All other aspects of the Contract Documents remain in full force and effect. Acknowledgment of this addendum is required on the Bid Form. This addendum will NOT extend the date of the Bid Opening. This Addendum includes the following additional information: A. Revised Bid Form (4 total pages): Additional alternate bid items have been added to the Bid Form. Bid Items #A2-1 and #A2-2 have been added to the project as alternates to Bid Item #2 for the deep installation of water line 48 below the elevation of the pavement of State Route Y. The alternates will be for boring this length of pipe in lieu of deep installation. In addition, the estimated quantity of linear feet for Tree Clearing (Bid Item #35) has been revised based on additional areas of tree clearing needed on the Basinger and Leeclair Smith Properties near Forsee Road. The original Bid Form has been replaced entirely with the attached revised Bid Form in which the top of the pages show ADDENDUM NO. 1. This revised Bid Form shall be used for the submission of Bids. B. Revised Project Plan Drawings C04 and C05 (2 sheets total): Additional clarification callouts and notes have been added to Project Plan Drawings C04 and C05 regarding the tree clearing and an open-cut creek crossing. The estimated tree clearing limits are now shown on the Plans. Revised Plan Drawings C04 and C05 are attached to this Addendum and will be distributed to each planholder. The original Plan Drawings C04 and C05 have been replaced entirely by the attached revised Plan Drawings C04 and C05. C. The minutes and sign-in sheet from the Pre-Bid Conference have also been attached to this Addendum for reference (4 total pages). D. Bid Opening will remain as the original date of Thursday, November 2, 2017, at 10:00 A.M. Local Time.

Project Description The project includes installation of approximately 10,800 feet of 12 C909 PVCO water line installation and 760 feet of 8 C909 PVCO, including valves and appurtenances. The project also includes an encasement bore under State Highway 63 and Forsee Road, and a bore under Peterson Lane. 2. Contract Award and Schedule A. Bid Opening November 2, 2017, at 10:00 A.M. B. Bid Tab presented for Contract award at the Board meeting Novmeber 9, 2017 C. Contract Approval and NTP at Board meeting December 14, 2017 D. 150 consecutive Calendar days to complete project, $ liquidated damages for each consecutive calendar day after the Completion Date E. Prevailing Wage Rates for Boone County F. Water District will provide Contractor with Sales Tax Certificate. 3. Rights-of-Way A. The easement widths vary from 20 feet to 30 feet wide. The property lines or edge of easement are staked in the field. B. The waterline is staked out for the entire project for viewing and construction. C. The Water District has obtained the MoDOT Permit for the Highway 63 and Forsee Road Encasement Bores. Contractor will need to be bonded with MoDOT. D. County Road Crossings Permit (573) shall be obtained by Contractor for open cut Forsee Road. E. City of Ashland Crossing Permit (573) shall be obtained by Contractor for Peterson Lane crossing and work in the Old Highway 63 Right of Way. F. The Water District is working on the Army Corp of Engineers Permit Application for Ravine/Creek Crossing in the wetland area. 4. Specific Construction Considerations A. On the Forest Lake Inc. property the water line must be installed at a minimum of 4 feet below edge of State Route Y. There is a bid item for deep installation which is shown as a plus/minus on the waterline plans. B. Tree Removal shall be paid as a separate line item by lineal feet. This pertains to Forest Lake Inc., Basinger and Smith properties. (Plans only show on Forest Lake Inc. Property) 1500 North Seventh Street, Columbia, MO

8 C. The Smith Culvert Pipe Replacement must be dug up and replaced in kind. D. There will be up to 4 water line test sections. Three on the 12-inch waterline and One on the 8- inch waterline. E. Location for storage of materials will be determined prior to notice to proceed. F. Coordination with other Utilities (MO-One Call). 5. Pay Items A. The pay items have been broken down into individual fittings and ancillary items. Unless identified on the plans or in the measurement and payment section of the documents, the item will be paid individually and not in combination with a complete installation. For example, a fire hydrant installation will have a 6 branch tee, anchor couplings, gate valve and box, rock, concrete thrust blocking, and hydrant. Each of those items is paid for separately. B. Solid rock excavation will only be measured and paid for when rock is encountered in open cut trench excavation. 6. Bored Pipe A. The State Highway Crossing of Highway 63 and Forsee Road payment is based on casing length. B. There will be a bore crossing Peterson Lane and a bore crossing the asphalt driveway on the Basinger Property. C. We don t anticipate any rock at the bore locations, but no borings were taken. D. The pipe and any open cut trench rock excavation on either side of the bore, if any encountered, will be paid for separately. 7. Seeding A. Temporary seeding may be necessary in some areas of the project. The application of temporary seeding will be as directed by the District Inspector. B. Permanent seeding is required upon completion of the work. C. NEW - Pursuant to General Condition GC-29 Guarantee, a deposit will be held by the District. Refer to Supplemental Conditions Subsection 6. This will allow the contract to be fully closed out with only the one-year warranty deposit in effect for defective work and the deposit held until site restoration and revegetation has occurred to the satisfaction of the District. 8. Miscellaneous A. The Water District will not be providing any fire hydrants on this project. All hydrants to be installed will need to be approved hydrants (per specifications) provided by the Contractor. These hydrants will be paid for as part of Bid Item #32, Furnish and Install Fire Hydrant Assembly. B. Flushing, pigging, disinfecting, and pressure testing the existing 8-inch water line along Peterson Lane will be included as an alternate bid item on this project. All temporary materials and labor needed to perform the line cleaning and testing shall be included in the lump sum alternate bid item as described in the Measurement & Payment Section. C. Indiana Gray Bat Tree clearing shall be performed between the dates of November 1 st and March 31 st to avoid endangering Indiana Gray Bat Habitat. D. Addendum No. 1 will be sent out soon. Addendum No. 1 will provide further clarification of tree clearing limits and bore classifications. Addendum No. 1 will also add alternate bid items for boring water line along Route Y in lieu of deep, open cut installation. 9. Questions/Comments - 2 -
