1 Supervisory Learning Series Application Materials Spring 2016 Cohort Spring Cohort Application Deadline: December 4, 2015
2 Selection and Notification Notification to Applicants will be via regarding the status of their application no later than one week after the application deadline. Selection is based on: Stated goals Departmental representation Demonstrated supervisor support Participation and Attendance Guidelines Attendance at all sessions is mandatory. The schedule is provided so that you may plan regular work-related commitments, professional conferences and vacations in advance to avoid conflict with program dates. Excused Absences In the event of illness, loss of a loved one or unexpected workplace emergency, the participant will inform Learning Services of her/his absence via telephone ( ) or (stephen.lunna@uvm.edu ). After your return to work, we will assist you in making up the course work as appropriate. Unexcused Absences Examples of unexcused absences include vacation plans, conferences, meetings and workload issues. Missing sessions for unexcused absences will result in needing to begin the cohort again in the future. Note that in the event a participant moves into another cohort, all core classes will need to be retaken. Inclement Weather and University Closing The sessions will occur as scheduled unless the University announces an official closure in which case the affected session will be rescheduled. Requirements The total time requirement for the Supervisory Learning Series includes approximately 24 hours of classroom time and significant independent work. It is imperative that you fully understand the time commitment required, prior to signing up for the series. Attend and fully participate in all required sessions Attend Preventing Sexual Harassment and Bias (NEO002) Attend two elective sessions Complete all required homework assignments, online courses and independent tasks Complete a project and participate in the poster session 1

Please limit your answers to no more than three pages. How will this program assist you with challenges you are facing as a supervisor? Be specific. How does this supervisory program align with your professional or personal goals? How do you envision that it will assist you in reaching those goals?
