6 1. PURPOSE AND REQUIREMENTS a. Purpose. Ths revew plan revses the revew actvtes defned n the Revew Plan for Herbert Hoover Dke Desgn and Constructon Phases, Martn and Palm Beach Countes, Florda, 27 September 2010, as amended 30 November Ths revson updates the constructon contracts that are envsoned for near future so that requred revew actvtes can be scheduled and completed. Revew actvtes consst of Dstrct Qualty Control (DQC), Agency Techncal Revew (ATR), and Type II Independent External Peer Revew (IEPR). Portons of the project are n the Pre-Constructon, Engneerng and Desgn (PED) Phase and the Constructon Phase. The related documents are Implementaton Documents that consst of Plans and Specfcatons (P&S) and Desgn Documentaton Reports (DDR). The P&S and DDRs addressed by ths revew plan are for the replacement and/or removal of the water control structures (culverts). Upon approval, ths revew plan wll be ncluded nto the Project Management Plan as an appendx to the Qualty Management Plan. Two other HHD Revew Plans are scheduled for FY They are: the HHD Dam Safety Modfcaton Report Revew Plan and the Alternatve Rehabltaton Plan Plot Test Letter Report wth Envronmental Assessment (Letter Report) and the Seepage Management Test Faclty P&S (Plot Test) Revew Plan. b. Ths revew plan was developed n accordance wth EC , whch establshes an accountable, comprehensve, lfe-cycle revew strategy for Cvl Works products by provdng a seamless process for revew of all Cvl Works projects from ntal plannng through desgn, constructon, and Operaton, Mantenance, Repar, Replacement and Rehabltaton (OMRR&R). The EC provdes the procedures for ensurng the qualty and credblty of U.S. Army Corps of Engneers (USACE) decson, mplementaton, and operatons and mantenance documents and other work products. The EC outlnes three levels of revew: Dstrct Qualty Control, Agency Techncal Revew, and Independent External Peer Revew. Refer to the EC for the defntons and procedures for the three levels of revew. d. Revew Management Organzaton (RMO). In accordance wth ER , the Insttute of Water Resources Rsk Management Center (RMC) s desgnated as the RMO. 2. PROJECT INFORMATION a. Background. Herbert Hoover Dke s an earthen embankment system located along the permeter of Lake Okeechobee, a large (724 square mle surface area) freshwater lake n south Florda. The lake s located about 30 mles west of the Atlantc Ocean and 60 mles east of the Gulf of Mexco. The lake and surroundng dranage area encompass approxmately 5,600 square mles. The dke was constructed prmarly to provde local flood protecton. Components of the embankment system have been bult ntermttently snce the early 1900 s. Federal nvolvement began n the 1930 s wth the constructon of dkes (for flood protecton) along portons of the north and south shores. 2
7 In the 1960 s, the crest elevatons of those dkes were ncreased and addtonal embankments were constructed on the northwest and northeast shores. As a result, the Herbert Hoover Dke system now encrcles Lake Okeechobee entrely, except n the vcnty of Fsheatng Creek on the western shore. The exstng embankments total about 143 mles n length wth crest elevatons rangng from 32 to 46 feet, Natonal Geodetc Vertcal Datum (NGVD). Adjacent land elevatons typcally range from 10 to 20 feet, NGVD. Lakesde levee slopes vary from 1V:3H to 1V:10H and landsde slopes range from 1:2 to 1:5. The Herbert Hoover Dke Major Rehabltaton Report (MRR) from 2000 dvded the 143 mle dke nto eght (8) Reaches wth the ntal focus on Reach 1. Ths Reach by Reach approach has been replaced wth a system wde rsk reducton approach as utlzed for USACE safety modfcatons to dams. The mplementaton of the 21.4 mle cutoff wall component n Reach 1 satsfes the majorty of the rsk reducton goals. The constructon of ths cutoff wall wll be complete n As part of ths rsk reducton approach, the 32 water control structures (culverts) operated by the Corps are beng replaced, removed or abandoned based on USACE approval n May One structure has been removed and four structures are currently under contract for replacement. AGENCY TECHNICAL REVIEW a. Scope. Agency Techncal Revew (ATR) s undertaken to "ensure the qualty and credblty of the government's scentfc nformaton" n accordance wth EC and ER An ATR wll be performed on the contract P&S submttals as noted n the below mlestone schedule. The RMC as RMO s responsble assemblng the ATR Team and completng ATR n accordance wth ths revew plan and USACE gudance. ATR comments wll be documented n the DrChecks sm model revew documentaton database. DrChecks sm s a module n the ProjNet sm sute of tools developed and operated at ERDC-CERL ( At the concluson of each ATR effort, the ATR team wll prepare a Revew Report summarzng the revew. Revew Reports wll be consdered an ntegral part