A certfed copy of the latest Montorng Report of the Regonal Offce of the Mnstry of Envronment and Forests as per crcular dated 30 th May, 2012 on the status of complance of condtons stpulated n all the exstng envronmental clearances ncludng Amendments shall be provded. In addton, status of complance of Consent to Operate for the ongong exstng operaton of the project from SPCB shall be attached wth the EIAEMP report. In case the exstng project has not obtaned envronmental clearance, reasons for not takng EC under the provsons of the EIA Notfcaton 1994 KADAM ENVIRONMENTAL CONSULTANTS OCTOBER,

2 and/or EIA Notfcaton 2006 shall be provded. Copes of Consent to Establsh/No Objecton Certfcate and Consent to Operate (n case of unts operatng pror to EIA Notfcaton 2006, CTE and CTO of FY ) obtaned from the SPCB shall be submtted. Further, complance report to the condtons of consents from the SPCB shall be submtted. 4 Ste Detals Locaton of the project ste coverng vllage, Taluka/Tehsl, Dstrct and State, Justfcaton for selectng the ste, whether other stes were consdered. A toposheet of the study area of radus of 10km and ste locaton on 1:50,000/1:25,000 scale on an A3/A2 sheet. Not applcable. 5 Forest and wldlfe related ssues (f applcable): Not applcable. Permsson and approval for the use of forest land (forestry clearance), f any, and recommendatons of the State Forest Department. The projects to be located wthn 10 km of the Natonal Parks, Sanctuares, Bosphere Reserves, Mgratory Corrdors of Wld Anmals, the project proponent shall submt the map duly authentcated by Chef Wldlfe Warden showng these features vsàvs the project locaton and the recommendatons or comments of the Chef Wldlfe Wardenthereon. Wldlfe Conservaton Plan duly authentcated by the Chef Wldlfe Warden of the State Government for conservaton of Schedule I fauna, f any exsts n the study area. Copy of applcaton submtted for clearance under the Wldlfe (Protecton) Act, 1972, to the Standng Commttee of the Natonal Board for Wldlfe. 6 Envronmental Status v Determnaton of atmospherc nverson level at the project ste and stespecfc mcrometeorologcal data usng temperature, relate humdty, hourly wnd speed and drecton and ranfall. AAQ data (except monsoon) at 8 locatons for PM 10, PM 2.5, SO 2, NO X, CO and other parameters relevant to the project shall be collected. The montorng statons shall be based CPCB gudelnes and take nto account the predomnant wnd drecton, populaton zone and senste receptors ncludng reserved forests. Raw data of all AAQ measurement for 12 weeks of all statons as per frequency gen n the NAQQM Notfcaton of Nov along wth mn., max., average and 98% values for each of the AAQ parameters from data of all AAQ statons should be provded as an annexure to the EIA Report. Surface water qualty of nearby Rer (100 m upstream and downstream of dscharge pont) and other surface drans at eght locatons as per CPCB/MoEF&CC gudelnes. Whether the ste falls near to polluted stretch of rer dentfed by the CPCB/MoEF&CC, f yes ge detals. Data for Post Monsoon, 2018 shall be furnshed. Project specfc parameters lke PM 10, PM 2.5, SO 2, NO X, CO, HC, HCl & Cl 2 s collected and shall be furnshed. Not applcable. v Ground water montorng at mnmum at 8 locatons shall be ncluded. v Nose levels montorng at 8 locatons wthn the study area. v Sol Characterstc as per CPCB gudelnes. x x Traffc study of the area, type of vehcles, frequency of vehcles for transportaton of materals, addtonal traffc due to proposed project, parkng arrangement etc. Detaled descrpton of flora and fauna (terrestral and aquatc) exstng n the study area shall be gen wth specal reference to rare, endemc and endangered speces. If ScheduleI fauna are found wthn the study area, a Wldlfe Conservaton Plan shall be prepared and furnshed. x Socoeconomc status of the study area. 7 Impact and Envronment Management Plan Assessment of ground level concentraton of pollutants from the stack emsson based on stespecfc meteorologcal features. In case the project s located on a hlly terran, the AQIP Modellng shall be done usng nputs of the specfc terran characterstcs for determnng the potental mpacts KADAM ENVIRONMENTAL CONSULTANTS OCTOBER,

