridor C-95) and C Second Floor at Corridor C-95 (patched areas at Corridor C-95). 4. Third Floor at Corridor C-95 (patched areas at Corridor C-95). D. Lead Hazards at James Denman Middle School 1. Lead has been detected in individual painted surfaces and surface coatings in concentrations greater than 5,000 parts per million (ppm) lead or 1.0 milligram of lead per square centimeter ( Where ranges of lead levels are indicated, Contractor shall presume the highest level is typical. These lead containing surfaces include, but are not limited to the following: a. The District has not verified that any paints, coatings, dusts, or materials are lead free or below 600 ppm. The Contractor shall treat all paints, coatings, dusts or materials as having a lead content greater than 600 ppm requiring dust controls and personal protective procedures for construction activities in conformance with the Cal/OSHA Lead Construction Standard, 8 CCR lead. Any paint, varnish, or other coating or finish not listed above shall be considered lead-based paint with lead levels at or exceeding 5000 ppm lead or 1.0 mg/cm 2 for this contract. 5. All firms, including sub-contracted firms who impact lead-based paint (LBP) (5,000 ppm lead or 1.0 mg/cm 2 or greater) at Child Occupied Facilities shall conduct all work in accordance with 40 CFR Part 745. This includes but is not limited to being an EPA certified firm; having an EPA Certified Renovator ; providing on-the-job training for workers; conducting pre-renovation notifications; following specific work practice procedures for containment, disturbance and final clean-up; and inspection requirements. The EPA certified Contractor or Sub-contractor(s) Certified Renovator shall be responsible for identifying the specific job activities which impact lead-based paint (LBP) during renovation that requires the use of containment as described in 40 CFR Part 745. Work also includes but is not limited to provide on-the-job training for workers; conduct pre-renovation notifications; follow specific work practice procedures for containment, disturbance and final clean-up; and inspection requirements as defined by regulation. 7. In addition to lead-containing paints and coatings, the Contractor shall assume that lead is present at detectable levels over 600 ppm in existing plumbing components and solders, glazing compounds, roof jacks, and surficial soils. E. Metallic Mercury and mercury compounds are present at this site in fluorescent lighting tubes, high intensity discharge lamps, mercury switches and mercury thermostats. All demolition and disposal of these items shall be conducted in accordance with applicable safety and environmental regulation and the requirements of the Contract Documents. F. All light ballasts at this site have been replaced and light fixtures contain nonpolychlorinated biphenyl (PCB) containing fluorescent lighting ballasts. G. Caulkings, glazing compounds and putties associated with wood windows, metal windows, metal curtain walls and metal window security screens at the campus were identified to be non-pcb containing based on laboratory analysis. H. Crystalline Silica is presumed present in all concrete, plaster, ceramic tile, grouts, and other cementitious materials at this site as well as soils. Worker protection and control of air dust during cutting, drilling, demolition and other construction operations is the responsibility of the Contractor. I. The Contractor shall take into consideration all existing known and presumed hazardous materials that may be disturbed or otherwise impacted by the Work of this project. All work of this project that disturbs or otherwise impacts hazardous material shall be considered included in the Work of the project and shall be conducted in accordance with all applicable regulations and the Contract Documents.
