4 Directorate NDIS Services The National Disability Insurance Scheme will work to support people with a disability to participate in and contribute to social and economic life and provide certainty that people with a disability will receive the care and support they need over their lifetime. The Scheme provides a number of strategies to assist people with disability to exercise choice, including in relation to taking reasonable risks, in the pursuit of their goals and the planning and delivery of their supports. The NDIS and Latrobe Community Health Partnership will provide Local Area Coordination (LAC) Services across the local government agencies in Central Highlands, Ovens Murray, Barwon, Wimmera South West, Inner Gippsland, Inner Eastern Melbourne and Outer Eastern Melbourne. We will also provide Early Childhood Early Intervention (ECEI) Services in Central Highlands and Wimmera South West regions. Position Information: Scope of Role: Reporting to the Area Manager LAC Service, and working under guidance from the Team Leader LAC Service, the Local Area Coordinator is responsible for undertaking the Scheme pathway services as described in the National Disability Insurance Agency Statement of Requirements (SoR). The role will: Assist people with a disability, their families and carers to build and pursue their goals for a normal life, exercise control and engage with the Scheme, and; Ensure that people with a disability can be supported outside of the Scheme by working with communities and mainstream services to build awareness and to become more inclusive of the range of needs and aspirations of people with a disability. Key Objectives, Duties and Responsibilities Operational Raise awareness and explain the National Disability Insurance Scheme (NDIS) to the community, including people with a disability who are not eligible for the Scheme, and people who are eligible for the Scheme. Support people with a disability (not eligible for Scheme), their families, and carers to build their capacity, and provide information linkage and support to connect to natural supports and other existing opportunities within their community. Support people with a disability (participants eligible for Scheme), their families, and carers with pre-planning, capacity building and linking them into community and mainstream services. Assist participants to develop a plan that includes the participant s goals, strengths, needs and wishes, options for community and mainstream support, informal support, and any reasonable and necessary supports recommended to be funded. Assist participants with initial implementation of plan and annual review of their plan. Build community inclusion by working with community groups, services, organisations, and other social structures to understand the need of participants, their families and carers so that they can better align their service offers to address their needs. Work with community based organisations to build and expand opportunities for greater social and economic participation for all people with a disability. 5 Meet service delivery key performance outcomes within productivity requirements. Undertake additional NDIS duties as and when required by the Area Manager Local Area Coordination Service. Best Practice Provide information to participants and their carers on their rights and responsibilities. Maintain participant/carer privacy and confidentiality. Implement mechanisms to ensure continuous quality improvement within the LAC Service. Ensure participant interactions are outcome orientated and consistent with legislation. Team Development Attend and contribute to regular team meetings. Participate in the growth and development of the team. Support peers while respecting individuality. Organisational Development Contribute to decision making processes. Provide reports as requested by the Team Leader LAC Service and Area Manager LAC Service. Maintain participant records in accordance with organisational and Agency standards. Be responsible for the collection and collation of data and statistics as determined by the Area Manager LAC Service. Complete required program data collection within the timelines of the program requirements. Participate in a duty worker roster as required. Community Development Attend community meetings as required. Monitor trends, changes and service gaps within the local community and report to the Manager with findings and/or recommendations. Provide community presentations as required. Professional Development Participate in staff development opportunities as planned and required. Participate in regular supervision with the Area Manager LAC Service and/or Team Leader LAC Service. Participate in an annual performance review and development plan. Maintain professional registration (where required) and keep up to date with professional practice issues. Practice in accordance with Commonwealth and State Government legislation and regulations. Special Projects LAC staff may from time to time be required to undertake additional work on special projects for the organisation. This work will be consistent, unless otherwise agreed to by both parties, with those duties of the Local Area Coordinator as set out above. 6 Policy and Procedures Comply with Latrobe Community Health Service s policy and procedures and ensure all re-issued ekey documents are read, acknowledged and knowledge is kept up to date. After Hours Service Participate in the After Hours Program Service roster and other activities, if required, which may encompass working some evening and/or weekends. Travel At times travel will be required to deliver LAC services across the service area. Social Model of Health Understand and apply the principles of the Social Model of Health, which aims to improve the health and wellbeing of clients through the development of appropriate service planning and program delivery. Occupational Health & Safety Ensure that work and services are provided in a safe manner at all times by regularly reviewing work practices and environment and by participating in OH&S training where required. Continuous Quality Improvement Identify continuous quality improvement opportunities; participate in the development of quality procedures and contribute to internal and external program reviews as required. Annual Performance Review Actively participate in individual performance review plans, which determines key responsibilities, strategies and performance indicators, in line with the organisation s Strategic Plan and Program Business Plan. Selection Criteria: All applicants must address the selection criteria for consideration. 1. Demonstrated understanding of the National Disability Insurance Scheme Act (2013), the National Disability Services Standards, and the needs of people with a disability, their families and carers. 2. Ability to assist people with a disability develop a plan that includes their goals, strengths, needs and wishes, options for community, mainstream and informal support, and any reasonable and necessary supports recommended to be funded. 3. Effective time management skills and demonstrated ability to achieve key positional performance indicators within a required time. 4. Strong knowledge of the principles of person centred care and demonstrated ability to apply this approach. 5. High level communication, negotiation and conflict resolution skills. 6. Proven ability to liaise with a variety of service providers, professionals, caregivers and other internal and external stakeholders. 7. Demonstrated experience in continuously improving the quality of customer services. 8. Demonstrated information technology competency in using computers and tablets. Mandatory Criteria: Demonstrated commitment to LCHS Values Ability to travel across the service delivery area; including satellite sites when required. 8 Other Job Requirements: All incumbents must satisfactorily meet the security check criteria prior to appointment. A Police Record Check regarding any criminal record will be required prior to appointment and updated every three (3) years. Credentialing documentation is required to be completed and verified prior to finalisation of the appointment. Prior to any person being appointed to this position it will be required that they disclose full details of any pre-existing injuries or disease that might be affected by employment in this position. This Position Description and Letter of Agreement will be reviewed from time to time in keeping with changing requirements.
