The objective of our evaluation was to identify asbestos which may require remediation in regard to the planned demolition of the structure. Additional hazardous materials identified at the site have also been noted for proper removal/disposal as required. 4 HAZARDOUS MATERIALS EVALUATION EXECUTIVE SUMMARY The suspect ACMs identified and sampled during the course of our investigation consisted of: Roofing Shingle and Felt Paper Roll Roofing & Felt Paper Roof Mastic Duct Sealant Concrete (Chimney Top) Formica and Adhesive Decorative Stone Panel Adhesive Caulk Remnant Floor Tile Mastic Ceramic Floor Tile, Grout, and Float Floor Tile under Ceramic Tile Wall Texture Joint Compound Drywall Ceiling Texture Stone Flooring Stucco and Felt Paper Concrete Masonry Unit (CMU) Block and Grout Decorative Stone Wall Tile Acoustic Ceiling Material Acoustic Overspray Sheet Vinyl Flooring Brick and Mortar Fire Brick Following the visual portion of the survey, a total of 80 bulk samples were collected from areas representing the homogeneous use of suspect building materials. Polarized Light Microscopy (PLM) indicated the presence of asbestos in excess of 1% by weight in eight of the materials tested. 5 HAZARDOUS MATERIALS EVALUATION Acoustic Ceiling Material Acoustic Overspray in Ducts and Electrical Junction Boxes It is the opinion of Converse, based on our understanding of the National Emission Standards for Hazardous Air Pollutants (NESHAP), that the following ACMs will require removal by a certified Nevada licensed abatement contractor in order to comply with Federal, State, and County regulations prior to the demolition process. It may also be necessary to perform third party air quality sampling after the removal activities to comply with Nevada OSHES and Clark County Department of Air Quality (CCDAQ) regulations. Ceiling Texture (Remnant from previous renovation) Wall Texture (Remnant from previous renovation) Acoustic Ceiling Material (Remnant in Laundry Closet from previous renovation) Remnant Overspray in Ducts Electrical and Junction Boxes It is the opinion of Converse that with the use of proper engineering controls (i.e. wet methods, perimeter air monitoring, and proper training of the demolition contractor), the approval of Nevada OSHES, CCDAQ, and if no recycling of waste materials is permitted, the following non-friable ACMs may be allowed to stay in place through the demolition process (however if the slabs will be left in place after building demolition any tile and mastic should be removed from the slab before building demolition): Roof Mastic Remnant Floor Mastic Joint Compound Damaged suspect lead-based paint (LBP) was identified and sampled during the course of our investigation and consisted of tan and white paint located on the fascia. The paint film coatings tested exhibited concentrations below EPA and HUD definition for lead based paint but above action levels as set by Fed-OSHA. The levels of lead did not meet the Environmental Protection Agency (EPA) definition of lead-based paint (e.g., > 5,000 parts per million (ppm) of lead); however, the Occupational Safety and Health Administration (OSHA) regulates employee exposure to materials with any detectable levels of lead and sets an action level of 600 ppm that triggers certain work practices. In regard to the future demolition of the structure, it is the opinion of Converse that the loose and/or peeling paint located on the exterior of the structure will require stabilization and/or component removal by a certified Nevada licensed lead abatement contractor utilizing workers that have undergone the necessary lead training and are EPA certified prior to the demolition process. Waste resulting from the stabilization must be characterized and profiled for lead content prior to determining the transportation and disposal methods. Other suspect LBP paint film coatings in fair condition may remain in place during the demolition process. Those building components with intact paint can be disposed of as non-hazardous construction waste NDOT_1821 Loch Lomond Way_DHM ii

6 HAZARDOUS MATERIALS EVALUATION The demolition contractor must be notified that they will be required to follow OSHA methods of compliance as stated in 29 CFR regarding employee protection. Additional hazardous materials that have been identified which will require proper disposal prior to the demolition process consist of the following: 2 Smoke Detector 0 Fluorescent Light Bulbs 0 Light Ballast 16 Compact Fluorescent Light Bulbs 1 HVAC Unit (Refrigerant) 0 Mercury-containing Thermostats All light fixtures must be evaluated for polychlorinated biphenyl (PCB) containing ballasts during the abatement process NDOT_1821 Loch Lomond Way_DHM iii

