N deplyment services will be ffered with this service. Deliverables Prject Management Plan Netwrk Readiness Assessment: Netwrk Readiness Assessment Questinnaire Template Data Cllectin Wrksheet template Netwrk Readiness Assessment Reprt Custmer Requirements Questinnaire Template(s) Custmer Requirements Reprt Architecture Validatin Reprt (Revised Custmer High Level Design) Site Requirements and Site Survey: Site Survey Templates Site Requirements Specificatin Detailed Design: Detailed Design Reprt Netwrk Implementatin Plan Slutin and Site Acceptance Test Plan Lcatin f Services Services are delivered remtely t Custmer as defined in this Service Descriptin. Prject Management Prvide Custmer with a list f designated Cisc persnnel rles and respnsibilities under this Service. Prvide a Prject Management Plan ( PMP ). PMP is a baseline dcument frm which the Cisc Prject Manager (PM) can manage deliverables, assess prgress, and manage change management issues and any n-ging questins. Wrk with Custmer t identify and dcument dependencies, risks and issues assciated with the successful cmpletin f the prject. Prvide a Prject Schedule highlighting deliverables, crrespnding milestnes, planned prject events, resurce(s) and timescales. Participate in scheduled prject review meetings r cnference calls, if required.

2 Page 2 f 7 Prvide Custmer with the identity f persnnel requiring access t Custmer premises, at least ten (10) Business Days prir t the scheduled date such persnnel requires access. Deliver a weekly prject status reprt t the Custmer. Prvide a handver; fllw n actins, lessns learned, and exceptin reprts (if necessary) upn prject cmpletin. Prvide the Cisc PM with a list f designated Custmer persnnel participating under this Service. Ensure that key Custmer persnnel (such as architecture design and planning, netwrk engineering, netwrk peratins persnnel) are available t prvide infrmatin and t participate in review sessins, wrkshps and ther infrmatin gathering activities. The Custmer PM will als ensure that Cisc is prvided with all infrmatin, data and dcumentatin, as Cisc reasnably required t prvide Services and cmply with Cisc s respnsibilities in this Service. This infrmatin includes, but is nt limited t: (i) infrmatin relating t Custmer s netwrk, design, business and ther applicable requirements; (ii) functinal and/r technical dcumentatin relating t such requirements; and (iii) tplgy maps, cnfiguratin infrmatin and existing and prpsed netwrk infrastructure. Identify primary and backup Custmer authrized site cntacts that shall prvide necessary infrmatin, btain access clearances and crdinate with ther rganizatins/third parties with respect t Services at that site. Participate in scheduled prject review meetings r cnference calls, if required. Crdinate with any external third parties, such as in cuntry Carrier/Telc activities, deliverables and schedules. Ensure that Cisc s request fr infrmatin r dcumentatin needed fr the prject is prvided within three (3) Business Days f Cisc s request, unless the parties agree in writing t anther time perid fr respnse. Netwrk Readiness Assessment Obtain frm the Custmer up t ten (10) sites where the Netwrk Readiness assessment will be cnducted. All remaining sites will be similar in nature t the selected sites. Obtain Custmer s up-t-date netwrk tplgy diagrams fr the sites t be assessed alng with infrmatin abut current netwrk architecture, cnfiguratins f devices, netwrk usage requirements, design gals, security plicy and utilizatin reprts with respect t the scpe and architecture f the prject a) Brderless Netwrk, b) Cllabratin, c) Data Center. Send the architecture slutin questinnaire and the respective white paper related t tls and security t the Custmer cntacts prir t the prject kick ff meeting. Cnduct interviews with key members f Custmer s rganizatin, invlved with the existing technical infrastructure. Review and verify Custmer netwrk infrmatin with respect t the scpe f prject a) business, applicatin, technical, and peratinal requirement, and b) future technlgy plans. Review and analyze the existing sftware/hardware revisins n the netwrk and recmmend the upgrades t sftware/hardware revisins t run the Cisc Architecture Slutin ( Slutin ) that may include: Cllabratin Architecture. Use the Data Cllectin Tls t simulate the traffic n the netwrk and measure delay, jitter and packet lss parameters and make recmmendatins t