1 AUTHORISTION For all Jobs seek approval from Divisional Finance Manager. VACANCY REQUEST AUTHORISATION Complete TRAC Authorisation - Vacancy Request - (Guidance can be found on One HR - - Process - Vacancy Requisition). Authoristation required by: 1 - Finance 2 - HR and ADOP 3 - Please note: For posts that are converted into an Apprenticeship (True) post please can Appointed Managers contact their Divisional Finance Manager (DFM) to create an apprentice position number. For Trainee Annex 21 conversions please can the recruitment team inform Kath who will contact Catherine Daly - Hindmarch and advise a start date. Finance HR and ADOP

2 Is this post duration appropriate length for an Apprenticeship? Level 2 Apprenticeships require fixed term contracts for mths (Depending if there is a standard or a framework). Level 3 Apprenticeships require fixed term contracts for 24mths. Higher qualifications please contact to discuss duration of the qualification. Trainee Annex 21 post are permanent postions unless Managers state otherwise. PLEASE NOTE ALL APPRENTICESHIP POST INCLUDING TRAINEE ANNEX 21 WILL REQUIRE 20% OF THEIR EMPLOYED TIME DOING OFF- THE -JOB TRAINING. THIS IS GUIDED BY THE TRAINING PROVIDER. Has the correct Apprenticeship wage been included in the advertisement? Apprentices any age in their first year of their apprenticeship will be entitled to minimum hourly rate of 3.50 (2017 National Minimum Wage for Apprentices) 3.70 (from April 2018 National Minimum Wage for Apprentices). Apprentices that have completed their first year are entitled to the minimum wage for their age if they both: Are aged 19 or over Have completed the first year of their apprenticeship Trainee Annex 21 Apprenticeship post- See Trainee Annex 21 guidance notes on One HR under Placement & - Apprentices - Trainee process Annex rates. Has the eligibility statement been included in the advertisement? Statement to be included - Due to the current Government eligibility criteria for Government funded apprenticeship schemes, we are currently only able to accept applications from candidates who have been resident in the UK for the last 3 years, and who are not already in any form of Government funded education. Has the BKSB assessment statement been included in the advertisement? Statement to be included - An initial assessment, will be completed by the sucessful candidate after the interview to ascertain what Level Numeracy, Literacy and ICT you are currently working at to ensure you are eligible to undertake an apprentice programme.

3 PRECHECKING Has the Apprenticeship qualification been included in the advertisement? Statement to be included - As part of the Apprenticeship you will be expected to undertake a ** Apprenticeship in**. qualification. ADVERTISE True Apprenticeships and Trainee Annex 21 post to be advertised on NHS jobs and Blackpool Teaching Hospital - One HR- Current Vacancies SHORT-LISTING to Shortlist. INTERVIEWING to follow the process. can assist with Interview questions if required.

4 APPOINTMENT/CONDITIONAL OFFER to follow process. to inform the details of the appointed Apprentice. will contact the Training Provider to inform them of the appointed Apprentice and negotiate the price of the Apprenticeship. Department to include in the conditional offer - BKSB assessment must be completed within 2 weeks of receiving the conditional offer. The BKSB assessment will take place on...( officers will add the next available date, time and venue of the BKSB assessments). Please bring along evidence of qualifications, this is for the Training Provider to ensure you are eligible to undertake an apprenticeship programme. Please can you contact or to confirm your attendance for the BKSB assessment date. to inform when BKSB completed and the results. PRE EMPLOYMENT CHECK to follow process. UNCONDITIONAL OFFER/CONTRACT & START to follow process. to inform of the Apprentice start date as soon as this has been agreed. It is the responsibility of the line manager to complete the necessary assignment change form to increase the rate of pay on an annual basis.

5 DIGITAL ACCOUNT SYSTEM (DAS) update the DAS when the unconditional offer and start date has been agreed. obtain information for the Apprentice from : Date of Birth Full Name TRAINING PROVIDER will provide the a sign up date for the apprentice to attend a sign up session (within 2 weeks of the apprentice starting employment) in order to complete the necessary sign up apprenticeship paperwork. Team to attend the sign up session to sign the apprenticeship paperwork. Training Provider HEALTH EDUCATION ENGLAND - NAM DATABASE to complete the NAM database. This information is procesed from the apprenticeship paperwork.. HEALTHROSTER Appointing manager to ensure that the apprentice Healthroster is updated when the apprentice is attending apprenticeship training.
