1 2018 Wellness Program Phases Live Well Pledge Outcomes Ac vi es Live Well Pledge Sign Pledge during Open Enrollment in 2017 Complete Pledge requirements by May 31, 2018 Make the Live Well Pledge by the end of the open enrollment period in 2017 and receive $100 in an HRA or HSA account on January 1, Pledging to: 1. Complete the Personal Health Assessment on the wellness portal (10 ) AND 2. Complete ONE (1) of the following: Biometric Screening on campus (10 ) Preven ve Care Exam by Personal Health Care Provider (10 ) Biometric data from your preven ve care exam MUST be provided to MSU s Health Coach/RN by May 31 for upload to your wellness portal to sa sfy the pledge requirements. For this purpose, data from preven ve exams during the me period of June 1, 2017 to May 31, 2018 will suffice. MSU expects employees and their spouses to stand by their Live Well Pledge; therefore, if you break your pledge, you will be expected to pay back the $100 HRA/HSA contribu on, which will be deducted equally from your June 15 and 30, 2017 pay releases.

2 Outcomes December 1, 2017 November 30, 2018 Achieve biometric targets or goals to earn to receive a contribu on in your HRA or HSA Achieve specified biometric targets in the categories of weight, cholesterol, blood pressure and glucose and receive a deposit in your HRA or HSA on July 1 or December 15, depending on when you meet the target range or goal. Biometric screenings are available to employees and covered spouses at no cost or you can provide the biometric data from personal health care provider to MSU s Health Coach/RN. Weight: Cholesterol: $50 and 5 for mee ng the target range or goal $50 and 5 for mee ng the target range or goal Blood Pressure: $50 and 5 for mee ng the target range or goal Glucose: $50 and 5 for mee ng the target range or goal Achieve targets or goals in all 4 categories and receive an EXTRA $75 for a total of $275 for achieving healthy outcomes! Also receive 20 Well- Points

4 Ac vity Phase Campaign 1 Campaign 1 Healthy Ac vity Ac vity Descrip on My Personal Health Mission Statement Health Coaching Preven ve Care Visit Weight Watchers Enter your personal health mission statement on your portal dashboard. On-site Health Coaching by a Cer fied Health Coach/RN in the Office of Counseling and Health Services is available to employees and their spouses by appointment. Self-report a preven ve care visit. These are annual, rou ne, preven ve exams that are considered age and gender appropriate and not an exam for maintenance of a diagnosis: physical exam, gynecological exam, vision exam, mammogram, dental exam, colonoscopy. Complete a Weight Watchers Series. The Weight Watchers At Work Program is offered at MSU year round with 3 Series. Each series is worth 50 MSU pays half the membership fee for employees. A end 75% of mee ngs in a Series to qualify for Weight: Log on Wellness Portal Monthly Weight is self-reported on weight log on the wellness portal. Enter your weight each month and receive Tobacco Cessa on Program Complete an Evidence-Based Tobacco Cessa on Program. MSU offers the Cooper Clayton program each semester to employees and spouses at no cost Immuniza on (flu, shingles-age appropriate) Self-report an annual immuniza on as a Healthy Habit on the wellness portal

5 Ac vity Phase, Con nued Healthy Ac vity Ac vity Descrip on Campaign 1 Campaign 1 Physical Ac vity Strength Training Steps Online Wellness Workshops On Campus Wellness Workshops On Site Events Physical ac vity is self-reported on the cardio log on the wellness portal. Receive 2 for each 30 minutes of physical ac vity. Receive 2 for each 30 minutes of ac vity up to 60 minutes (4 Well- Points) per day. Strength training is self reported on the strength training log on the wellness portal. Receive 2 Well- Points for 30 minutes of strength training per day. Steps are self-reported in the Steps Log or via a Fitbit sync. Receive 2 for each entry of 5,000 steps up to 10,000 steps per day. Your wellness portal has a variety of informa ve and interes ng workshops that can be completed to receive 5 per week of the workshop. A variety of workshops are offered on campus as part of our partnership with St. Claire Regional Medical Center and other health professionals and organiza- ons. Special events are held throughout the year, i.e., Wellness Day, Breast Cancer Awareness Walk, Group Walks, Scavenger Hunts, etc. These events will be worth 10 unless stated otherwise Farmers Market on Campus Visit the Farmers Market on campus (seasonal)

6 Ac vity Phase, Con nued Healthy Ac vity Ac vity Descrip on Campaign 1 Campaign 1 Massage Therapy Self-report massage therapy as a Healthy Habit on the wellness portal. 5 per month Fruit and Veggie Tracker Receive 1 WellPoint for consuming at least 3 servings of fruits and 3 servings of veggies and logging them on the tracker located on the wellness portal dashboard or in the food log. Water Tracker Receive 1 WellPoint for each 32 oz. of water consumed and logged on the wellness portal I took the stairs instead of the elevator Self-report taking the stairs as a Healthy Habit on the wellness portal Star Theater Relaxa on Session Programs are comprised of soothing music, accompanied by various types of visual imagery at the Space Science Center. Challenges Wellness Challenges will be offered throughout the year and WellPoint values will be announced for each challenge Plaque Index Test performed by MSU s Dental Hygienist to check for plaque build up and other dental issues
