1 Expression of Interest Brisbane North PHN Community Advisory Committee PART A: About the Brisbane North PHN The Brisbane North PHN supports clinicians and communities in Brisbane s northern suburbs, Moreton Bay Regional Council and parts of Somerset Regional Council. It covers approximately 4,100 km 2 of urban, regional and rural areas, with a population of over 900,000. It is one of 31 PHNs across Australia, all of which came into being on 1 July The key objectives of the PHN are: increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes improving coordination of care to ensure patients receive the right care in the right place at the right time. It will achieve these outcomes by: understanding the health care needs of our community through analysis and planning, helping to identify and address service gaps; providing practice support services so that GPs are better placed to provide care to patients, keeping them healthy and out of hospital; supporting general practices in attaining the highest standards in safety; assisting general practices in understanding and making meaningful use of ehealth systems; working with other funders to purchase or commission health services for local groups most in need, including patients with complex chronic conditions or mental illness. 2 PART B: About the Community Advisory Committee Purpose and functions The Brisbane North PHN is forming the Brisbane North Community Advisory Committee (the Committee), as a forum to bring together diverse perspectives across a range of population and disease groups. The Committee is particularly focused on those populations with unmet health needs who contribute to avoidable use of hospitals, and diseases that are chronic and contribute to significant avoidable hospitalisation. The purpose of this committee is to represent the communities of the region and assist the PHN with strategic input, planning and communications, as well as the development of local clinical pathways. Governance, membership and term The Committee plays an important role in providing recommendations and input to the PHN Board of Directors. Membership of the Committee will consist of between 7-12 individuals living, working or receiving services in the Brisbane North PHN region. Members will be drawn from targeted communities in the region, which may include: Older people; People with a disability; People with a chronic condition; People with mental illness; People from Culturally and Linguistically Diverse populations; Aboriginal and Torres Strait Islander people; LGBTI people; Children and Young people; Homeless people; Geographic areas of concentrated disadvantage. Membership shall be actively reviewed by the Committee and the PHN Board on a regular basis, with members serving a term of one year. Members may be reappointed at the conclusion of each term. Selection Process and Criteria Members will be selected by the PHN Board. Applicants must be supported, by way of a reference, by at least one relevant stakeholder organisation known to the PHN. Applicants must demonstrate: an understanding of the health needs and current priorities of targeted communities; established networks across the region among these targeted populations; and an ability to engage constructively in a diverse committee environment or willingness to learn same. Applicants will be selected as individuals, not as representatives of any organisation. The PHN reserves the right to invite and remove members of the Committee from time to time, but will seek advice from Committee members before doing so.

3 Remuneration Members will be remunerated by the PHN for meeting attendance time and agreed preparation time. If members are remunerated by other organisations for their time while on the Committee or preparation time, then no further remuneration from the PHN shall apply. Members may also be eligible to receive reimbursement of agreed expenses upon application. No payment will be made for travel time. Meetings The Committee will meet three times a year. Working groups may be formed from time to time to progress specific initiatives in between regular Committee meetings. Members will also be required to participate in PHN induction activities and will be strongly encouraged to attend PHN forums held throughout the year.