4 of the project on the AAQ. Cumulate mpact of all sources of emssons (ncludng transportaton) on the AAQ of the area shall be assessed. Detals of the model used and the nput data used for modellng shall also be provded. The ar qualty contours shall be plotted on a locaton map showng the locaton of project ste, habtaton nearby, senste receptors, f any. Water Qualty modellng n case of dscharge n water body. Not applcable Impact of the transport of the raw materals and end products on the surroundng envronment shall be assessed and provded. In ths regard, optons for transport of raw materals and fnshed products and wastes (large quanttes) by ral or ralcum road transport or conveyorcumral transport shall be examned. A note on treatment of wastewater from dfferent plant operatons, extent recycled and reused for dfferent purposes shall be ncluded. Complete scheme of effluent treatment. Characterstcs of untreated and treated effluent to meet the prescrbed standards of dscharge under E(P) Rules. v Detals of stack emsson and acton plan for control of emssons to meet standards. v Measures for fugte emsson control v v x x x Detals of hazardous waste generaton and ther storage, utlzaton and management. Copes of MOU regardng utlzaton of sold and hazardous waste n cement plant shall also be ncluded. EMP shall nclude the concept of wastemnmzaton, recycle/reuse/recover technques, Energy conservaton, and natural resource conservaton. Proper utlzaton of fly ash shall be ensured as per Fly Ash Notfcaton, A detaled plan of acton shall be provded. Acton plan for the green belt development plan n 33 % area.e. land wth not less than 1,500 trees per ha. Gng detals of speces, wdth of plantaton, plannng schedule etc. shall be ncluded. The green belt shall be around the project boundary and a scheme for greenng of the roads used for the project shall also be ncorporated. Acton plan for ranwater harvestng measures at plant ste shall be submtted to harvest ranwater from the roof tops and storm water drans to recharge the ground water and also to use for the varous acttes at the project ste to conserve fresh water and reduce the water requrement from other sources. Total captal cost and recurrng cost/annum for envronmental polluton control measures shall be ncluded. Snce, ths beng exstng unt, only ~15 % of total plot area wll be developed / mantaned as green belt, based on the avalablty of space. x Acton plan for postproject envronmental montorng shall be submtted. x Onste and Offste Dsaster (natural and Manmade) Preparedness and Emergency Management Plan ncludng Rsk Assessment and damage control. Dsaster management plan should be lnked wth Dstrct Dsaster Management Plan. 8 Occupatonal health Plan and fund allocaton to ensure the occupatonal health & safety of all contract and casual workers. 5 Detals of exposure specfc health status evaluaton of worker. If the workers' health s beng evaluated by pre desgned format, chest x rays, Audometry, Sprometry, Vson testng (Far & Near vson, color vson and any other ocular defect) ECG, durng preplacement and perodcal examnatons ge the detals of the same. Detals regardng last month analyzed data of above mentoned parameters as per age, sex, duraton of exposure and department wse. Detals of exstng Occupatonal & Safety Hazards. What are the exposure levels of hazards and whether they are wthn Permssble Exposure level (PEL). If these are not wthn PEL, what measures the company has adopted to keep them wthn PEL so that health of the workers can be preserved, Annual report of health status of workers wth specal reference to Occupatonal Health and Safety. 9 Corporate Envronment Polcy 10 Does the company have a well lad down Envronment Polcy approved by ts Board of Drectors? If so, t may be detaled n the EIA report. Does the Envronment Polcy prescrbe for standard operatng process / procedures to brng nto focus any nfrngement / devaton / volaton of the envronmental or forest norms / condtons? If so, t may be detaled n the EIA. What s the herarchcal system or Admnstrate order of the company to deal wth the envronmental ssues and for ensurng complance wth the envronmental clearance condtons? Detals of ths system may be gen. Does the company have system of reportng of noncomplances / volatons of envronmental norms to the Board of Drectors of the company and / or shareholders or stakeholders at large? Ths reportng mechansm shall be detaled n the EIA report. Detals regardng nfrastructure facltes such as santaton, fuel, restroom etc. to be provded to the labour force durng constructon as well as to the casual workers ncludng truck drers durng operaton phase. 