2 Wellness Premium Credit 2018 AISD $100 Wellness Premium Credit I. General Overview Employees on an Austin ISD medical plan must earn a total of 100 points prior to October 28 th, 2018 to earn the $100 Wellness Premium Credit on their December 2018 paycheck. Points are earned by completing the required activities plus your choice of additional activities offered throughout the year. Q. What is the ewellness Program and the Wellness Premium Credit? A. ewellness is Austin Independent School District s employee wellness program. The program strives to enhance the overall wellbeing of all AISD employees by promoting and implementing healthy lifestyle programs, thus creating a positive organizational culture that values every employee. The Wellness Premium Credit is designed to engage employees in taking a more active role in managing their health and supporting them in making positive health changes as they work to reach personal health and wellness goals. It does this by offering a variety of health and wellness challenges, wellbeing classes, and wellness initiatives that give employees the opportunity to earn incentive points which can help lower their medical premiums. Eligible employees can earn a $100 Wellness Premium Credit. Q. Why is Austin ISD offering this? A. Your health and wellness are important to Austin ISD. These programs are being enhanced to encourage our employees to take a positive, and rewarding, step towards living a more healthy, balanced and active lifestyle. Austin ISD is committed to the development of the Whole School, Whole Community, and Whole Child. This means taking care of our employees so they may best take care of our students and community. Q. Why should I participate? A. In addition to getting healthier and learning little ways to make positive lifestyle changes, eligible employees can also earn the $ Wellness Premium Credit on their paycheck at the end of the 2018 calendar year. Q. How do I get started? A. The path to better health begins at where you can track your total points, register for solo and team challenges, see the wellbeing class schedule, and more. If you haven t already done so, take a look around and explore the portal and all it has to offer! Create your free account with your Employee ID #. 2

3 Q. Is participation mandatory? A. No. While participation is highly encouraged, it is completely voluntary. Always consult your physician before participating if you have any questions or concerns. Q. Are there any required activities? A. Yes. To be eligible to receive the $100 Wellness Premium Credit, the employee must have earned a total of 100 wellness points before the deadline of October 28 th. Required activities include completing a biometric screening and the Health Risk Assessment. Completion of those two activities will earn the employee a total of 70 points. The employee can then choose from several challenges, wellbeing classes, and other events throughout the year to earn the remaining 30 points needed to earn the $100 Wellness Premium Credit. Q. What if I have a medical issue that prevents me from fully participating in the program? A. If participating in certain programs is unreasonably difficult, or not medically advisable due to a medical condition, please notify the AISD Wellness Coordinator in the Benefits Department. We will work with you to develop another way to qualify for the incentive. Q. Who do I call if I have questions about the Wellness Premium Credit? A. Contact the Austin ISD benefits office to speak with your benefits technician or contact AISD s Wellness Coordinator, Melody Carlton if you have questions. II. Eligibility Q. Who is eligible to receive the Wellness Premium Credit? A. Eligibility Requirements 1) You must be enrolled in a AISD sponsored, Aetna medical plan 2) You must be an active, salaried or hourly employee. Q. What if I complete all requirements, earn a total of at least 100 points, but go out on leave during the month the Wellness Premium Credit is paid out? A. Employees who meet all eligibility requirements and earn the 100 points required for the Wellness Premium Credit but are out on district approved FMLA leave, will still receive the $100 Wellness Premium Credit. III. Privacy Q. How does Aetna and Live Healthy AISD protect my personal information? A. At Aetna, the privacy and security of your personal information is top priority. No one at Austin Independent School District will be provided with your specific personal health information, biometric screening results, health assessment responses or results from specific wellness challenges/initiatives. In addition, their technology embraces the highest internet security protocols ensuring your information will not be compromised. 3

