2 DAY 1-28 th September 2016 Minutes and Action Items from April meeting, Steve Larder, BG Last Forum s Review of meeting minutes from Houston HSE forum IOGP Activities and Updates (Statistics, other workgroups, 423) Carvalho, Mariana, IOGP IOGP 432 update Patrice Le Garrec & Joanna Oustad Marine related topics Shariman ABDUL HALIM, Total Review of safety performance data, 49 of 57 members reported ( 20 members contributed to 90% of hrs), 3719 mil manhrs (-15% than 2014). 54 (12 company and 42 contractor ) fatalities reported, FAR= 1.45, 41% higher than in Largest category was explosions or burns followed by caught between. 2 fatalities related to seismic and 7 to land transport, not wearing seatbelt was the main cause. 423, aim to publish Jan 2017, in line with 510, focus on risk and assurance, working together and contractor responsibilities Final draft target Dec 2016, aligned with 510 and 423. Flexibility wrt Crew HSE and Project Plan, content rearranged to improve flow land vs marine. Many references that need to be managed. Focus on 1) marine competency & experience safety critical activities; close passes, emergency towing, coming alongside, small boats, personnel transfer. 2) maintenance equipment must be operated in a safe condition; maintenance process (PMS), safety critical equipment, spare parts management. Assurance steps defined for both at each phase. In a case of serious event- contractor status can be affected
3 Last Forum s DAY 1-28 th September 2016 continued continuing New approaches for subcontractors management- Michael Poltz, Geokinetics Seismic vessels safe towing capacity Erwan L Arvor, Total Leadership and safety culture- Hugh Campbell, Total Presented by Patrice Le Garrec on 28 th Sept. US market commoditized, smaller and private clients, few know of IOGP, most services are contracted. Capability assessment via an online internal qualification rather than audit process. Risk control ; management meeting (post award), activity specific start up with focus on each task (at site), positive behavior program, performance monitoring. Mobilization and kick off activity more critical than ever. Larger spreads increased risk of streamer tangles safe towing capacity: max number of streamers a vessel can tow safely in a given configuration (w/technical limitations). Determine; hydrodynamic drag/power thresholds, deck capacity ( max cables = streamer winches & tow points), propulsion capacity, safe towing capacity. Tool to select a vessel within their operational limits to perform safely in normal operations Focus on working closer with contractors, they contribute to Total s performance (75% of working hours, 90% of injuries). Culture based survey conduced resulted in focus areas: 1) look deeper into human and organizational factors for safety, 2) leadership need to be reinforced to reach an integrated safety culture 3) importance to work more in a partnership model with contractors

United nations 2030 sustainability agenda, IPIECA, Dow Jones Sustainability indices Doing no harm is no longer sufficient, expectation of participatory approach delivering a proven positive contribution to local communities CGG priorities defined by relevant by external stakeholders vs impact level for CGG. This panel was kicked off by a presentation about Do we need health checks and why? in today context: ageing population, obesity and most recurrent diseases (top 1: cardio-vascular) and associated business risks (e.g. costs & schedule) and organizational risks (e.g. duty of care, CSR and liability). As 32% of the hospitalizations lead to medical evacuation, there is an opportunity to improve medical screening. The risk of medevac increases when the country medical risk increases. The panel emphasised on the importance to adjust the screening criterion based on the country risks (yc. facilities) and remoteness.

Over 120 reported incidents, robbery/ theft most common, followed by kidnapping, acts of terrorism and other. Incidents involving firearms / weapon (23) with shorts fired (11). 2 fatalities. Assaults and violent acts most fatal US- permits in limbo for 1.5, GOM settlement agreement in 2014 with NRDC, non scientific measures included. NOOA/ NMFS, endangered species. Canada- FORRI Frontier Offshore Regulatory Renewal Initiative, OHS Occupational Health and Safety initiative. Mexico Social impacts regulations (SENER), working on providing exemption/ scaled down onshore seismic, Management System Regulation (ASEA), seeking exemption from the regulation. Uruguay recently released draft offshore mitigation guidelines, IAGC developing comments, Brazil- reduced activity, Ireland- Non-scientific mitigation measures, General challenges : Increased engo pressure, Onshore and offshore restrictive designations, Abuse of the court systems and laws to limit seismic activities, Public perception

Be careful, apply for permission Recent incidents resulted in grounding of fleet, Superpuma will not operate again in NS, 4 serious incidents since Helioffshore ( formed w/ 4 work steam; system reliability and resilience, operational effectiveness, safety enablers, survivability. Assurance standards and requirements IOGP 390 &420, through audit process, competent operator contract holders, mature HSSE processes and risk awareness IOGP 390- pilot experience ( commander 2000hrs TT, co-pilot 500hrs TT)- many pilots not compliant. Risk reduction- V2V transfer, No Medevac at night / No Medevac, man portable drills, pre-load (no casing), Airline assessment ASAM Flight safe ( )

Primary and secondary safe systems: seat belts, curtain air bags, electronic stability control, speed limiters. Considerations for ROPS: vehicle selection, speed limits, road conditions, driving exposure, local driving conditions. Types of ROPS; internal, external, internal/ external. Should have no effect on existing systems; crash structures, seatbelt mountings, dashboard airbags, curtain airbags. Manufacturing quality important, little legislation exists Draft document has been finalized, and reviewed by IAGC. In the process of being reviewed by IOGP. 6 companies presented HIPOs. Will be posted on IAGC website. Eelco presented the significant update of the database : events added yc. all IOGP offshore helicopters crashes) and - new functionalities. Then, analyses from accidents were presented; for details, refer to the presentation available. Finally, the updated risk matrix was introduced (IOGP 432 supplement 02)
