1 Position Definition Position: Agreement: Classification: Grade 2 Position reports to: Effective date: July 2017 Approved by: Clinical / Health Psychologist Public Community Health Sector Enterprise Agreement ( the Agreement ) Manager CHSP/Community Services Director of Community Services Organisational Context- Plenty Valley Community Health (PVCH) provides primary healthcare, social and psychological support services to people of all ages, focusing on the growing outer northern metropolitan area of Melbourne. Our services have a strong focus on treatment, prevention, early identification, rehabilitation and maximisation of independent living skills. PVCH is a major provider of primary healthcare in our region and is committed to establishing a well-integrated primary healthcare service that is accessible to people in the City of Whittlesea and neighbouring metropolitan and rural municipalities. PVCH partners with General Practitioners, Hospitals, the three levels of government, local agencies and community members to achieve an effective universal primary healthcare system. PVCH is a registered Community Health Centre and a Company Limited by Guarantee. It is funded by government grants and other self-managed revenue activities. It is governed by a Board of Directors elected and appointed from the community. Our expectation is that staff will operate as a co-operative team. Our staff will be flexible, multi-skilled and able to work with a diverse range of individuals, multidisciplinary groups and community agencies. Our staff will continually update their knowledge to take on new responsibilities and challenges. Purpose and Scope The Health Psychologist will be accountable for providing quality client care through the competent application of clinical skills in assessment, planning and implementation of evidence-based psychological interventions. The Health Psychologist will work with clients who present to the service with psychological issues that impact upon adjustment, treatment adherence or capacity to self-manage a chronic health condition. The position will work closely with the multidisciplinary team including allied health clinicians and nursing staff to promote best practice and ensure excellence in client care.

2 Key Responsibilities and Duties Service delivery: Apply best practice clinical knowledge and expertise in the area of rehabilitation psychology including persistent pain management Complete clinical assessments and implement appropriate evidence-based individual and group interventions. Apply demonstrated knowledge of the interplay between biological, psychological and social factors and their influence on health and illness Use a person centred approach to work with clients and carers regarding decision making and care plans Access relevant community resources, information and referral processes, and utilise this knowledge in clinical practice. Apply highly developed communication, negotiation, and conflict resolution skills when working with clients Record appropriate information and outcomes (e.g. in medical histories) in an accurate and timely manner using TrakCare Complete written reports to relevant outside organisations as required Foster and model a detailed understanding of culturally and linguistically diverse communities (CALD). Identify and implement relevant outcome measures to demonstrate effectiveness of psychological interventions. Apply knowledge and application of health behaviour change theories, principles and practice techniques in clinical practice. Participate in multidisciplinary case conferences and team meetings as appropriate. Provides psychological input into multidisciplinary program development as required. Undertake appropriate professional duties as directed by Manager Actively seek out all learning opportunities and fosters this in others. The capacity to learn to effectively use the PVCH electronic records systems (for example VHIMS, TRIM, TrakCare, Titanium). Financial: Not applicable Key Selection Criteria Mandatory: Successful applicants are required to provide certified copies of the mandatory qualifications listed in this section. Bachelor Qualification in Psychology and Postgraduate qualification in clinical, health or counselling Psychology Eligibility to obtain and maintain professional registration with the Australian Health Practitioners Registration Agency (AHPRA) and must provide evidence of registration to PVCH annually. Substantial relevant experience working with clients experiencing chronic health conditions as well as high prevalence to mental health conditions Demonstrated knowledge and experience in the area of rehabilitation psychology and self management of persistent pain management and chronic disease Highly developed interpersonal skills working within a multidisciplinary team Strong practical and appropriate negotiation and conflict resolution skills Current Victorian Drivers Licence Desirable: Current endorsement in clinical psychology or health psychology as approved area of practice with the Australian Health Practitioner Regulation Agency (AHPRA) Experience in the provision of rehabilitation psychology services or working with client populations with chronic disease and complex medical conditions. Previous experience working in a community health setting Proficiency in a community language 4 Private & Confidential Plenty Valley Community Health

5 Conditions of Employment Remuneration: The employee will be paid in accordance with the Award/Agreement. Salary Packaging is available for permanent and fixed term employees, in accordance with prevailing legislation and PVCH Policy. Novated Car Leasing is available for all permanent employees. Location: The employee will be expected to work, either temporarily or permanently, at any site operated by PVCH. Hours of duty: The hours of duty will be as rostered by the employee s Supervisor. Pre-employment checks: A Police Check is mandatory. In accordance with PVCH Policy, relevant convictions will be taken into consideration when considering applicants for this position. All employees of PVCH must be either an Australian Citizen, Permanent Resident or have an appropriate Australian visa that will legally enable them to fulfil the obligations of the contract of employment. A Working With Children card (WWC) is mandatory for this position. Failure to obtain and retain a WWC card will make the employee automatically ineligible for employment in this position. Probationary period: Confirmation of new employees to PVCH is subject to a 6 month probationary period, during which time the employee must demonstrate satisfactory capability to perform the Key Responsibilities and Duties. This probationary period is regarded as an extension of the interview process.