11 Enterprse Socal Commtment (ESC) Adequate funds (at least 2.5 % of the project cost) shall be earmarked towards the Enterprse Socal Commtment based on Publc Hearng ssues and temwse detals along wth tme bound acton plan shall be ncluded. Socoeconomc development acttes need to be elaborated upon % of captal nvestment wll be earmarked as Corporate Envronment Responsblty funds as per CER polcy vde dated 1 st May Any ltgaton pendng aganst the project and/or any drecton/order passed by any Court of Law aganst the project, f so, detals thereof shall also be ncluded. Has the unt receed any notce under the Secton 5 of Envronment (Protecton) Act, 1986 or relevant Sectons of Ar and Water Acts? 6 Thermal Power Plant 1 The proposed project shall be gen a unque name n consonance wth the name submtted to other Government Departments etc. for ts better dentfcaton and reference Vson document specfyng prospecte long term plan of the project shall be formulated and submtted. Latest complance report duly certfed by the Regonal Offce of MoEF&CC for the condtons stpulated n the envronmental and CRZ clearances of the prevous phase(s) for the expanson projects shall be submtted. The project proponent needs to dentfy mnmum three potental stes based on envronmental, ecologcal and economc consderatons, and choose one approprate ste havng mnmum mpacts on ecology and envronment. A detaled comparson of the stes n ths regard shall be submtted. Execute summary of the project ndcatng relevant detals along wth recent photographs of the proposed ste (s) shall be provded. Response to the ssues rased durng Publc Hearng and the wrtten representatons (f any), along wth a tme bound Acton Plan and budgetary allocatons to address the same, shall be provded n a tabular form, aganst each acton proposed. Harnessng solar power wthn the premses of the plant partcularly at avalable roof tops and other avalable areas shall be formulated and for expanson projects, status of mplementaton shall also be submtted. The geographcal coordnates (WGS 84) of the proposed ste (plant boundary), ncludng locaton of ash pond along wth topo sheet (1:50,000 scale) and IRS satellte map of the area, shall be submtted. Elevaton of plant ste and ash pond wth respect to HFL of water body/nallah/rer and hgh tde level from the sea shall be specfed, f the ste s located n proxmty to them. Layout plan ndcatng breakup of plant area, ash pond, green belt, nfrastructure, roads etc. Land requrement for the project shall be optmzed and n any case not more than what has been specfed by CEA from tme to tme. Item wse break up of land requrement shall be provded. Present land use (ncludng land class/ksm) as per the revenue records and State Govt. records of the proposed ste shall be furnshed. Informaton on land to be acqured ncludng coal transportaton system, layng of ppelne, ROW, transmsson lnes etc. shall be specfcally submtted. Status of land acquston and ltgaton, f any, should be provded. Snce the exstng project does not requre EC or CRZ clearance, latest complance report duly certfed by the Regonal Offce of PPCB for the condtons stpulated n the CTE shall be provded. 7 If the project nvolves forest land, detals of applcaton, ncludng date of applcaton, area appled for, and applcaton regstraton number, for derson under FCA and ts status should be provded along wth copes of relevant documents. The land acquston and R&R scheme wth a tme bound Acton Plan should be formulated and addressed n the EIA report. Satellte magery and authentcated topo sheet ndcatng dranage, croppng pattern, water bodes (wetland, rer system, stream, nallahs, ponds etc.), locaton of nearest habtatons (vllages), creeks, mangroves, rers, reservors etc. n the study area shall be provded. Locaton of any Natonal Park, Sanctuary, Elephant/Tger Reserve (exstng as well as proposed), mgratory routes / wldlfe corrdor, f any, wthn 10 km of the project ste shall be specfed and marked on the map duly authentcated by the Chef Wldlfe Warden of the State or an offcer authorzed by hm. Topography of the study area supported by Toposheet on 1:50,000 scale of Survey of Inda, along wth a large scale map preferably of 1:25,000 scale and the specfc nformaton whether the ste requres any fllng shall be provded. In that case, detals of fllng, quantty of requred fll materal; ts source, transportaton etc. shall be submtted. A detaled study on land use pattern n the study area shall be carred out ncludng dentfcaton of common property resources (such as grazng and communty land, water resources etc.) avalable and Acton Plan for ts protecton and management shall