4 Q. Does Aetna have to comply with HIPAA and state privacy laws? A. Yes. The information you provide is Protected Health Information and subject to all of the requirements under HIPAA and applicable state privacy laws. Q. Does Aetna share my individual Health Assessment responses or Biometric Screening results with Austin ISD Benefits or Wellness employees? A. NO. No one at Austin ISD will have access to your specific Health Assessment, Biometric Screening or healthy activity results unless you volunteer the information. Q. What information does Aetna or Live Healthy AISD share with Austin ISD? A. Aetna and Live Healthy AISD will track and provide Austin ISD with the total number of points you have earned towards the Wellness Premium Credit in order to facilitate payment. In addition, Aetna and Live Healthy AISD will provide de-identified and aggregated health reports for use in understanding the health of our population as a whole and to help determine appropriate health management programs to offer. Q. Why do you ask for personal information during the registration process? A. In order to ensure your privacy, Aetna wants to validate some personal data to confirm your identity and eligibility to participate in the Wellness Premium Credit program. IV. Accessing the Live Healthy AISD Wellness Portal Q. How do I access the Live Healthy AISD Wellness Portal? A. You can access the Live Healthy AISD portal directly at Employees will create their free account using their Employee ID number for the unique ID. (No e just the 6 digits). Employees who download the AISD Benefits App can also access the Live Healthy AISD portal through the app. Q. Who should I call if I am having trouble registering or having technical issues with the wellness portal? A. You can contact Live Healthy directly if you are having technical issues or trouble registering your account. Their customer service line is You can also always call the AISD Wellness Coordinator, (515) Q. What is the Live Healthy AISD Wellness Portal for? A. Employees can use the Live Healthy AISD (LHA) Wellness Portal for a number of things. This portal will be the main hub for all district wellness challenges, which are one of the ways employees can earn points towards their $100 Wellness Premium Credit. Employees will sign up for challenges and track participation through the Live Healthy AISD portal. The LHA Wellness Portal will also be where employees can track how many total points they have earned so far towards their goal of 100 points for the Wellness Premium Credit. Note* As challenges, classes, and events happen we have to upload the participation roster into the portal. This may cause a delay from when you complete a wellness incentive activity to when you see your points added into your account. Finally, the LHA Wellness Portal will also give employees access to hundreds of healthy recipes with nutritional information as well as over 100 workout videos that employees can enjoy anywhere. 4

5 V. Wellness Premium Credit Rules Q. How do I earn points? A. Employees can earn points by completing healthy activities. The two required activities are completing a Biometric Screening and completing your Aetna Compass Health Assessment. Completing these two activities will give the employee 70 points. Then employees may choose how they earn the remaining 30 points required to reach 100 total. Activities to choose from will include Wellness Challenges, Wellbeing Classes, and participation in the Cap10K race in April. To view a comprehensive listing of all incentive opportunities, visit the AISD wellness website at You can also download the AISD Benefits App and see all opportunities to earn Wellness points there. Q. How do I view my activity history/ how many points I have? A. You can view your completed activities and total points on the homepage of your Live Healthy AISD Wellness portal. Remember there may be a short delay from when you complete an activity to when we upload participation records into the system. Please allow time for the system to be updated to reflect your newly earned points. Q. How do I earn the $100 Wellness Premium Credit? A. If you are eligible for the Wellness Premium Credit program (as described on page 4 of this document), you must earn 100 wellness points before the deadline of October 28 th. Employees must complete the required Biometric Screening and Aetna Compass Health Assessment, which will earn the employee a total of 70 points. Then the employee must earn 30 more points from the additional wellness activities offered throughout the year. Q. What if I earn less than 100 points? Will I get that amount of incentive credit instead? A. NO. The Wellness Premium Credit program is all or nothing. Employees must hit the total 100 points to earn the $100 credit. Q. How many points can I earn each year? A. As many as you want! Completing the two required activities (Biometric Screening and Health Assessment) will give you 70 points. After that employees must earn at least 30 more to hit the 100 goal and earn the Wellness Premium Credit. However, the real prize is that you are taking steps to better your overall health and wellbeing. Employees can and are encouraged to continue to participate in any wellness challenges, attend Wellbeing Classes, or other ewellness activities even if the employee has already accumulated the necessary 100 points. There will be opportunities for other giveaways during each wellness activity. Q. When/How will I receive the $100 Wellness Premium Credit if I earned the required 100 points? A. Eligible employees who complete all required activities and earn 100 or more total wellness points will receive the $100 Wellness Premium Credit on their December check. The $100 credit will be applied in the form of a medical premium offset. Q. How will I receive my Wellness Premium Credit if I go on short term disability or other approved leave of absence? A. Eligible employees who complete all required activities and earn 100 or more total wellness points will still receive the Wellness Premium Credit, as if you were actively at work, with their December paycheck, provided you are receiving a paycheck from the AISD. 5

6 Q. What can I earn points for? A. There are a variety of activities an employee may earn points for. Employees must complete a Biometric Screening and their Health Risk Assessment to qualify, and then can choose from various physical, nutritional, and mental/emotional wellness initiatives to earn the remaining 30 points needed to hit 100 points and earn the $100 Wellness Premium Credit. Q. What is the deadline to have earned 100 points and receive the $100 Wellness Premium Credit? A. Biometric screenings will be offered onsite at each campus beginning January-June. The Biometric screening is a required activity employees must complete to earn the credit. The Health Risk Assessment and all other additional activities must be completed by October 31 st, 2018 to earn the Wellness Premium Credit on your December check. Q. I believe I completed healthy activities to earn the Wellness Premium Credit, but I do not see those points reflected on my Live Healthy AISD portal? A. If you believe you completed an activity but have not received the points please contact the AISD Wellness Coordinator, Melody Carlton at melody.carlton@austinisd.org or (512) and we will look into it. 6

