1 REQUEST FOR QUOTATION RFQ Number: AFENET LAB COMSUMABLES 001/LAGAIS For procurement of Health and Non-Health Commodities for use in Lagos, Nigeria Issuance Date: October 5, 2017 Due Date and Time for Submission of Quotes: October 19 th, 2017, 5:00pm GMT Africa Field Epidemiology Network (AFENET) is the umbrella organization for applied epidemiology training programs in Africa. It oversees the Field Epidemiology (and Laboratory) Training Programs (FETP) in over 30 countries of Africa. It has been vested with the responsibility of implementing Lagos AIDS Indicator Survey (LAGAIS). In view of this, AFENET requires prospective bidders to submit quotations for the under listed Laboratory consumables for the field work in the 20 Local Government Areas of Lagos State. 6 Quotes shall remain valid for 60 calendar days from the due date for receipt of quotes. In exceptional circumstances, prior to expiry of the original offer validity period, AFENET may request that the Bidder extend the period of validity for a specified additional period. A Bidder agreeing to the request will not be required to modify their quote. Due Date and Time for Submission of Quotes: Quotes must be received by AFENET no later than the due date and time as shown above and should be duly recorded in the bid submission register. AFENET may, at its discretion, extend the due date and time for the submission of quotes by amending this RFQ. Any quote received by AFENET after the due date and time for submission of quotes will be rejected. Evaluation of Quotes: Quotes will be evaluated based on the following criteria: Overall responsiveness of the quote Conformation to product specifications Total price Timeliness of delivery Past performance Positive feedback on reference check Information for Bidders Clarification of Quotes: During evaluation of the quotes, AFENET may, at its discretion, ask the bidder for a clarification of their quotes. Clarifications are limited exchanges with a Bidder to resolve minor or clerical errors; they do not offer an opportunity for a Bidder to modify or change an offer. Award Criteria: AFENET is not obligated to make an award by having issued this solicitation. AFENET can only make an award if the product fully complies with the technical specifications mentioned in the contract terms. Any award(s) will be made to the responsible bidder whose offer(s) has/have been determined to be most advantageous to AFENET. AFENET has the right to withdraw or change this RFQ at any time. Bidders quotes are done so at the sole cost and expense of the bidder with no obligation on the part of AFENET. Confidentiality: Information submitted in response to this RFQ will be held in confidence. Contract Type: The contract awarded will be on a Fixed Price. Bidders should review the complete RFQ but special attention should be given to the following areas: 1. Ordering Instructions on order processing will be issued at contract award. 2.
