1 DEPARTMENT OF WATERSHED MANAGEMENT SITE DEVELOPMENT PLAN REVIEW SUBMITTAL REQUIREMENTS ONE & TWO FAMILY RESIDENTIAL I. General 1. Cmplete plans- All plans submitted fr permit review must be cmplete and prvide all applicable items listed in this checklist. 2. Survey- All plans fr new buildings r prpsing wrk within r near a stream buffer, fldplain, wetland r sewer easement must include a recent bundary and tplgical survey drawing shwing existing cnditins signed, sealed and dated by a Gergia registered land surveyr. Such survey is highly recmmended fr ther prjects needing Site Develpment review and may be required after plan review. If the lt is lcated within a subdivisin, a cpy f the final plat shuld als be included. 3. Sealed plans- Grading plans must be signed and sealed by a Gergia registered engineer, land surveyr, architect, landscape architect, r ther qualified prfessinal as apprpriate. Plans with disturbed area greater than 1.0 acre r fr new buildings r prpsing wrk within r near a stream buffer, fldplain, r wetland must include the certificatin f the GSWCC Level II Design Prfessinal (name, number and expiratin date f certificate), wh prepared the ersin, sedimentatin & pllutin cntrl (ES&PC) plan. 4. Demlitin plans- All plans fr redevelpment must include a demlitin plan, clearly indicating all existing structures and site features t be demlished and thse structures and site features that will remain. II. Grading/Site Plan 1. General requirements- A grading plan is required fr all prjects invlving any significant changes f grade. The grading plan shwing must shw existing and prpsed grund cnturs and elevatins fr cut and fill peratins, and all pertinent infrmatin related t grading and infrastructure. A grading plan is nt required fr minr imprvements with n grade changes and minimal land disturbance. All site plans must shw: a) All buildings and structures clearly identified as existing r prpsed; b) Driveways, sidewalks, patis and ther paved areas; c) All prpsed site imprvements; d) Apprpriate ersin and sediment cntrl plan; and e) All existing and prpsed easements (drainage, sewer, etc.). Grading plans must als shw: f) Lcatins and tp elevatins f sanitary and strm sewers and structures; g) Tp and bttm f fting and grund elevatins fr retaining walls; h) Finished flr elevatins fr existing and prpsed building structures; i) Lcatin and size f existing public sanitary sewer (fr sewer cnnectin when required); and j) Dispsitin f excavated materials. 2 2. Retaining walls- Prpsed retaining walls greater than three (3) feet in height must have fully detailed design drawings; fr lwer retaining walls a typical detail may be used. Fting details are required fr any wall alng a prperty line. 3. Dirt Statement- Identify the grss quantities fr each f cut, fill, demlitin debris, and haul vlumes in cubic yards. 4. Graded slpes- Every grading plan must include the fllwing statement: N graded slpe shall exceed 2h: 1v. III. Sewers 1. Sanitary sewers- All sanitary sewers and appurtenances shall cnfrm t COA Standard Details and such details shall be shwn n plans. a) Shw lcatin, size and slpe f existing r prpsed sanitary sewer cnnectins. b) New cnnectins t the public sewer shall be made with a wye fitting, and shall nt exceed 15 degrees frm perpendicular t sewer main (r main line). c) New cnnectins t public sewer manhles are nt permitted. d) Prvide COA standard clean-ut n existing r new sanitary sewer lateral, lcated just inside the prperty line, and shw n plan. e) Permit required fr new sewer cnnectins (see Permit sectin f checklist). 2. Cnstructin ver sewers- Cnstructin f buildings r structures ver existing and prpsed sewers r within drainage easements is prhibited. Prpsed structures must be lcated cmpletely utside all easement limits. Fr existing sewers deeper than 10 feet, the prpsed cnstructin plans must demnstrate that the bttm f a prpsed structure s fting is belw a line running at a 1:1 slpe frm the bttm utside edge f existing sewer. 