2 3802 Cherry Ave. Wilmington, NC Tel: Fax: June 19, 2017 The Walker Group Architecture, Inc. Attn: Beth Walker PO Box 541 New Bern, NC Re: Asbestos Bulk Sampling & Analysis at: Camp Lejeune Building 500 Repair Indoor Physical Fitness Center Contract No. N D-0857 Client Project No.: PEI Project No.: AL On June 14, 2017, Jonathan Guetta and Michael Krupa (NC Asbestos Building Inspector Accreditation Nos and 12880, respectively) of Precision Environmental, Inc. (Precision) conducted an asbestos survey within the above referenced building. The inspection included all materials scheduled to be disturbed/removed during the upcoming renovation project as described in the written scope of work supplied to Precision by the Client. The purpose of the survey was to verify the presence or absence of asbestos-containing materials (ACM) associated with the building prior to renovation procedures. Prior to sample collection, a visual inspection was conducted in order to determine homogeneous materials/areas and sample locations. Asbestos-containing materials are defined as materials that contain greater than 1% asbestos via Polarized Light Microscopy (PLM). Homogeneous Areas (HGA) are determined by the material s color and texture. Asbestos-containing materials are defined by the following descriptions: surfacing material (SM), thermal system insulation (TSI) and miscellaneous materials (M). Both friable and non-friable materials were included in the inspection. Friable materials are defined as those that can be pulverized by hand pressure. Bulk sampling of suspect ACM was conducted in accordance with the sampling requirements promulgated by the United States Environmental Protection Agency s Asbestos-Containing Materials in Schools Rule (40 CFR 763, Subpart E), commonly referred to as the Asbestos Hazard Emergency Response Act or AHERA regulations. Specific compliance to these requirements include, but are not limited to, the type and number of samples to be collected. Sample locations were selected at random. As a result, a total of thirty-seven (37) bulk samples were collected from sixteen (16) suspect asbestos-containing materials. A listing of identified suspect ACM materials and the number of samples collected from each homogeneous area (HGA) is provided in Table 1 below: Table 1: Identified Suspect Asbestos Materials Suspect material (HGA) Description Friable/Nonfriable Sample Location No. of Samples Collected 1. All samples were analyzed by Polarized Light Microscopy (PLM) via EPA method 600/M4/82/020. Multi layered samples were separated prior to analysis and analyzed separately per EPA protocol. In an effort to reduce cost, Precision instructed the laboratory to STOP analysis at the first positive sample for each suspect material HGA and not to analyze the remaining samples from the same HGA. As a result, a total of thirty-nine (39) samples were analyzed. Results Laboratory analysis of the bulk samples collected revealed that suspect material 1 contains asbestos in quantities greater than 1% and is considered to be an asbestos-containing material. The remaining suspect materials revealed no asbestos detected and may be treated as NON-ACM. Listing of identified ACM and NON-ACM materials is provided in Table 2 below: Table 2: Identified ACM and NON-ACM Materials Material Description Friable/ Non-friable Location Laboratory Result Approx. If the materials are to be disturbed/removed, the materials must be removed in accordance with all applicable Federal, State and Local regulations. In addition, the materials must be disposed of as asbestos-containing waste and may not be disposed of as construction debris. A physical/visual inspection revealed the following: No destructive investigation techniques were used during this inspection. It should be understood that suspect asbestos-containing pipe insulation materials may exist within presently concealed wall and/or ceiling cavities and pipe chase spaces. Non-suspect ceramic tile floors were noted in bathrooms within the building. Non-suspect FRP wall board material was noted within interior bathrooms. Non-suspect T1-11 boards were noted within the Gym area. Non-suspect wood base molding was noted in various areas within the building. Non-suspect fiberglass pipe insulation and associated fiberglass and/or PVC fittings were noted throughout the interior spaces (including bathrooms) within the building. Non-suspect rubber flooring material was noted in the locker rooms. Non-suspect fiberglass duct insulation was noted on the ductwork in the locker rooms. The ceiling of the main gymnasium space was noted to consist of confirmed non-asbestos-containing wallboard and joint compound materials. Non-suspect fiberglass pipe insulation and fiberglass duct insulation were noted in the attic. Piping within the confined space steam pit at the rear of the building appeared to be insulated with non-suspect fiberglass pipe insulation. Non-suspect fiberglass pipe insulation and duct insulation materials were noted within the crawlspace. The exterior soffit/overhang was noted to consist of non-suspect concrete. No suspect asbestos-containing window glazing compound was noted at the windows. Non-suspect vinyl siding was noted at the upper exterior facades of the building. All efforts were made to discover/sample all suspect asbestos-containing materials. If additional materials not addressed during this inspection are to be disturbed, Precision strongly recommends that those materials either be assumed to be asbestoscontaining, or that bulk samples be collected to determine the materials asbestos content prior to their disturbance.

(Precision) was retained by The Walker Group Architecture, Inc. to conduct a lead paint survey and analysis of coated surfaces associated with the above referenced facility. The inspection included all materials scheduled to be disturbed/removed during the upcoming renovation project as described in the written scope of work supplied to Precision by the Client. The purpose of the survey was to determine if lead painted components exist within and on the exterior of the the structure prior to renovation procedures. The bulk sampling was performed by Precision s representative, Jonathan Guetta (NC Risk Assessor/ Lead Inspector Certificate No ) on June 14, As a result, a total of twenty-two (22) different suspect lead based paint (LBP) coatings were identified. A listing of the identified LBP coatings and the number of bulk samples collected per suspect material is provided in Table 1 below: Table 1: Identified suspect LBP coatings materials Suspect Material Location No. of Samples Collected 1.