7 VI. Biometric Screenings Q. Who can participate in a Biometric Screening? A. Biometric Screenings follow the same eligibility rules as the Wellness Premium Credit: 1) You must be enrolled in a AISD sponsored, Aetna medical plan 2) You must be an active, salaried, hourly, AISD employee Q. Do the Biometric Screenings cost money? A. No. The district has covered the cost to provide these health screenings to encourage employees to BE WELL. Q. Where do I complete my Biometric Screening? A. Every campus principal will have the opportunity to schedule 1-2 (depending on size of school) screening days onsite at their campus. Employees at that campus will be able to schedule a 15 minute time slot to complete their Biometric Screening. An appointment is REQUIRED to complete your biometric screening at your campuses clinic. Walk-ins will not be allowed. If an employee misses their campuses onsite screening date or if the campus elects not to have an onsite screening date, employees have a second option to complete their Biometric Screening at a Quest Patient Service Center on their own time. (Several locations throughout the Austin area.) An appointment is still required for employees wishing to complete their screening at a Patient Service Center. Q. Do I need to fast prior to the Biometric Screening? A. No. These screenings are designed to be non-fasting screenings, which means you can have eaten prior to completing your screening. Q. What does a Biometric Screening include? A. Biometric Screenings at your campus require you to make an appointment and take between minutes to complete. During the screening you will measure the following things with a finger-stick test. (This means they prick your finger to collect a small blood drop.) The Biometric Screening will measure your Total Cholesterol, HDL, TC/HDL ratio, Glucose, Height, Weight, Blood Pressure, and BMI calculation. The onsite coordinator will go over your results with you in person upon completion. Biometric Screenings completed at a Patient Service Center will measure all the same health components as listed above. The only difference is Patient Service Center s use a venipuncture method, meaning they will draw blood from a vein instead of a finger prick, to complete your screening. An Appointment is still required. Q. How do I schedule my Biometric Screening? A. All Biometric Screening appointments can be made online or over the phone. Scheduling Online Instructions 1. To schedule online AISD employees can go to and click Register Now to get started. 2. Then enter our registration key, austinisd and click continue. 7

8 3. Under Eligibility Verification enter your Unique ID for AISD employees this is your Employee ID# in the format , (no e). Then create your account with a username and password. 4. Fill out the Personal Information Section and click continue. 5. Read through the terms and consent page and click I accept. 6. Once on the Welcome Page, click Get started to schedule. 7. This takes you to the screening page where you will click Participate Now 8. Then choose. If you are scheduling your appointment for your Campuses Clinic Day you will click on Make an Appointment under the At An Event column. If you are choosing to schedule your screening at a Patient Service Center on your own time, click Schedule a Screening under the At A Patient Service Center column. 9. You will then select your location (either your campus or whichever Patient Service Center location you d like) and the day & time for your appointment. Then click next. 10. Review the day, time and location you selected and if all accurate click confirm. Scheduling over the Phone Employees can also schedule their appointments over the phone with the Quest Wellness Call Center at Monday-Friday from 7:00am-8:30pm and Saturdays 7:30am-4:00pm. You will still need to provide the scheduler with our registration key, austinisd, as well as your Employee ID number. Q. I just had my annual physical/bloodwork done with another doctor recently. Do I still need to participate in the Quest Biometric Screening to earn the Wellness Premium Credit? A. While participation in the Wellness Premium Credit program is entirely voluntary, to be eligible to receive the $100 Wellness Premium Credit an employee MUST complete their Biometric Screening with Quest at either a campus screening clinic or a Patient Service Center. Q. Can I receive more than one Biometric Screening per calendar year? A. No. Biometric Screenings are provided for eligible employees at no cost, once per calendar year. VII. Erroneous and/or Fraudulent Incentive Credit Q. What are erroneous and/or fraudulent incentive credits? A. Erroneous incentive credits are due to improper administration that when executed result in administrative error(s). Administrative error(s) include, but are not limited to, incorrect data entry, incorrect computer programming or other error(s) during data analysis. Note: Austin ISD reserves the right to audit and seek recovery of erroneous incentive credits, without participants consent, as a result of an administrative error(s). Fraudulent incentive credits arise from a participant s misstatements, misrepresentations, or omissions of information and/or participation. Any participant knowingly falsifying participation in wellness activities constitutes fraud against the district. Reimbursement of the Wellness Premium Credit will be paid back to the district and disciplinary action may be taken. Note: Austin ISD reserves the right to audit and request proof to substantiate credit received at any time. 8