eliminate/minimize these values t prvide acceptable Quality f Service (QS) n the netwrk. Perfrm an Availability Analysis frm the netwrk design review t identify ptential netwrk design and cnfiguratin issues that culd affect the netwrk resiliency and availability. Prvide the Netwrk Readiness Assessment Reprt t Custmer. Custmer is respnsible fr cmmunicating and scheduling persnnel t attend the kick ff meeting and interviews. Ntify Cisc f up t ten (10) sites where the Netwrk Readiness Assessment will be cnducted. The remaining f the sites need t be similar in nature t the select sites. Prvide Cisc Netwrk Cnsulting Engineers with up-tdate netwrk tplgy diagrams and infrmatin abut the current netwrk architecture, cnfiguratin f these devices, netwrk usage requirements, design gals, security plicy and fllwing utilizatin reprts, with respect t the scpe and architecture f the prject a) Brderless Netwrk; b) Cllabratin; and c) Data Center. Arrange fr security access fr Cisc Persnnel at designate Custmer sites where Infrastructure Readiness Assessment will be perfrmed. Identify the apprpriate staff members invlved at each site and c-rdinate the scheduling fr all meetings and data gathering. Custmer acknwledges that cmpletin f Services is dependent upn Cisc s use f Data Cllectin Tls and/r third party tls. Custmer will prvide the requested infrmatin five (5) Business Days prir t the installatin f the Data Cllectin Tls. Infrmatin requested may include but is nt limited t: seed file, SNMP, Telnet, shw cmmands, syslg server.

3 Page 3 f 7 Custmer agrees t make its Netwrk available fr installatin f Data Cllectin Tls; t prvide a secure area with limited physical access; prvide fr secure installatin behind the Custmer s firewall; and prvide fr access t all devices n the Netwrk. Custmer will ntify Cisc abut changes made t the Netwrk such as Prduct(s) added/deleted and changes made t Prduct credentials, and any changes t Syslg, DNS, prxy and gateway servers IP address. Custmer must immediately return Data Cllectin Tl(s) t Cisc, as instructed by Cisc, upn the earlier f: (i) cmpletin f Services; r (ii) Cisc s request t Custmer that the Data Cllectin Tl(s) be returned t Cisc. Review remtely the Netwrk Readiness Assessment Reprt with Cisc. Custmer Requirements Develpment Obtain frm the Custmer any up t date Custmer strategy, radmaps and planned designs (if existing) dcumentatin fr review prir Custmer Requirements meetings(s) and/r interview(s). Prvide Custmer with the Custmer Requirements Questinnaire Template(s) fr each applicable Custmer Site in scpe f the Services. Review with the Custmer the number f site mdels in scpe fr this engagement. Wrk with Custmer t assist in the cmpletin f Slutin and Custmer Requirements Questinnaire(s). Review cmpleted Custmer Requirements Questinnaire(s) with Custmer t validate the cllected infrmatin. Cnduct (if applicable) ne (1) wrkshp and schedule interviews with Custmer designated emplyee representatives Custmer Requirements meetings(s) and/r interview(s) t gather and dcument the Custmer s business and technical requirements. Cnduct necessary t qualify Cisc s understanding f their requirements and review the Custmer-prvided netwrk design against the requirements prvided in the Slutin and Custmer Requirements Questinnaire. Analyze Custmer requirements and map them t features and functinalities available in scpe f the Cisc Unified Cmmunicatins netwrk requirements. Cmpare the functinal requirements and, if relevant, the call handler flws with the Cisc Unified Cmmunicatin Manager r Cisc Unity available features in accrdance with the Custmer s prvided netwrk high-level design. Create the Custmer Requirements Reprt by incrprating the infrmatin gathered frm the Custmer Requirements sessins and/r interviews. Review remtely the Netwrk Readiness Assessment Reprt with Cisc. Identify and schedule key knwledgeable cntacts that shall be available fr attending remte Custmer Requirements sessin(s) and/r interview(s). Submit t Cisc any up t date business and technical requirements dcuments such as strategy, radmaps, netwrk tplgy diagrams, functinal specificatins, planned designs within ten (10) Business Days f starting the Custmer Requirements sessins and interviews. Prvide the fllwing UC dcumentatin and infrmatin five (5) Business Days prir t the requirements interviews: a). Infrastructure tplgy diagrams including current high level design, lw level