1 Assessment Feedback Incentive exchange (AFIX) Presented by: Carolyn Perry, RN, BSN March 19, Overview Quality Improvement Program for Health Care Providers enrolled in CDC s Vaccines for Children (VFC) program. 1

2 March 19, Goal Increase vaccination rates of children and adolescents with all Advisory Committee for Immunization Practices (ACIP)- recommended vaccines. Reduce missed opportunities to vaccinate. 3 March 19, Local Health Department Immunization Action Plan Performance Measure AFIX visit to at least 25% of county VFC providers annually. Performance Measure 100% of providers met the selection criteria. 50% of providers in the state-wide bottom quartile. 50% Other Provider visits. March 19, % Other Provider AFIX visits based on the following criteria: Providers newly enrolled in the VFC program. Providers with low vaccination coverage rates (measures that do not meet Healthy People 2020 targets). Providers serving a large population (e.g., > 30 patients in the Assessment cohort). Providers motivated to improve their vaccination service delivery. Providers requesting AFIX and QI. Providers with new staff or high staff turnover. 3

4 March 19, AFIX Site Visit Checklist The AFIX site visit checklist provides information about materials that are both required and beneficial in preparing for and conducting an AFIX site visit. It is recommended with each AFIX site visit. What do I bring to an AFIX Site Visit? What do I need to know prior to the AFIX Site Visit? What to leave with the Provider? March 19, Observation Tool A supervisor or experienced Reviewer should observe an AFIX Reviewer during an AFIX review after training and periodically, at a minimum once per year. The state will provide an AFIX Site Visit Observation Tool template. 4

5 March 19, AFIX Components Assessment Feedback Incentives exchange March 19, Assessment A quantitative assessment (assessment of a provider s coverage data and missed opportunities) is required for every AFIX visit. The Feedback and exchange date should be < 7 days from the Assessment Date. Childhood Assessments include patients aged months. Vaccination status is assessed at 24 months of age (the child s 2nd birthday). Adolescent Assessments include patients aged years. Vaccination status is assessed on the date of Assessment. Report all AFIX Feedback and exchange visits in the AFIX Online Tool within ten (10) days of conducting the visit. 5

6 March 19, The AFIX Product The AFIX Product for AFIX assessments will take the place of CoCASA. oit is also available to NYSIIS users in provider offices to run assessments and coverage rates for their own organization. othe AFIX Product creates charts and tables displaying rates and missed opportunities. The video will buffer and then begin to play. Make sure you have speakers or headphones for audio. March 19, Feedback In-person as a face-to-face feedback with all clinic staff who participate in the vaccination process. Review every question in the AFIX Site Visit Questionnaire with the provider staff, even if they answered the Questionnaire prior to the site visit. Select 2-3 QI strategies to incorporate into the QI plan for implementation and follow-up. For each action step, designate a point person who will be responsible for implementing the strategy, monitoring the initiative and motivating staff to complete the assignment within the allotted time frame. 6

7 March 19, Incentives Incentives can be a powerful motivator for improving vaccination coverage rates. Recognize improved performance quality by providers and clinic staff who are making practice-based changes, developing more effective immunization delivery systems, and ultimately improving immunization coverage. Monitor provider progress and offer support. Ensure providers have the necessary resources and information to improve the quality of their immunization services. Record providers status toward implementation of selected QI strategies. 7

8 March 19, Subsequent exchange A continuation of the initial exchange to continue to assess progress toward implementation of the chosen QI strategies. Occurs within an agreed-upon time frame. There are three different approaches for contact during subsequent exchange: Telephone In-person visit Educational visit If the provider does not plan to reach 100% implementation of the selected QI strategies, do not continue to invest time and resources. Document the reason(s) the strategy will not be implemented in the on-line tool and close the AFIX. March 19, Online Tool Assessments, feedback and initial exchanges must be documented in the Online Tool within 10 days of completion. IAP Performance Measure All AFIX Components are Documented completely, accurately, and timely in the AFIX Online Tool. 8

March 19, University of North Carolina AFIX study New York (Erie, Monroe and Westchester counties), Arizona, and Wisconsin. Research Aims Aim 1: Evaluate the impact of AFIX and physician-to-physician (P2P) engagement on HPV vaccination coverage among adolescents. Aim 2: Compare AFIX and P2P engagement on implementation outcomes, including reach, adoption, and cost. Aim 3: Evaluate the effectiveness of delivering AFIX and P2P booster visits at 12-month follow-up for further increasing HPV vaccination coverage. 9