7 HAZARDOUS MATERIALS EVALUATION HAZARDOUS MATERIALS SURVEY SURVEY On June 9 & 10, 2015, Converse Consultants (Converse) conducted a visual and sampling survey at the residential structure located at 1821 Loch Lomond Way in Las Vegas, Clark County, Nevada. The objective of our evaluation was to identify asbestos and/or damaged lead-containing paint film coatings which may require remediation in regard to the planned demolition of the structure. Additional hazardous materials identified at the site have also been noted for proper removal/disposal as required. The survey was performed in general accordance with our proposal dated July 18, 2012 and your written authorization to proceed on October 15, The site consisted of an approximate 2,116 square foot (SF) slab-on grade residential structure. The structure was constructed in Interior walls are finished with textured drywall, and ceramic tile. The exterior of the building is stucco clad with asphalt shingle roofing with asphaltic mastic. METHODOLOGY ASBESTOS A visual survey was performed to classify each suspect ACM by location and condition in order to establish homogeneous areas for bulk sample collection. Homogeneous areas refer to areas in which similar application, age, and appearance of building materials exist. Following the visual portion of the survey, a total of 80 bulk samples were collected from areas representing the homogeneous use of suspect ACMs. A summary of these samples regarding each material location and condition is found in the chain of custody documentation in Appendix A. In general, suspect ACM bulk samples were obtained by (1) adequately wetting the sample area with a water and surfactant mixture and (2) placing bulk pieces of the building materials into labeled plastic bags. Where possible, bulk samples were obtained to the depth of the affected area. Bulk samples were transferred to an analytical laboratory with continuous chain-of-custody documentation. Our sampling methodology followed the general guidelines for bulk asbestos sampling as presented in Section 40, Part 763 (AHERA) of the Code of Federal Regulations (CFR) NDOT_1821 Loch Lomond Way_DHM 1

8 HAZARDOUS MATERIALS EVALUATION LBP A visual survey was performed to identify damaged suspect lead-containing paint film coatings in poor condition (loose and/or peeling) at the site. Sampling of loose and/or peeling paint film coatings was conducted to determine lead concentrations and whether the paint film coatings would require stabilization prior to the demolition process. Two paint chip samples were collected. LBP sample collection was conducted by removing the peeling/damaged paint film coating with a hand chisel and then uplifting the representative paint layer. The paint was removed as closely as possible to the substrate. Once the samples were collected, they were transferred to a labeled, sealable sample bag. Following collection, each sample was assigned a field sample number and listed on a laboratory submittal form. A summary of the samples is presented on the EMS laboratory submittal form located in Appendix B. All other paint film coatings observed appeared to be intact and in good condition. Our sampling methodology generally followed the Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing published by the Department of Housing and Urban Development (HUD) in ASBESTOS AND LBP SURVEY RESULTS ASBESTOS The 80 bulk samples collected from the Property of suspect homogeneous ACMs were submitted to the Converse asbestos lab located in Reno, Nevada for analysis by Polarized Light Microscopy (PLM US EPA Method 600/R-93/116). The Converse asbestos lab is accredited by the U.S. Department of Commerce, National Institute of Standards and Technology (NIST), under the National Voluntary Laboratory Accreditation Program (NVLAP) for bulk asbestos analysis. Per regulations, each layer of a sample must be analyzed as a separate material. Due to layering exhibited in the 80 samples, a total of 172 separate analyses were performed on the samples. A summary of the materials sampled and the analytical results is presented in the following table. LBP The paint chip samples collected from the Property was submitted to EMS Laboratories, Inc. located at 117 West Bellevue Drive, Suite #3, in Pasadena, California for analysis by atomic absorption spectroscopy Method EPA 3050M/7420. EMS Laboratories, Inc. is EPA-NLLAP accredited and enrolled in and proficient throughout the American Industrial Hygiene Association Lead Analysis (101634) ELPAT Program. A summary of the materials sampled and the analytical result is presented in the following table. CONCLUSIONS AND RECOMMENDATIONS ASBESTOS Current State and Federal standards define an asbestos-containing material as any material containing asbestos in excess of one percent by weight. It is noted that Federal OSHA regulates worker exposure to airborne asbestos fibers with Permissible Exposure Limits (PELs) and requires specific work practices and procedures, per 29 CFR , when disturbing materials containing asbestos; State of Nevada protects building occupants from airborne asbestos exposure. CCDAQ does not allow any airborne asbestos fiber generation NDOT_1821 Loch Lomond Way_DHM 6