design; b). Current UC architecture; c). Cnfiguratin f UC devices including but nt limited t Cisc; d). Unified Cmmunicatins Manager (CUCM), UC endpint and UC gateways; e). Security plicy and security requirements; f). Backup and Restre System (BARS) file r the CUCM Publisher Hard Disk Drive (HDD); g). IP addressing plan, identifying the IP rute summary in the netwrk cre and IP address allcatins fr telephny); h). Security plicy and security requirements. Prvide the fllwing infrmatin five (5) Business Days prir t the installatin f the Data Cllectin Tls: seed file, SNMP, Telnet, sh cmmands, Syslg server. Custmer may nt schedule any Change Management in their netwrk during the requirements review prcess such as: a). Prduct(s) added/deleted; b). Changes made t Prduct credentials; c). Any changes t Syslg, DNS, prxy and gateway server IP addresses. In the event that Cisc Data Cllectin Tls are nt used n the Custmer s CUCM, Custmer must prvide the Cisc Backup and Restre System (BARS) file. Upn cmpletin return immediately Data Cllectin Tl(s) t Cisc, as instructed by Cisc, upn the earlier f: a). Cmpletin f Services r b) Cisc s request t Custmer that the Data Cllectin Tl(s) is returned t Cisc. Make any required mdificatins t firewall rules and/r access-lists required t enable Cisc Data Cllectin Tls t access all devices n the Netwrk. Prvide Cisc with security exemptin(s) in rder t access Cisc r third party sftware n the Netwrk while gathering inventry and perfrmance data, as apprpriate. If Cisc prvides Data Cllectin Tls r scripts lcated at Custmer's site, Custmer shall ensure that such Data Cllectin Tls r scripts are lcated in a secure area, within a Netwrk envirnment prtected within a firewall and n a secure LAN, under lck and key and with access restricted t thse Custmer emplyee(s) r cntractr(s) wh have a need t access the Data Cllectin Tls and/r a need t knw the cntents f the utput f Data Cllectin Tls. In the event Data Cllectin Tl prvided by Cisc is Sftware, Custmer agrees t make apprpriate cmputers available and dwnlad Sftware as needed. Custmer shall remain respnsible fr any damage t r lss r theft f the Data Cllectin Tls while in Custmer s custdy.

4 Page 4 f 7 Make the Netwrk available fr installatin f Data Cllectin Tls including prviding: a). Secure area with limited physical access; b). Secure installatin behind the Custmer s firewall; c). Access t all devices n the Netwrk. Schedule key Custmer stakehlders and prject spnsrs t attend the Cisc s remte presentatin f the Infrastructure Custmer Requirements Specificatin. Review remtely the Custmer Requirements Reprt with Cisc. Architecture Validatin Develpment Cnduct three (3) Business Days f remte interviews with designated Custmer persnnel t qualify Cisc s understanding f Custmer s UC business and technical requirements. Cmpare slutin level requirements identified in the apprved Slutin and Custmer Requirements Reprt with Cisc Unified Cmmunicatins architectures and leading practices. Create and prvide the Architecture Validatin Reprt including: identify and dcument key risks in the Custmer s prpsed high level design, if applicable; define alternate design recmmendatins t help mitigate identified risks; recmmendatins fr UC security design best practices; and identify and prvide recmmendatins fr revising the Bill f Materials, as required. Review remtely the Architecture Validatin Reprt with the Custmer. Prvide Cisc with the names f key netwrking security resurces and current dcumented security plicies that will assist in analyzing the existing netwrk security implementatin. Agree t make n changes t the UC infrastructure during the time perid when Cisc is cnducting data cllectin fr the Architecture Validatin Reprt. Review the draft Architecture Validatin Reprt with Cisc fr cmment and apprval befre Cisc cmmences drafting the final Architecture Validatin Reprt. Review remtely the Architecture Validatin Reprt with Cisc. Site Requirements Survey and Site Survey Template Develpment Review the Slutin and Custmer Requirements Reprt and the revised Custmer prvided High-Level Design dcuments t identify the cmpnent requirements fr each Custmer Site. Develp a Site Requirements Specificatin (SRS) fr the Mdel Sites in scpe represented in the revised Custmer prvided High-Level design. The SRS prvides detailed site specificatins and may include the fllwing: hardware cmpnents; Telc circuits; type f user per