be formulated. If acquston of grazng land s nvolved, t shall be ensured that an equal area of grazng land be acqured and developed and detaled plan submtted. A mneralogcal map of the proposed ste (ncludng sol type) and nformaton (f avalable) that the ste s not located on potentally mneable mneral depost shall be submtted. Detals of fly ash utlzaton plan as per the latest fly ash Utlzaton Notfcaton of GOI along wth frm agreements / MoU wth contractng partes ncludng other usages etc. shall be submtted. The plan shall also nclude dsposal method / mechansm of bottom ash. The water requrement shall be optmzed (by adoptng measures such as dry fly ash and dry bottom ash dsposal system, concept of zero dscharge) and n any case not more than that stpulated by CEA from tme to tme, to be submtted along wth detals of source of water and water balance dagram. Detals of water balance calculated shall take nto account reuse and recrculaton of effluents. Water body/nallah (f any) passng across the ste should not be dsturbed as far as possble. In case any Nallah / dran s proposed to be derted, t shall be ensured that the derson does not dsturb the natural dranage Not Applcable, as no forest land s nvolved n the proposed expanson project. Not Applcable, as no R&R ssues envsaged n the proposed expanson project. Nangal Wldlfe Sanctuary s wthn 10 Km of the project ste. Locaton of the same wll be marked on the map duly authentcated by the Chef Wldlfe Warden of the State or an offcer authorzed by hm and provded n EIA report. No fllng s requred n the proposed project. 8 21 22 pattern of the area. Detals of proposed derson shall be furnshed duly approved by the concerned Department of the State. It shall also be ensured that a mnmum of 500 m dstance of plant boundary s kept from the HFL of rer system / streams etc. and the boundary of ste should also be located 500 m away from ralway track and Natonal Hghways. Hydrogeologcal study of the area shall be carred out through an nsttute/ organzaton of repute to assess the mpact on ground and surface water regmes. Specfc mtgaton measures shall be spelt out and tme bound Acton Plan for ts mplementaton shall be submtted Detaled Studes on the mpacts of the ecology ncludng fsheres of the Rer/Estuary/Sea due to the proposed wthdrawal of water / dscharge of treated wastewater nto the Rer/Sea etc shall be carred out and submtted along wth the EIA Report. In case of requrement of marne mpact assessment study, the locaton of ntake and outfall shall be clearly specfed along wth depth of water drawl and dscharge nto open sea. Source of water and ts sustanablty even n lean season shall be provded along wth detals of ecologcal mpacts arsng out of wthdrawal of water and takng nto account nterstate shares (f any). Informaton on other competng sources downstream of the proposed project and commtment regardng avalablty of requste quantty of water from the Competent Authorty shall be provded along wth letter / document statng frm allocaton of water. Detaled plan for ranwater harvestng and ts proposed utlzaton n the plant shall be furnshed. Feasblty of near zero dscharge concept shall be crtcally examned and ts detals submtted. Optmzaton of Cycles of Concentraton (COC) along wth other water conservaton measures. Zero Lqud Dscharge concept wll be acheved, hence no dsposal of waste water on any water body s envsaged due to the proposed project. There s no sea n the near vcnty of the project. 28 Plan for recrculaton of ash pond water and ts mplementaton Detaled plan for conductng montorng of water qualty regularly wth proper mantenance of records shall be formulated. Detal of methodology and dentfcaton of montorng ponts (between the plant and dranage n the drecton of flow of surface / ground water) shall be submtted. It shall be ensured that parameter to be montored also nclude heavy metals. A provson for longterm montorng of ground water table usng Pezometer shall be ncorporated n EIA, partcularly from the study area. Socoeconomc study of the study area comprsng of 10 km from the plant ste shall be carred out havng detaled assessment of the mpact on lelhood of the local communtes. Acton Plan for dentfcaton of local employable youth for tranng n sklls, relevant to the project, for eventual employment n the project tself shall be formulated and numbers specfed durng constructon & operaton phases of the Project. If the area has trbal populaton t shall be ensured that the rghts of trbals are well