3. Septic systems- When public sewer is nt available and a septic system is prpsed fr installatin r renvatin the plans must shw the lcatin f the septic tank and drain fields. A stamped, apprved cpy f the septic system installatin plan and a cpy f the applicable Cunty Health Department permit must be included with the building permit applicatin. Such plans cannt be reviewed r apprved withut prir Health Department apprval. IV. Fld hazard areas 1. Applicability- Any prpsed cnstructin, recnstructin, repair, mdificatin r demlitin f structures within a Special Fld Hazard Area is regulated by City Cde Chapter 74 Article VI Fld Area Regulatins. 2. Prhibitins- New building cnstructin r any placement f fill within fldplains is prhibited. 3. Delineatin f fldplains- As applicable, delineate the limits f the 100-year fld and the 100-year base fld elevatins(s). a) Demnstrate that prpsed cnstructin is 2-feet higher and 15-feet hrizntally away frm the 100-year base fld cntur. b) Fr prperties with creeks r streams that are nt identified within the FEMA fld maps, a fld study signed and sealed by a Gergia registered prfessinal engineer is required that identifies the 100-year high water elevatin(s) and cntur lcatin. 3 rebuilding n an equivalent ftprint area. The elevatin f such structure s lwest flr shall be the greater f either: three (3) feet abve the base fld elevatin r tw (2) feet abve the highest bserved fld. V. Ersin, Sedimentatin, and Pllutin Cntrl (ES&PC) Plan 1. General requirements- Prvide an ersin and sediment cntrl plan that identifies ersin and sediment cntrl practices, in cnfrmance with and using the unifrm cding symbls frm the Manual fr Ersin and Sediment Cntrl in Gergia, Chapter 6. a) Prvide vicinity map identifying the site relative t the surrunding streets. b) Identify the name, address, and phne number f the general cntractr n the plan. c) Identify the ttal acreage f the site and the acreage f disturbed area. d) Delineate and label the limits f cnstructin. e) If prpsed cnstructin activity invlves ne acre r greater r is part f a cmmn develpment (secndary r tertiary permittees), the ES&PC plans must meet all requirements fund in the applicable NPDES General permit. 2. Best Management Practices- Practices typically required fr single family residential cnstructin include but are nt limited t: a) Cnstructin Exit (C); b) Perimeter Cntrl Silt Fence (Sd1-C); c) Disturbed Area Stabilizatin (Ds1, Ds2, Ds3, Ds4); d) Stne Check Dams (Cd-S) in areas f cncentrated flw; and e) Other BMPs as apprpriate fr specific site. 3. Required ntes- Prvide the fllwing ntes n the ersin and sediment cntrl plan: a) The escape f sediment frm the site shall be prevented by the installatin f ersin and sediment cntrl measures prir t r cncurrent with land-disturbing activities. b) Ersin cntrl measures will be maintained at all times. If full implementatin f the apprved plan des nt prvide fr effective ersin cntrl, additinal ersin and sediment cntrl measures shall be implemented t cntrl r treat the sediment surce. c) Disturbed areas left idle 14 days shall be stabilized with temprary vegetatin and mulch; disturbed areas remaining idle 30 days shall be stabilized with permanent vegetatin. d) Ersin and sediment cntrl measures shall be inspected at least weekly, after each rain, and repaired as necessary. e) Additinal ersin and sediment cntrl measures shall be installed if determined necessary by n-site inspectin. f) Silt fence shall be Type C as per the Manual fr Ersin and Sediment Cntrl in Gergia, and be wire reinfrced (see attached detail). 4. State waters / wetlands- Delineate all state waters and wetlands that are within 200 feet f prpsed prject limits. Call Site Develpment ( ) fr a determinatin f state waters if needed. 5. Buffers Delineate all applicable buffers n state waters and wetlands: a) State Waters 25 ft undisturbed buffer measured frm the pint f wrested vegetatin. b) Perennial and intermittent streams 75 ft undisturbed buffer measured frm the pint f wrested vegetatin. c) Jurisdictinal wetlands 25 ft undisturbed buffer measured frm the edge f delineated wetland. d) Lng Island Creek Basin Streams that are a tributary t the water supply and within a 7-mile radius f the reservir bundary shall be prtected by the fllwing criteria: One and Tw-Family Residential Submittal Requirements -- Updated 5/16/2013 Page 3 f 6

