QUESTIONS & ANSWERS: REVISED EXHIBIT C PRICE PROPOSAL PAGES 5 & 9 ONLY, SEE BELOW. 1. Will storm water control devices be required for scarifying and backhoe operations? Answer: No 2. Will NPDES NOI permits be required for scarifying and backhoe operations? Answer: No 3. Can we visit some typical sites? Answer: 1821 N Andrews Ave., Pompano Beach 1301 & 1307 S. SR 7, Hollywood 1041 & 1065 W. 8 th St, Riviera Beach 4. Does debris removal include all trash and debris removal associated with all items in the contract? Please specify. Answer: See definition of Debris in Exhibit A, Page A-1 5. Please clarify pruning and trimming pay item and the description of work i.e. 0-4 wide. Answer: Width will be determined by DBH 6. Can you please provide the historical data for the current contract? Answer: This should be via a public records request, clarifying the specific data they are looking for. 7. Can you please send a map showing the locations of the parcels/lots that are a part of the scope of services? Answer: PREVIOUSLY PROVIDED IN ADDENDUM NO. 1 We will not be providing Right of Way (R/W) maps for this advertisement as the work needed will be for indefinite quantities at locations that are indefinite-it is a pushbutton type contract-when we need work done anywhere within the District, we will push the button and task the contractor with the work. Our R/W maps are project specific and this is not a project specific contract, thus we are unable to provide R/W maps. This Districtwide contract will cover work that may be needed in any of the five counties that comprise District 4 (Broward, Palm Beach, Martin, St. Lucie and Indian River counties), as shown on the map below: 1

Answer: Yes, Ag-Scape Services, Inc. 11. What is the current contract amount for this contract? Answer: The current contract award amount is $1,000, The current amount spent $370, Is there a budget for this contract? Failure to provide Exhibit C - Bid Sheet and documents requested will constitute a non-responsive and shall not be considered. Alteration to this Exhibit C Bid Sheet, Price Proposal, will in effect cause this Bid to become a Conditional Bid and will be found non-responsive and shall not be considered. Do not create or replace the Department s Exhibit C Bid Sheet with your own version. Fill in all blank areas with a dollar figure. A firm price for each blank on Exhibit C - Bid Sheet shall be provided by the Bidder. If a 0 (zero) is entered, then that is the fee the Bidder will receive if contracted to do the work. If a space is left blank, a space is marked N/A or the Department s Exhibit C Bid Sheet is not utilized by the Bidder, the Bid will be considered non-responsive. 1.0 BID ITEMS I. Mowing Services a. Routine mowing for each parcel: 1) 1 2,500 square feet; 2) 2,501 5,000 square feet; 3) 5,001 10,000 square feet; 4) 10,001 20,000 square feet; 5) 20,001 30,000 square feet; 6) 30,001 45,000 square feet; 7) 45,001 75,000 square feet; 8) 75, ,000 square feet. b. 4 The mowing services item category and the total number of square feet to be maintained shall be computed and agreed upon by the Vendor and the Contract Manager, prior to the issuance of any Letter of Authorizations (LOAs) and with approved encumbrance(s). The total fee for each Letter of Authorizations (LOA) shall be computed by determining which square footage size is appropriate (this is for the area of grass/vegetation only, not the entire lot size) under the correct mowing service item category to be performed (mowing, scarifying, or backhoeing) for each parcel to be maintained and adding those amounts to arrive at a total amount. NOTE: The LOA can also include tire disposal, debris removal, and/or tree services. II. Tire Disposal & Debris Removal a. Fee for tire disposal per tire; and b. Fee for debris removal per cubic yard. The total fee for each Letter of Authorizations (LOA) shall be computed and agreed upon by the Vendor and the Contract Manager, prior to the issuance of any Letter of Authorizations (LOA) with approved encumbrance(s). The total fee per Letter of Authorizations (LOA) shall be computed as follows: Multiply the number of tires to be disposed of by the unit price. For debris removal, multiply the total number of cubic yards of debris to be removed by the unit price. NOTE: The Letter of Authorizations (LOA) can also include mowing or tree services. III. Tree Removal, Stump Grinding and Pruning Services a. Fee for tree removal: 1) 0 12 inches DBH; 2) inches DBH; 3) inches DBH; 4) inches DBH; 5) 49+ inches DBH; b. 5 The tree services item category and the total amount to be removed shall be computed and agreed upon by the Vendor and the Contract Manager prior to the issuance of any Letter of Authorizations (LOAs) and approved encumbrance(s). The total fee for each LOA shall be computed by determining which size is appropriate under the correct tree service item category to be performed (tree removal, stump grinding or pruning and trimming) for each parcel to be maintained and adding those amounts to arrive at a total amount.