13 HAZARDOUS MATERIALS EVALUATION ACMs containing greater than 1% asbestos are required to be abated prior to the demolition/renovation or remodeling activities that will impact or disturb the asbestoscontaining materials resulting in the creation of airborne asbestos fibers. It is the opinion of Converse, based on our understanding of the National Emission Standards for Hazardous Air Pollutants (NESHAP), that the following ACMs will require removal by a certified Nevada licensed abatement contractor in order to comply with Federal, State, and County regulations prior to the demolition process. It may also be necessary to perform third party air quality sampling after the removal activities to comply with Nevada OSHES and CCDAQ regulations. 14 HAZARDOUS MATERIALS EVALUATION All asbestos abatement workers must have current asbestos training documentation, current medical exams and releases, and current fit tests for the use of personal protective equipment (PPE). The asbestos abatement contractor shall be responsible for estimating and verifying dimensions and quantities of ACMs to be abated. The asbestos abatement contractor shall also be responsible for providing any required notifications in order to comply with CCDAQ and NV-OSHA regulations. In addition, Converse recommends that asbestos abatement activities be monitored by a NV-OSHA certified third party consultant that will document the asbestos abatement contractor s use of 40-hour asbestos trained workers, engineering controls and work practices, perform final visual inspections for completeness of the abatement work, perform final air clearance testing for re-occupancy and monitor proper waste handling, transport, and disposal. LBP The EPA and HUD define LBP as those materials, which contain greater than 1.0 mg/cm 2, 0.5%, or 5,000 parts per million (ppm) lead. Fed-OSHA defines LBP as those materials which contain greater than 0.6 mg/cm 2 or 600 ppm lead, which consequently triggers their lead regulation requiring training, personal protection equipment, and specific work practices. Damaged suspect lead-based paint (LBP) was identified and sampled during the course of our investigation and consisted of pink pant located on the facia of the south roof dormer and off-white paint located on the eve of the south roof dormer. The paint film coatings tested exhibited concentrations below EPA and HUD definition for lead based paint. The off-white paint exhibited concentrations above action levels as set by Fed- OSHA. The levels of lead did not meet the Environmental Protection Agency (EPA) definition of lead-based paint (e.g., > 5,000 parts per million (ppm) of lead); however, the Occupational Safety and Health Administration (OSHA) regulates employee exposure to materials with any detectable levels of lead and sets an action level of 600 ppm that triggers certain work practices. LIMITATIONS This report has been prepared for the sole benefit and exclusive use of the Nevada Department of Transportation as it pertains to the Project Neon property located at 1821 Loch Lomond Way in Las Vegas, Clark County, Nevada. Our services have been performed with generally accepted practices in the environmental sciences. No other warranty, either expressed or implied, is made. Converse is not responsible for any claims or liable for any claims or damages associated with the accuracy or completeness of information provided by others. This letter report should not be regarded as a guarantee that no further asbestos-containing materials and/or lead-based paint, beyond that which was suspected to be present (and sampled) during our investigation, is present at the Property. Other suspect materials may be uncovered that were previously hidden during renovation or demolition. Additional samples of these materials should be collected and analyzed for asbestos and/or lead-based paint if this occurs. In the event that changes in the nature of the Property occur, or additional relevant information about the Property is brought to our attention, the conclusions and recommendations contained in this assessment may not be valid unless these changes and additional relevant information are reviewed and the recommendations of this assessment are modified or verified in writing. Converse Consultants follows EPA Method EPA/6001R 93/116. July 1993 and EPA/6001M December Each sample was initially examined under a stereoscopic m;croscopic at a magnification of 10x to 60x. Fibrous material was examined fof morphology and content. Portions of each sample were immersed in a flukj with a known refractive index. The sample was examined under polarized tight using a Oylmpus BHT PLM microscope with a McCrone Dispersion Staining objective under 100X magnification. Optical characteristics of the fibrous material were examined to determine the mineralogy of the fiber. The observed optical charactenstjcs Include angles of extinetion, signs of ek>ngation and dispersion staining cotors.