of the ATR documentaton and shall: Identfy the document(s) revewed and the purpose of the revew; Dsclose the names of the revewers, ther organzaton afflatons, and nclude a short paragraph on both the credentals and relevant expertse of each revewer; Include the charge to the revewer; Descrbe the nature of ther revew and ther fndngs and conclusons; Identfy and summarze each unresolved ssues (f any); and Include a verbatm copy of each revewers comments (ether wth or wthout specfc attrbutons), or represent the vews of the group as a whole, ncludng any dsparate and dssentng vews. b. ATR Dscplnes. ATR wll be conducted by ndvduals and organzatons that are external to the Jacksonvlle Dstrct. The ATR Team Leader wll be a USACE employee outsde the South Atlantc Dvson. As stpulated ER , ATR members were sought from the followng sources: regonal techncal specalsts (RTS); apponted subject matter experts (SME) from other dstrcts; senor level experts from other dstrcts; Center of Expertse staff; apponted SME or senor level experts from the responsble dstrct; experts from other USACE commands; contractors; academc or other techncal experts; or a combnaton of the above. The ATR Team s comprsed of the followng dscplnes; knowledge, sklls and abltes; and experence levels. Hydrology and Hydraulcs. One or more team members may be requred to revew the hydraulc desgn, hydraulc modelng, hydrologc modelng, and wnd/wave analyses. The team member(s) should be regstered professonals wth 10 or more years experence n conductng and evaluatng hydrologc and hydraulc analyses for flood rsk management projects. Experence wth 2D hydraulc modelng, 3D hydrologc and groundwater modelng, wnd/wave analyss, and performance of rsk assessments s requred. 4
9 Geotechncal Engneerng. The team member should be a regstered professonal engneer and have 10 or more years experence n geotechncal engneerng. Experence needs to nclude geotechncal evaluaton of flood rsk management structures. Experence needs to encompass statc and dynamc slope stablty evaluaton; evaluaton of the seepage through earthen embankments and under seepage through the foundaton of the flood rsk management structures, ncludng dams, levee embankments, floodwalls, cut off walls, closure structures and other pertnent features; and settlement evaluatons. Geologst. The team member should be a regstered professonal and have 10 or more years experence n engneerng geology. Experence needs to encompass flood rsk management structures, ncludng dams, cut off walls, levee embankments, floodwalls, closure structures and other pertnent features; and related geologcal evaluatons. Structural Engneerng. The team member should be a regstered professonal engneer and have 10 or more years experence n structural engneerng. Experence needs to nclude the engneerng and desgn of flood rsk management project features such as water control structures, conveyance culverts, and spllways. Mechancal and Electrcal Engneerng. The team members should have 10 or more years experence n mechancal and electrcal engneerng. Experence needs to nclude engneerng and desgn of flood rsk management project features such as water control structures, related systems and components. Cvl Engneerng. The team member should be a regstered professonal engneer and have 7 or more years experence wth cvl/ste work projects to nclude embankments, roads and hghways, real estate mplcatons, relocatons, pavng and dranage. Constructon Management/Qualty Control. The team member should have 10 or more years demonstrated n the experence wth dam and/or levee safety projects. Project experence should encompass cutoff walls, cofferdams and seepage management features. ATR Team Leader. The ATR Team Leader should have 7 or more years experence wth Cvl Works Projects and have performed ATR Team Leader dutes on complex cvl works projects. The ATR Team Leader can also serve as one of the revew dscplnes. 5. INDEPENDENT EXTERNAL PEER REVIEW a. General. EC provdes mplementaton gudance for both Sectons 2034 and 2035 of the Water Resources Development Act (WRDA) of 2007 (Publc Law (P.L.) ). Sectons 2034 and 2035 call for peer revew procedures for both the Plannng and the Desgn and Constructon (PED) phases. The EC terms the Secton 2034 Independent Peer Revew, Type I Independent External Peer Revew and the Secton 2035 Safety Assurance Revew, Type II Independent External Peer Revew. b. Type I Independent External Peer Revew (IEPR) Determnaton (Secton 2034). Type I IEPR s generally for decson documents. No decson documents or other applcable Secton 2034 products are addressed by ths Revew Plan. Therefore Type I IEPR s not applcable to the mplementaton documents addressed by ths Revew Plan. c. Type II Independent External Peer Revew (IEPR) Determnaton and Methodology. A desgn phase Type II IEPR has been completed for water control structures (Culvert Contracts 1 and 2). Per above reference b. (3), no addtonal Type II IEPR s requred for the desgn phase for the culvert contracts. A Constructon Phase Type II IEPR s requred for the HHD System and s slated for completon fscal year A Constructon Phase Type II IEPR s also requred for the Seepage Management Test Faclty P&S (Plot Test) n fscal year The EC charges for the constructon phase are: 5

