You will find within the information package: An overview of Murrumbidgee Primary Health Network A copy of the advertisement Selection criteria and position description About us Murrumbidgee Primary Health Network (MPHN) is one of 31 new primary health care organisation being established with the key objectives of: Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and Improving coordination of care to ensure patients receive the right care in the right place at the right time. The Department of Health has set PHNs six (6) key priority areas for targeted work; Mental health, Aboriginal and Torres Strait Islander health, Population health, Health workforce, ehealth and Aged care. The MPHN will be informed by our local health professionals and communities through four regionallybased clinical councils, and 33 Local Health Advisory Committees informing a single Community Advisory Council. We will meet our objectives by undertaking population health planning and priority setting. Our decision making in the region will be based on reliable health data and evidence and on criteria that is open to public scrutiny. We will establish an ongoing capacity to scan and monitor trends in service needs and use, personal risk behaviours and other areas that likely to impact on the future health status of the community. Region Map MPHN covers a land area of 124,412 square kilometres and services a population of 240,755 covered by 28 Local Government areas. The organisation will work across fours sectors within the MPHN region, they include the Riverina Sector, Wagga Wagga Sector, Border Sector, and the Western Centre. 4 Position Description Position Title: Location: Reports to: Position Objective: Under the direction of the Executive Manager Primary Health, the Central Intake Mental Health Clinician is responsible for undertaking individual assessments and providing focussed psychological strategies for a diverse range of clients with mental health needs. This role will also work collaboratively with key stakeholders including General Practitioners, Local Health District Clinicians, non government organisations (NGOs) and government organisations to ensure high quality outcomes for clients. Role Requirements: Wagga Wagga Executive Manager Primary Health Conditions of Employment: firsthealth Enterprise Agreement Letter of offer Position Description Use strong assessment skills to inform the allocation of referrals to the most clinically appropriate commissioned service or facilitate a referral to another external service under the No Wrong Door philosophy in collaboration with other service system stakeholders Provide clinical expertise to more complex referrals Ensure that clients receive seamless and timely access to services Provide education about mental health issues to GPs, clients, families and significant others of clients. Provide outreach, face to face and phone based liaison as required in order to determine client needs and goals for treatment (including screening) Undertake triage/intake activities Provide clinical advice and support for consumers using clinician-assisted digital supports Participate in clinical audits of commissioned services as needed Undertake continuing education as required to ensure job skills and knowledge remain current Attend scheduled meetings, conferences and work related functions within and outside usual work hours including travel to other regions where overnight stays may be required Contribute to a culture of continuous improvement by regularly reviewing administrative processes and procedures, and provide recommendations to improve efficiency Contribute to Workplace Health and Safety (WHS) by complying with the Work Health and Safety Act 2011, and the organisations WHS management system Other duties as directed This position description is only intended to provide an outline of the key responsibilities of the position. Knowledge, Skills and Experience Psychologist / Social Worker / Occupational Therapist / Mental Health Nurse. Experience in assessment and diagnosis of mental health disorders. Experience in the delivery of evidence-based treatments for those with mental health concerns. Knowledge of, and commitment to, primary health care principles Well-developed written, verbal and interpersonal skills. Ability to work independently in the delivery of clinical services, but operate within a multidisciplinary team.

5 Ability to liaise, consult and provide advice to GPs, clients, families and significant other health professionals. Knowledge of current Mental Health Reforms Current NSW Drivers Licence