site/mdel; slutin firewall and Custmer equipment fr IP Trunking; slutin equipment electrical requirements; envirnmental/physical space requirements; equipment rack and cabinet lcatins and layuts; physical UC security requirements. Develp Cisc Site Survey templates fr design and Custmer requirements, custmized fr each site mdel represented in the updated Custmer prvided High Level Design (HLD). Review remtely the SRS and custmized Site Survey Templates with the Custmer. Schedule the necessary facilities fr remte meetings (such as: cnference rms, prjectrs and cnference bridges). Participate in regularly scheduled prject review meetings r cnference calls. Custmer is respnsible fr perfrming statin review and site surveys. Crdinate any third party activities, (such as in-cuntry carrier/telc activities), deliverables r schedules as required fr Cisc s cmpletin f the services. Ntify Cisc f any scheduling changes related t this prject at least ten (10) Business Days f the scheduled activity. Review remtely the SRS and custmized Site Survey Templates with Cisc. Detailed Design Develpment Review the Custmer Requirements Reprt, Revised Custmer High Level Design (HLD), and Custmer Netwrk Readiness Assessment Reprt. Create the Detailed Design Reprt including: Designing the Custmer s netwrk infrastructure t supprt the Cisc Unified Cmmunicatins slutin which may include the fllwing: prviding guidance and/r review f the Custmer s IP addressing fr vice netwrk devices; reviewing f cnfiguratins and building f the vice template cmpnents t an existing data template fr devices such as Cisc Unified Cmmunicatins Manager, servers and Cisc vice gateways; Develping quality f service (QS)

5 Page 5 f 7 cnfiguratin fr LAN/WAN as it pertains t vice; Defining netwrk services such as DHCP/DNS/TFTP/ NAT related t Cisc Unified Cmmunicatin slutin. Design the Cisc Unified Cmmunicatins slutin which may include the fllwing: Call Manager cluster sizing and design ptins; Designing ptins t prvide Call Admissin Cntrl (CAC) like Lcatins based CAC Gatekeeper based CAC r RSVP; Designing guidance n LDAP directry integratin r directry access; Dial plan architecture and emergency call ruting; Designing ptins t implement Survivable Remte Site Telephny; Designing f security features arund Cisc Unified Cmmunicatins cmpnents; Designing ptins t integrate legacy PBX/Vice Mail systems with UC system; and Designing and sizing f vice gateways and gatekeepers. If SIP Trunking is included, PSTN cnnectivity slutin cvering TDM and/r IP trunks and Vice Gateway, IPIP gateways, Prxies fr feature transparency, security, and resiliency. Identify recmmendatins fr current sftware and hardware cde revisins related t UC cmpnents including gateways, IOS, and appliances. Fr the Cisc Unity applicatin ne r mre f the fllwing may be included: Designing the Unity deplyment mdel; Designing and sizing f the message stre capacity and licensing requirements; Sizing the Unity prts and sessins; Designing f data prtectin strategies; Designing the Unity integratin with Call Manager r PBX systems; Designing f Unity Call Handlers; Designing Unity Netwrking Intercnnecting Unity with ther vice mail systems; Defining Unity custmizatin; Defining methds fr securing access t the Unity servers. Review remtely the Detailed Design Reprt with Custmer. Designate representative Custmer emplyees frm all lines f business including telephny, supprt, netwrk architects, peratinal staff and management representatives t attend the interviews and wrkshp(s). Prvide Custmer Site building layuts, including the flr plans, lcatin f cables and pwer surces as required. Prvide dcumented Custmer requirements (business and technical) including high level netwrk architecture design specificatins and the current Custmer netwrk high level design. Prvide the fllwing infrmatin alng with cmpleted Custmer Requirements Questinnaire: existing legacy vice system netwrk diagram; current hardware/sftware details f legacy PBX systems and future expansin capabilities and interperability supprt; current WAN bandwidth utilizatin; current PBX trunk utilizatin; current vice mail system utilizatin; traffic analysis: busy hur vice t PSTN (i.e., lcal and lng distance); busy hur vice call vlume between sites; call flw diagrams; existing dial plan infrmatin including currently cnfigured class f restrictins; tll-free numbers currently in use; and if Aut Attendant r equivalent features are