12 UIQ SOFTWARE FORM The Quiq software form is for adding Quiq Payments to an existing account Complete the merchant s business information Select a package Have the merchant sign the form Submit to Account Changes NOTE: There is a separate form with a blank monthly fee for partners who want to mark-up the MSRP. In all situations, the Gateway Auth Fee cannot be changed.

13 MERCHANT APPLICATION List the equipment in the Equipment section as either Quiq Silver, Quiq Gold, or Quiq Platinum on the Payment Gateway line. Pricing must be completed in the Schedule of Fees section. Insert the fee for the selected package in the Payment Gateway Fees section on the Monthly Access fee line.
