Cmpleted tree permit applicatins, alng with the required supprting dcumentatin, must be submitted t the City f Miami s Building Department. Depending n the scpe f wrk, sme prjects can be reviewed the same day while thers may need t be drpped ff fr review. Review: Tree Permit Applicatins can be btained in persn at 444 S.W. 2 nd Avenue, 4 th Flr, Miami, FL r n line at a. Checklists (n the fllwing pages) have been designed t list the supprting dcumentatin needed fr cmpliance. The Checklists are t be used fr the fllwing prjects: New cnstructin with the remval, relcatin r rt pruning f trees (A) New cnstructin with n tree remvals, relcatins r rt pruning (B) Stand Alne Tree Permit fr trees being remved, relcated r pruned that s unrelated t cnstructin (C) Tree planting in the City Right-f-Ways (Stand Alne) (D) *A cmbinatin f checklists may be applicable depending n the scpe f wrk *Master Building Plans must include the Tree Permit plans as it will be prcessed as a sub permit. 2. Tree Permit Applicatins are reviewed by the Envirnmental Resurces Divisin either thrugh walkthrugh r drp-ff. 3. Applicatins requiring additinal infrmatin and/r crrectins will be returned t the applicant t crrect and resubmit fr further review. Status f applicatins can be viewed using the City s ibuild system at 4. Once the applicatin is apprved, the Department will issue a ntice f Intended Decisin. This ntice is psted nt the subject prperty r adjacent t fr ten (10) calendar days which prvides all interested parties the pprtunity t review and appeal the Intended Decisin. Issuance f Permit: 5. If n appeals are received within the appeal perid, the applicant is ntified t pick up the applicatin and pay fr the permit. 6. Once payment is made, the permit is issued accrdingly.

2 Page 2 f 6 Final Inspectin: 7. The final inspectin shall be cmpleted n mre than six (6) mnths frm the apprval date f the tree permit. This inspectin shall be apprved prir t requesting a Temprary Certificate f Occupancy (TCO) r Certificate f Occupancy (CO) frm the Building Department. 8. T request the final inspectin, the applicant shall call the number indicated n the permit card r the Envirnmental Resurces Divisin at The inspectin is cnducted within hurs f the requested date. 10. Upn apprval f the final inspectin, the permit is apprved and finalized.

3 Page 3 f 6 Checklist (A) NEW CONSTRUCTION Tw (2) Signed and Sealed sets f the fllwing plans inserted in the Master Building set: Tree/Bundary survey drawn t scale, identifying the tree species, lcatin, and listing the height, spread and diameter f all existing trees including all trees within the right-f-way. This survey shall be prepared by a prfessinal Land Surveyr, licensed in the state and dated n mre than 12 mnths. The Base Building Line, establishing the prperty bundaries, shall be marked n said Bundary Survey by the Building r Public Wrks Department. Dispsitin Plan / Site Plan- drawn t scale, identifying and listing all existing trees and specifying the cnditin f each tree and whether said trees are t remain, t be remved, r t be relcated. The tree dispsitin plan shall superimpse all prpsed new cnstructin. Plan shall include lt dimensins, Center Line, Mnument Line and adjacent street names. Tree Replacement/Landscaping Plan - drawn t scale, illustrating all prpsed new cnstructin, new lcatins f relcated and newly installed trees. Said plan shall include lt dimensins, Center Line, Mnument Line and depict adequate tree spacing between trees, hardscapes and utility lines. If the minimum tree requirements are nt prvided n-site, applicant must meet alternative requirements via ff-site replacement r Tree Trust Fund. Tree Prtectin / Planting / Maintenance Plan identifying all trees t remain n site with the apprved tree prtectin plan and prtectin barrier detail. Said plan shall als include the details n newly installed planting materials and a ne (1) year maintenance guarantee. *All plans shall be prepared by and bear the seal f a registered Landscape Architect currently licensed in the state r by persns authrized by F.S. ch. 481 t prepare landscape plans r drawings. Tree Assessment Reprt (when applicable) - prviding an assessment n all existing trees with the trees specificatins (cnditin, species, size, rt ball etc.) and includes clr phtgraphs in supprt f the assessment. Fr trees and/r palms prpsed fr relcatin, the reprt shall include relcatin specificatins, in accrdance t ANSI A300 Standards. Additinal infrmatin such as the Tree Prtectin Zne (TPZ), Critical Rt Zne (CRZ), tree prtectin and/r pruning recmmendatins, may be required fr trees t remain and/r t be relcated. Said reprt shall be dated n mre than three (3) mnths.

4 Page 4 f 6 CHECKLIST (B) NEW CONSTRUCTION WITH NO TREE IMPACTS (All trees n site will remain and be prtected) New Cnstructin Plans (including Demlitin): Tree/Bundary survey drawn t scale, identifying the tree species, lcatin, and listing the height, spread and diameter f all existing trees including all trees within the right-f-way. This survey shall be prepared by a prfessinal Land Surveyr, licensed in the state and dated n mre than 12 mnths. Written Statement - frm prperty wner r authrized representative indicating the reasn(s) fr the wrk Dispsitin Plan Use an existing bundary/prperty survey r Site Plan, drawn t scale t depict: The lcatin f all existing structures, prperty bundaries, adjacent rads and any verhead and/r undergrund utilities. Bundary Surveys must have the Base Building Line marking. The lcatin, species, height and DBH f all existing trees n site. This includes trees within the adjacent rightf-way and which trees are t be relcated, remved r t remain The prpsed lcatin, species, height and DBH f all relcatin and required replacement trees t be planted. A Cvenant will be issued fr trees being remved, relcated, pruned and/r planted in the right-f-way Tree Assessment Reprt (when applicable) - prviding an assessment n all existing trees with the trees specificatins (cnditin, species, size, rt ball etc.) and includes clr phtgraphs in supprt f the assessment. Said reprt shall be dated n mre than three (3) mnths. Measuring Tree Diameter 1. Measure the circumference (like measuring yur waist) f the tree at 4 ½ feet abve the grund level. 2. Use a calculatr t divide the circumference by This will give yu the diameter at breast height (DBH). DBH = Circumference Fr example, a Live Oak with a 45 in. circumference wuld have a diameter f 14.3 in.

Plan shall illustrate the lcatin f all existing structures and any verhead and/r undergrund utilities.