deplyed, the scripts and prmpts currently in use. Prvide cmpleted Slutin and Custmer Requirements Questinnaire(s) (Cisc Unity/Unity Cnnectin sectin) and dcumented infrmatin including: existing legacy vice mail system netwrk diagram; current message stre utilizatins and capacity reprts; current utilizatin f netwrk services such as name reslutin, authenticatin; current tplgy f messaging and directry infrastructure; current vice mail system plicies and security requirements; current hardware/sftware details f legacy vice mail systems and future expansin capabilities and interperability supprt. Review remtely the Detailed Design Reprt with Cisc. Site Specific Netwrk Implementatin Plan Develpment Review the Custmer prvided requirements and Revised Custmer High Level Design dcument t identify the netwrk implementatin requirements at each lcatin, including: Detailed Design Reprt; site cntacts, lcatin and cnsideratins; current site-specific installatin requirements; site-specific infrmatin frm BOM; prduct dcumentatin, including Administratin Guides, Tech Ntes, and Field Ntices. Review and/r cllect site requirements fr envirnment, Telc circuits, and equipment per Mdel Site. The data cllectin prvides detailed site specificatins and includes the fllwing: hardware cmpnents; Telc circuits; equipment electrical requirements; envirnmental/physical space requirements; equipment rack and cabinet lcatins and layuts. Create and prvide a site-specific Netwrk Implementatin Plan (NIP) per Custmer lcatin at least seven (7) Business Days befre the scheduled cmmencement f the Services. Prvide recmmendatins fr suitable envirnmental cnditins (e.g. racking, flr space, and pwer supplies, air cnditining and site access) within seven (7) Business Days f cmpletin f the NIP. Prvide a list f any Custmer equipment related t the prject scpe that must be prvisined prir t implementatin within seven (7) Business Days f the NIP cmpletin and any recmmendatin, if applicable. Implementatin means the physical installatin plus lgical activities that are required t place prducts in t a Custmer site, cnfigure, cmmissin and cnnect them tgether t prduce a wrking netwrk. Prvide the fllwing recmmended specificatins t the Custmer: server hardware minimum requirements; sftware tl requirements; ptinal hardware and

6 Page 6 f 7 sftware requirements; netwrk service requirements; ptinal DHCP/DNS updates; Cisc Unity Cnnectin server platfrm requirements fr the existing MCS server; Cisc Unity Cnnectin memry and prcessr speed requirements fr any applicable third party platfrm. Review remtely the Netwrk Implementatin Plan with the Custmer. Identify facility and infrastructure requirements fr testing. Identify tls required fr executing test cases. Create and prvide the Slutin and Site Acceptance Test Plan defines and recrds the specific set f prcedures and/r tests develped by Cisc and agreed t by the Custmer that are necessary t test the Cisc prduct and t declare that it is ready fr use. Review remtely the Slutin and Site Acceptance Test Plan with the Custmer. Ensure that key netwrking, telecmmunicatins, and peratinal persnnel are available t participate in interview sessins as required t enable Cisc t validate business and technical requirements. Prvide Custmer s existing high level design and Bill f Material (BOM) prir t prject kick-ff. Ensure key cntacts frm all relevant third party vendrs are available fr nging infrmatin and feedback during architecture discussins. Designate and ensure key Custmer netwrking cntacts are available fr nging infrmatin gathering and feedback during architecture discussins. Prvide the fllwing requirements: Custmer s Unified Cmmunicatins slutin design requirements t Cisc (fr example, details cncerning scalability, resilience and number f phnes); Custmer s security requirements and CUCM features planned (fr example, emergency services/e911, Music n Hld, analg phne / fax / mdem integratin and extensin mbility); types f applicatins impacted by the Netwrk design requirements (fr example, vice mail/unity, Unified Messaging, IPCC Express, Presence, Mbility and Aut Attendant,); and future grwth requirements and netwrk build ut time frames. Prvide physical and lgical netwrk schematics fr the Custmer s existing netwrk where applicable. Prvide current netwrk tplgy, including access, distributin, and cre layers, types f switches and ruters in each layer, IP addressing and sub-netting