Type II Independent External Peer Revew (IEPR) Insttute for Water Resources Rsk Management Center (RMC) Procedures. The EC requres Type II IEPR be managed and conducted outsde the Corps of Engneers under the purvew of the RMC. The RMC wll provde a vetted Archtect-Engneer frm to manage and execute Type II IEPR outsde USACE n accordance wth Natonal Academy of Scences procedures. To nsure ndependence and to obtan the requred expertse, the Type II IEPR Independent Experts wll be acqured by the Archtect-Engneer frm. Independent Experts wll submt and comply wth Natonal Academy of Scences, Background Informaton and Confdental Conflct Of Interest Dsclosure, BI/COI FORM 3, current edton. The RMC s a techncal center wthn the USACE Insttute for Water Resources. Addtonal nformaton about the RMC can be vewed at the followng web ste. e. Type II IEPR Experts and Members. Followng are the professonal descrptons for the Constructon Phase Type II IEPR ndependent experts. Type II IEPR Geotechncal Engneerng Independent Expert. The Geotechncal Engneerng Independent Expert should be a regstered professonal engneer from academa, a publc agency, or an Archtect- Engneer or consultng frm wth a mnmum of 15 years experence. Experence should be n the analyss, desgn, and constructon of embankment dams and levees ncludng subsurface nvestgatons; sol mechancs; seepage and ppng; flter and dranage elements; cutoff wall desgn and constructon; cofferdam desgn and constructon; long-term consoldaton and settlement; deformaton problems assocated wth embankments constructed on karstc and soluton prone rock formatons; and sheet ple cut-off walls and temporary cofferdams. Experence wth the Dam Safety program s desred. Actve partcpaton n related professonal socetes s encouraged. Type II IEPR Engneerng Geologst Independent Expert The Engneerng Geologst Independent Expert should be a regstered professonal from academa, a publc agency, or an Archtect-Engneer or consultng frm wth a mnmum of 15 years experence. The geologst should be profcent n assessng seepage and ppng through and beneath dams constructed on or wthn karstc and soluton prone rock formatons. In addton, the expert should be famlar wth dentfcaton of geologcal hazards; subsurface exploraton technques; feld & laboratory testng; nstrumentaton and performance montorng; and should be experenced n the desgn of grout curtans & cutoff walls and must be knowledgeable n grout rheology, concrete mx desgns, and other materals used n foundaton seepage barrers. Experence wth the Dam Safety program s desred. Actve partcpaton n related professonal socetes s encouraged. 6
11 Type II Structural Engneerng Independent Expert. The Structural Engneerng Independent Expert should be a regstered professonal engneer from academa, a publc agency, or an Archtect-Engneer or consultng frm wth 15 years experence n conductng and evaluatng structural analyses for water control structures, coffer dams, dams and mpoundments. Experence wth the Dam Safety program s desred. Actve partcpaton n related professonal socetes s encouraged. Type II IEPR Hydrology and Hydraulcs (H&H) Independent Expert. The H&H Independent Expert should be a regstered professonal from academa, a publc agency, or an Archtect-Engneer or consultng frm wth 15 or more years experence n conductng and evaluatng hydrologc and hydraulc analyses for flood rsk management projects. Experence wth water control structures, dam hydrodynamcs, Water Control Manuals, coffer dam desgn and dam operatons are requred. Experence wth the Dam Safety Program s desred. Actve partcpaton n related professonal socetes s encouraged. f. Type II IEPR Report Approval. The approval authorty for Type II IEPR Reports s the South Atlantc Dvson (SAD). Approval actvtes and responsbltes are stpulated n EC Related excerpt s below. 10. Dstrct Responsbltes to complete the SR Report. a. The host dstrct Chef of Engneerng s responsble for coordnatng wth the RMO, for attendng revew meetngs wth the SAR revew panel, communcatng wth the agency or contractor selectng the panel members, and for coordnatng the approval of the fnal report wth the MSC. b. After recevng a report on a project from the peer revew panel, the Dstrct Chef of Engneerng shall consder all comments contaned n the report and prepare a wrtten response for all comments and note agreement and subsequent acton or dsagreement wth an explanaton. The revewer s report and the Dstrcts responses shall be submtted to the MSC for fnal approval and made avalable to the publc on the Dstrct s webste. 