4 i. A natural vegetative buffer shall be maintained fr a distance f 100 feet n bth sides f the stream. ii. N impervius surface shall be cnstructed within a 150 ft setback area measured hrizntally frm the stream banks. iii. Septic tanks and septic tank drain fields are prhibited in the setback area. e) Any prpsed nn-exempt buffer encrachment will require an authrized encrachment frm the City f Atlanta (75 ft buffer), and a state waters buffer variance frm GA EPD (25 ft buffer when applicable) prir t permit issuance. f) Fr all prpsed buffer encrachments, plans must demnstrate hw cntractr will access and withdraw frm, stabilize, and restre the area (GDOT apprved matting and blankets in cnjunctin with native riparian vegetatin preferred) after cnstructin. V. Strmwater Management 1. Minimum Requirements- Fr all prjects, the fllwing requirements must be adhered t when managing strmwater: a) Lts and buildings shall be develped in a manner t ensure that strmwater exiting individual parcels r lts under pst-develped cnditins des nt adversely impact the adjacent parcels r lts as a result f cncentrated flws, flding, ersin, r depsits f silt r sediment. b) The strmwater discharge frm a dwnsput, cistern, r any water cllectin device shall be lcated a distance f n less than ten feet frm cmmn prperty line and riented s directin f cncentrated flw is nt tward the adjacent prperty line. c) Discharge frm any dwnsput described in (a) must be dissipated, infiltrated, r diverted such that flws will nt be cncentrated. d) N persn shall erect, cnstruct, r therwise permit any bstructin that prevents the natural r cntained flw f water t any cmpnent f the strmwater system f the City f Atlanta, unless such bstructin is allwed as part f an apprved permit. 2. Green Infrastructure Requirements- Prjects invlving the cnstructin f a new hme r the creatin, additin, r demlitin and replacement f mre than 1,000 ft 2 f impervius cver must capture the first 1.0 f runff frm the new r replaced impervius areas, and infiltrate, evaptranspire, r reuse this cllected water in accrdance with the City f Atlanta s Strmwater Guidelines: Green Infrastructure fr Single Family Residences. Only the majr impervius areas f the prperty need t be treated. This includes the rftp f the main structure and garage, parking areas, and paved pati areas. It excludes minr ut buildings, walkways, small miscellaneus paved areas, and the entry driveway leading frm the rad t parking and turn arund areas. 3. Green Infrastructure Details- Apprved details frm the Green Infrastructure fr Single Family Residences must be selected, filled ut, and attached t each cpy f the site plan. Site plans must reference these details and delineate the flw rute f water t verify feasibility f the selected practice(s). If a manufactured prduct is t be used, a cpy f the manufacturer s specificatins must als be attached. 4. Drainage system guidelines- Strmwater cnveyance practices, which may include but are nt limited t culverts, strmwater drainage pipes, catch basins, drp inlets, junctin bxes, headwalls, gutter, swales, channels, ditches, and energy dissipaters shall be prvided when necessary fr the prtectin f public right-f-way and private prperties adjining prject sites and/r public rights-f-way. Strmwater cnveyance practices that are designed t carry runff frm mre than ne parcel, existing r prpsed, shall meet the fllwing requirements: a) Methds t calculate strmwater flws shall be in accrdance with the Gergia Strmwater Management Manual; One and Tw-Family Residential Submittal Requirements -- Updated 5/16/2013 Page 4 f 6