fr each layer, features, and services that have been enabled n the netwrk. Review the high level design and Bill f Materials recmmendatins with Cisc fr cmment and apprval prir t the NIP being finalized. Review remtely the NIP with Cisc. Slutin and Site Acceptance Test Plan Develpment Review remtely the Slutin and Site Acceptance Test Plan with Cisc. Server Cre Applicatin Installatin (Ready fr Prvisining Prvide pwer and space requirements fr the data center as per the apprved Design Validatin Reprt and Cisc UCS design guide. Prvide Rack and Stacking f equipment as required fr each Cre Build as defined in the Master Plan and Schedule. Prvide Netwrk Requirements (IP Address, DNS, NTP etc.) fr installing the UCS servers, UCS system management cnsle, UCS Directr, Hypervisr, and UC Applicatins. Prvide details f the virtual netwrk device and Strage Area Netwrk appliances as per UC fr UCS design guide. Prvide the netwrk latency requirements fr UC Applicatins t be installed in the data center. Prvide bandwidth requirements fr UC Applicatins t functin acrss data center. Prvide necessary QOS requirements fr UC Applicatins t functin acrss data center and branches. Install the fllwing UC Applicatins as mentined in the apprved High Level Design Validatin Reprt: Cisc Unified Cmmunicatins Manager Cisc Unity Cnnectin Cisc Emergency Respnder Cisc Unified Attendant Cnsle Cisc Instant Messaging & Presence Review the fllwing t cllect testing requirements: Slutin and Custmer Requirements Reprt; Detailed Design Dcument; and Netwrk Readiness Assessment Reprt. Ensure that netwrk latency is within the allwable limits as prvided by Cisc.

Prvide the required physical access and remte access t the data center t Cisc Engineers t d the installatin. Cnfigure all Layer 2 and Layer 3 devices. Ensure that netwrk cnnectivity is available between the data center and the branches. Trublesht any cnnectivity issue between the data center and the branches Prcure the required equipment including cabling and racks. Participate in cnducting the necessary testing fr UCS servers, virtualized envirnment fr UC applicatins, and access t the installed UC applicatins. General All infrmatin (such as but nt limited t: designs, tplgies, requirements) prvided by Custmer is assumed t be up-t-date and valid fr the Custmer s current envirnment. Cisc Services are based upn infrmatin prvided t Cisc by Custmer at the time f the Services. Custmer acknwledges that the cmpletin f Services is dependent upn Custmer meeting its respnsibilities as indicated herein. Identify Custmer s persnnel and define their rles in the participatin f the Services. Such persnnel may include but is nt limited t: architecture design and planning engineers, and netwrk engineers. Ensure Custmer s persnnel are available t participate during the curse f the Services t prvide infrmatin and t participate in scheduled infrmatin gathering sessins, interviews, meetings and cnference calls. Supprt services prvided by Cisc cmprise technical advice, assistance and guidance nly. Custmer expressly understands and agrees that the Services shall take place and cmplete within ne hundred-fifty (150) calendar days frm issuing a Purchase Order t Cisc fr the Services herein. Delays caused by Custmer may require change requests and additinal csts. Custmer retains verall respnsibility fr all business prcess impact and all prcess change implementatins. Services may be dependent upn Cisc s use f Data Cllectin Tls. Custmer will arrange the purchase f third party services and prducts, including but nt limited t cmmunicatins services, ffice space, cmmunicatins rms, irn wrks, air cnditining, and ffice services. Custmer is respnsible fr placing phnes n desks, wiring and cabling, pwer, HVAC, trash remval, and deinstalling legacy equipment. Custmer is respnsible fr prelading UC client n desktps and perfrming all phne upgrades/lads. Custmer wns the physical installatin, cutver and supprt with Cisc prviding nly plan and design services. General Assumptins The services will cver up t tw-hundred-fifty (250) branches and/r 5000 users within a single theater. Cisc des nt prvide equipment staging. N integratin (legacy PBX, third party applicatins, etc.) Call Cntrl nly with Cisc Unity Cnnectin (CUC), Unified Cntact Center Express (UCCX), Cisc Emergency Respnder (CER), etc. Invicing and Cmpletin Invicing Services will be inviced upn cmpletin f the Services.
