2 Identified Priority 1 Concerns - The internal columns at ground level are overstressed. - Cracks on 4 th floor and 5 th floor columns.
3 1 st Priority 1 Concern The internal columns at ground level, shown below, are close to being overstressed. The size of these columns is 11 x 11 at ground floor and carries 5 storeys of this building with 19 6 to 19 7 and to 12 0 length spans. And this building possibly dated from 1960 s hence the strength of concrete and reinforcing steel are not known. For the purposes of this report, we have continued to use the Accord guidelines. Being overstressed In situ Drawing Size 11 x 11 N/A From the information above, for 5 storeys, the calculation check of these columns show that they are close to being overstressed (FoS = 1.20). Vertical rebar 8 Ø12mm (1.20%) N/A Aggregate brick N/A Remark: The sizes of the column s rebar are assessed to be similar to those exposed at top level.
4 2 nd Priority 2 Concern 4 th Floor 4 th Floor 5 th Floor Cracks on 4 th floor and 5 th floor columns noted. The cause of cracks need to be investigated as part of a DEA, a Detailed Engineering Assessment.
5 Identified Priority 2 Concerns - Cracks on 5 th floor beams and slab soffits. - Incomplete structural drawings. - No loading plans exist for the structure. - Capacity check of roof slab supporting water tanks - Stability and capacity of lightweight roof. - Anchorage of communication pole at top level.
6 1 st Priority 2 Concern 5 th Floor 5 th Floor 5 th Floor Hairline cracks on 5 th floor beams and slab soffits. The cause of cracks should be investigated by appropriate methods and then repaired.
7 2 nd Priority 2 Concern Incomplete structural drawings, some information is missing; as listed below. Floor plans, column schedule and etc. are missing from the structural drawings. RC beam details and RC slab details are incomplete.
8 3 rd Priority 2 Concern There are no loading plans for the structure in any area.
9 4 th Priority 2 Concern Concerns on the roof slab supporting water tanks. A capacity check of these slabs and all related elements need to be carried out.
10 5 th Priority 2 Concern Concerns with the lightweight roof at the top level. Stability and capacity must be checked especially the connections between steel structural elements and brick wall. And stability under wind uplift needs to be checked in particular.
11 6 th Priority 2 Concern Concerns on the communication pole at top level. Anchorage of this item needs to be checked.
12 Identified Priority 3 Concerns - Reinforcement left exposed at top level. - No fireproofing material for structural steel elements (if required).
13 1 st Priority 3 Concern Reinforcement is left exposed at top level.
14 2 nd Priority 3 Concern Concerns on stability of bamboo leaning on the wall at top level. Whilst not a structural issue, this is a significant health and safety risk and should be placed properly or removed.
15 3 rd Priority 3 Concern No fireproofing material was noted on structural steel elements (if required).
16 Overall Stability System
17 We require that the stability of this building is investigated as part of a Detailed Engineering Assessment. The lateral loading resistance for this 6-storey building is provided by moment frame action without any shear walls and the façade brickwork walls lack moment capacity due to their poor connections with the surrounding columns and beams.

19 Water distress was noted on slabs, beams, columns and walls. Water ponding was also found on the top level.

Maintain and control the floor loading on 1st floor, 2nd floor and 3rd floor to ensure the total load does not exceed 2.0 kpa. Factory Engineer to review design, loads and columns stresses in area identified above. Verify in-situ concrete stresses either by cores or existing cylinder strength data for [the identified columns / cores from 4 columns]. Verify in-situ reinforcing steel stresses by sampling and tension test for [the identified columns]. A Detailed Engineering Assessment of Factory to be commenced immediately. Immediate - Now 2 Priority 1 - The internal columns at ground level are overstressed. Produce and actively manage a loading plan for all floor plates within the factory giving consideration to floor capacity and column capacity. The Detailed Engineering Assessment to be completed and any recommended actions completed. 6-weeks 3 Priority 1 - The internal columns at ground level are overstressed. Maintain and continue to implement floor load plans. 6-months 4 Priority 1 - Cracks on 4th floor and 5th floor columns. Investigate the cause of cracks as part of a Detailed Engineering Assessment. Immediate - Now 5 Priority 1 - Cracks on 4th floor and 5th floor columns. The distress found is to be suitably rectified and then repaired. 6-weeks 6 Priority 1 - Cracks on 4th floor and 5th floor columns. Factory Engineer to monitor to ensure that repairing is correctly done so that problems do not arise in the future. 6-months

Factory Engineer to state the maximum weight that is allowed per square meter. State the values clearly on notice boards in these areas. (Loading Plans). 6-weeks 11 Priority 2 - No loading plans exist for the structure. Maintain standards of quality control to ensure that storage procedures are correctly followed. 6-months 12 Priority 2 - Capacity check of roof slab supporting water tanks. Factory Engineer to review design, loads and related elements in area identified above to confirm capacity for these items. 6-weeks 13 Priority 2 - Stability and capacity of lightweight roof. Factory Engineer to review design, loads and related elements in area identified above to confirm capacity and stability for these items. 6-weeks