5 b) All culverts, pipe systems and pen channel flw systems shall be sized in accrdance with the strmwater management plan, pursuant t Sectin , using the methds included in the Gergia Strmwater Management Manual; and c) Design and cnstructin f strmwater cnveyance facilities shall be in accrdance with the criteria and specificatins fund in the Gergia Strmwater Management Manual and applicable sectins f the Castal Strmwater Supplement. VI. Permits and Bnds 1. Haul rute- A Haul Rute Permit is required frm the Traffic and Transprtatin Sectin ( ) f the Department f Public Wrks (DPW-Traffic) when mre than 500 cubic yards f material is hauled t r frm the site. 2. Qualified Cntractr- A Qualified Cntractr Permit is required frm DPW-Traffic fr cnstructin f new sewer cnnectin, sidewalks, driveway aprn r ther wrk in the public rightf-way. Qualified Cntractr permits require prf f an in-frce general liability insurance plicy in the amunt f $3 millin, and valid business license and payment f applicable fees. The City f Atlanta shall be shwn as the certificate hlder n the plicy. 3. Sewer cnnectin- A sewer cnnectin permit is required frm DPW-Traffic fr new sewer cnnectins. New sewer service requires capacity certificatin prir t apprval. 4. Septic system- A septic system permit issued by the Cunty Health Department having jurisdictin is required fr installatin r ther wrk n private septic sewerage systems. 5. Perfrmance Bnd- An Ersin Cntrl Perfrmance Bnd is required in the amunt f $3,000 per disturbed acre fr sites with disturbed acreage f greater than 1.0 acres r where the prpsed cut and fill quantities exceed 500 cubic yards ($3,000 minimum). Bnd must be issued by a licensed surety with pwer f attrney in the State f Gergia. The bnd ensures that disturbed areas can be stabilized in the event the wner r cntractr cannt r will nt stabilize the site. This Surety Bnd must have a raised seal and bnd number. 6. NOI- A Ntice f Intent (NOI) fr cverage as a secndary r tertiary permittee fr cnstructin within a cmmn develpment must be sent t the EPD Muntain District-Atlanta Satellite ffice 14 days prir t beginning land disturbance activity (fr mre infrmatin, visit A cpy f the NOI and certified mail receipt (r equivalent) must be submitted t the City prir t permit issuance and an additinal cpy present during the Pre-Cnstructin meeting and psted n site thrughut the life f the prject. VII. Site Develpment Fees 1. Site Develpment Fees- Cmmercial, Multi-family and Subdivisins Site plan review $100 Site inspectin $130 Inspectin f new sewer cnnectin $250 Sewer Capacity Certificatin new single family residential $600 Starting wrk withut a permit Duble fee VIII. Plan Resubmittals 1. Red-lines- Refer t the red-lined plans enclsed with this checklist fr additinal site plan review cmments. 6 2. Required plans- Fur (4) sets f plans are required fr Site Develpment apprval. IX. Cnstructin and Final Inspectin 1. Pre-cnstructin meeting- Befre starting any land-disturbing activities, the cntractr is required t schedule a pre-cnstructin meeting with the Envirnmental and Cnstructin Enfrcement Divisin. Call Failure t schedule may result in a Stp Wrk Order, Citatin, r permit revcatin. 2. Revisins t permitted plans- Revisins t permitted plans must be submitted thrugh the Office f Buildings. Fr Site Develpment purpses, fur (4) sets f plans are required; hwever, additinal sets f plans may be required by the Office f Buildings fr ther agency reviews. 3. Final stabilizatin and final sign-ff- Fllwing the cmpletin r cessatin f land-disturbing activities at a site, all unpaved areas and areas nt cvered by permanent structures shall be unifrmly cvered (ne-hundred (100) percent f the sil within the disturbed area) with permanent vegetatin with a density f seventy (70) percent r greater, r equivalent permanent stabilizatin measures, including, but nt limited t, rip rap, gabins, permanent mulches, r getextiles. A final sign-ff by the Department f Watershed Management shall be required fr all prjects invlving land-disturbing activities, and said sign-ff shall ccur prir t the issuance f a Certificate f Occupancy r the recrding f any final plat.