protected. The project proponent shall accordngly dentfy trbal ssues under varous provsons of the law of the land. 9 A detaled CSR plan along wth acttes wse break up of fnancal commtment. CSR component shall be dentfed consderng need based assessment study and Publc Hearng ssues. Sustanable ncome generatng measures whch can help n uplftment of affected secton of socety, whch s consstent wth the tradtonal sklls of the people shall be dentfed. Separate budget for communty development acttes and ncome generatng programmes shall be specfed. Whle formulatng CSR schemes t shall be ensured that an nbult montorng mechansm for the schemes dentfed are n place and mechansm for conductng annual socal audt from the nearest government nsttute of repute n the regon shall be prepared. The project proponent shall also provde Acton Plan for the status of mplementaton of the scheme from tme to tme and detals of the same wth any Govt. scheme(s). CSR detals done n the past should be clearly spelt out n case of expanson projects. R&R plan, as applcable, shall be formulated wheren mechansm for protectng the rghts and lelhood of the people n the regon who are lkely to be mpacted, s taken nto consderaton. R&R plan shall be formulated after a detaled census of populaton based on soco economc surveys who were dependent on land fallng n the project, as well as, populaton who were dependent on land not owned by them. Assessment of occupatonal health and endemc dseases of envronmental orgn n the study area shall be carred out and Acton Plan to mtgate the same shall be prepared. Occupatonal health and safety measures for the workers ncludng dentfcaton of work related health hazards shall be formulated. The company shall engage full tme qualfed doctors who are traned n occupatonal health. Health montorng of the workers shall be conducted at perodc ntervals and health records mantaned. Awareness programme for workers due to lkely adverse mpact on ther health due to workng n nonconduce envronment shall be carred out and precautonary measures lke use of personal equpment etc. shall be provded. Revew of mpact of varous health measures undertaken at ntervals of two to three years shall be conducted wth an excellent follow up plan of acton wherever requred. One complete season ste specfc meteorologcal and AAQ data (except monsoon season) as per latest MoEF&CC Notfcaton shall be collected and the dates of montorng shall be recorded. The parameters to be covered for AAQ shall nclude PM 10, PM 2.5, SO 2, NO x, CO and Hg. The locaton of the montorng statons should be so decded so as to take nto consderaton the upwnd drecton, predomnant downwnd drecton, other domnant drectons, habtaton and senste receptors. There should be at least one montorng staton each n the upwnd and n the pre domnant downwnd drecton at a locaton where maxmum ground level concentraton s lkely to occur. In case of expanson project, ar qualty montorng data of 104 observatons a year for relevant parameters at ar qualty montorng statons as dentfed/stpulated shall be submtted to assess for complance of AAQ Standards (annual average as well as 24 hrs). Not Applcable, as no R&R ssues envsaged n the proposed expanson project. Data for Post Monsoon, 2018 shall be furnshed. Not applcable as there s no expanson n thermal power plant. Detals of auxlary fuel, f any, ncludng ts quantty, qualty, storage etc. should also be furnshed. Quantty of fuel requred, ts source and characterstcs and documentary evdence to substantate confrmed fuel lnkage shall be furnshed. For the expanson projects, the complance of the exstng unts to the sad Notfcaton shall also be submtted. Detals of transportaton of fuel from the source (ncludng port handlng) to the proposed plant and ts mpact on ambent AAQ shall be sutably assessed and submtted. If transportaton entals a long dstance t shall be ensured that ral transportaton to the ste shall be frst assessed. Wagon loadng at source shall preferably be through slo/conveyor belt. For proposals based on mported coal, nland transportaton and port handlng and ral movement shall be examned and detals furnshed. The approval of the Port and Ral Authortes shall be submtted. Detals regardng nfrastructure facltes such as santaton, fuel, restrooms, medcal facltes, safety durng constructon phase etc. to be provded to the labour force durng constructon as well as to the casual workers ncludng truck drers durng operaton phase should be adequately catered for and detals furnshed.
