1 Procedure for Accreditation & Training Registration 01. Accreditation & Training Mission Statement The Accreditation & Training Program is in line with the Trakhees Mission to ensure adherence of rules and regulations mandated by the authority through innovation and use of technology to enhance both performance and customer satisfaction. 02. Introduction: This Accreditation & Training Program is specially designed for the service providers who handle various jobs within the jurisdiction. It is mandatory that all the companies working within PCFC jurisdiction are hereby made aware of their obligation to comply with the requirements to get their employees registered as per the requirements depicted on the prequalification procedures. 03. Purpose: The purpose of this procedure is to facilitate the companies in getting their employees registered as per the requirements depicted on the prequalification procedures. The procedure describes the method to apply & get registered under various categories of Training & accreditation, for the companies working under the jurisdiction of Trakhees. 04. Scope: This procedure is applicable to all service providers under the jurisdiction of Trakhees. 05. Course Fees Refer Appendix Training Duration and Validity Refer Appendix 2. Registration Procedure: 07.1 Registration The Registration Form must be completed in all respect and submitted to EHS- Accreditation at along with all required documents (refer section 08) The applicant must hold qualifications relevant to their job functions and roles as described in the prequalification criteria (see Appendix 2) Trakhees reserves the right with respect to the decision for attendance or registration for Training & Accreditation Submission of any forged documents (certificates, qualifications, experience etc.) will lead to disqualification/ban as well as legal action against the applicant Review and Payment The applicant details will be reviewed by our Assessment Team if qualified to go through our Training Course. It takes 2-5 working days waiting period to obtain assessment results. 3 3.3.2 TRAKHEES Cash Counter International City, Dragon Mart - First Floor Emirates Road, Dubai Ph: Note: Cash Counter Working Hours: hrs (Sunday Thursday) Excluding Public Holidays 07.3 Schedule Receipt copy must be submitted to EHS Accreditation to obtain the training schedule for the applicant Training details such as location map, outline and gate pass will be sent to the client at least one week prior the training scheduled date If the participant cancelled, missed his/her scheduled training or did not complete the session; a reschedule fee of will be applied If the participant wishes to cancel/reschedule the appointed session, then they can do so (without any charges) not later than 7 days before the confirmed date, after which section applies Failure to attend the course within six months from the date of payment will lead to the cancellation of current registration and client shall need to re-register for attending the respective course. Payment will be charged for the re-registration as per the tariff Assessment For SARS examination, the minimum passing marks for the examination will be 60%. Candidate scoring less than 60 % but equal to or more than 50% marks will be entitled to re-appear for the written examination on the next training batch. 4 charged for the re-examination. For candidates who score less than 50% marks in the first attempt, has to re-apply for the SARS course. If the candidate fails on the second attempt, he/she shall be required to reappear the SARS training after a period of 3 months only, provided they are able to demonstrate the steps they have undertaken to develop proficiency in safety. A candidate shall be allowed a maximum of 3 attempts to appear in the examination For PTW Ship Repair Surveyor and Supervisor, the minimum passing marks for the examination will be 60%.If candidate fails on the examination; he/she has to re-apply for the course For FZIO Accreditation, the minimum passing marks for the examination will be 50%. Candidate scoring less than 50% has to re-apply for the FZIO Accreditation course Training Card All applicants who have successfully completed the Training & Accreditation Course will be issued with Accreditation/Training Card. The Accreditation/Training Card remains the property of EHS and must be surrendered upon request. All registered applicants are required to carry their Accreditation/Training Card during working hours and must present if requested by authority In case loss of Accreditation/Training Card, the concerned sponsor must report it immediately to EHS where a replacement card will be issued upon submission of request letter (Please refer to last page, ADF-01B, as sample). Fee of 120 will be at cost, payable prior to the issuance of the card If the registered person changes his sponsor, it must be informed to Trakhees Accreditation department and Accreditation/Training Card needs to be updated with new sponsor s name. Previous card must be surrendered upon collection of amended card Training card must be collected within 6 months from the date of training. Uncollected cards beyond 6 months will be disposed. Re-issuance of card will be at cost of Card Renewal Before the expiration of the card, applicants are required to attend a refresher course only with prior approval by the authority. The updates will be provided to all registered applicants. After attending the refresher course, a new registration card will be issued to applicant in which will be valid for further on Trainings and on Accreditation. 08. Required documents for Registration: Colored copies of the following documents must be submitted by 1. Registration Form (must be computerized) 2. Passport copy with valid visa page. 3. Trade license copy of the sponsoring company 4. Colour photo in JPEG Format, Size 2 cm x 2 cm 5. Detailed CV of the candidate (only for PTW Surveyor, MSAP, SARS, and SSRS application) 6. Authenticated Academic Qualification (only for MSAP, SARS, and SSRS application) 7. Professional Qualification (only for SARS, and SSRS application) 8. Building Permit or Modification Permit copy issued by Trakhees to candidate s sponsor (only for MSAP, SARS, and SSRS application) Rev. 02 October 2016 Page 5 of 11