6. PRODUCTS, SCHEDULES AND BUDGETS a. Culvert Contract Descrptons, Project Delvery Team (PDT) Dscplnes and ATR Dscplnes. The culvert contracts address the removal and replacement of culverts. Culvert 3 (S-277): Culvert 3 s a double barrel, 10 foot dameter structure. The structure wll be removed and replaced wth a trple, 7 x 7 foot square barrel structure. Culvert 4A (S-276): Culvert 4A s a double barrel, 10 foot dameter structure. The structure wll be removed and replaced wth a trple, 7 x 7 foot square barrel structure. Culvert 7, Culvert 9 and Taylor Creek Culvert (TCC): Culverts 7, 9 and TCC were abandon n the 1980s do to ther poor condton. The proposed mprovements nclude a seepage cutoff feature to ntercept any seepage paths and/or vods n or around the condut. In addton a downstream embankment toe dran wll be desgned to allow safe release of any seepage. Culvert 5 (S-282): Culvert 5 s a trple barrel, 10 foot dameter structure. 17 Herbert Hoover Dke Rehabltaton Project Progress Update 22 February 2012 Background: The Herbert Hoover Dke (HHD) Major Rehabltaton Report (MRR) from 2000 dvded the 143-mle dke nto eght (8) Reaches wth the ntal focus on Reach 1. Ths Reach by Reach approach has been replaced wth a system wde rsk reducton approach as utlzed for USACE safety modfcatons to dams. The mplementaton of the cutoff wall component n Reach 1 and the water control structure (culvert) replacements and removals around Lake Okeechobee satsfy the majorty of the rsk reducton goals for these areas. The goal of the project s to satsfactorly reduce rsks thereby lowerng the current DSAC I ratng. Reach 1 Cutoff Wall Constructon: The fnal Multple Award Task Order Contract (MATOC) task orders were awarded for the 21.4 mles of cutoff wall nstallaton between Port Mayaca and Belle Glade. Currently 18 mles (84%) of nstalled wall have been accepted wth 100% completon by fall of Cutoff wall task order #4(D): 17,600-ft of cutoff wall complete (17,600-ft total) and accepted. Fnsh work wll contnue through February Cutoff wall task order #5(E): 14,530-ft of cutoff wall complete (20,050-ft total) and accepted, 15,612-ft of cutoff wall nstalled wth a scheduled completon date of December 7, 2013 Cutoff wall task order #6(F): 8,650-ft of cutoff wall complete (14,070-ft total) and accepted, 10,160-ft of cutoff wall nstalled wth a scheduled completon date s February 14, 2013 Cutoff wall task order #7(G): 16,795-ft of cutoff wall complete (18,210-ft total) and accepted, 18,210-ft of cutoff wall nstalled wth a scheduled completon date s May 25, 2013 Cutoff wall task order #8(H): (3,340-ft total) contractor antcpates moblzng by the end of February. Culvert 14 removal was completed n February Culverts 1, 1A, 11 and 16 replacements are currently under constructon. Culverts 3, 4A, 5, 10, 12 replacements are scheduled for awards n FY 2012 and FY Culverts 7, 9 and TCC removals (abandonments) are scheduled for awards n FY Dam Safety Modfcaton Report (DSMR): A dam safety modfcaton report s beng developed whch wll address the entre dke as a system and nclude a rsk reducton approach to mplementng features based on prorty and reducng rsk as quckly as possble. Potental falure mode analyss, rsk assessment and rsk reducton measures are ongong and wll be requred for each Reach segment around Lake Okeechobee. The draft DSMR s scheduled for completon n Aprl 2013 wth fnal approval by March Constructon on future modfcatons could then start n FY 2016.
18 LAKE OKEECHOBEE Herbert Hoover Dke FACTS & INFORMATION SPRING 2012 Lake Okeechobee, the naton s second largest freshwater lake and the largest lake n Florda s the heart of the Kssmmee- Okeechobee-Everglades system. The orgnal lake was lkely flled wth salt water, whch was eventually replaced by freshwater from ranfall. Water flowed from the Kssmmee basn nto Lake Okeechobee. Today, the lake provdes drnkng water for communtes around the lake and to the more than 6 mllon people lvng along the lower east coast. It serves as a source of rrgaton for a $1.5 bllon-a-year agrcultural ndustry that produces sugar cane, wnter vegetables, ctrus and rce. HISTORICAL BACKGROUND In the early 1900s, local governments and resdents bult up the lake s natural embankments wth sand, shell, muck and marl. By 1927, sx large dranage canals and numerous smaller canals totalng 440 mles had been constructed. Despte these actons, hurrcane wnds n 1926 and 1928 caused the lake s waters to quckly overwhelm ts shallow edge. Ths flow resulted n massve floodng and the loss of many lves. To help prevent a repeat of ths type of dsaster, the U.S. Army Corps of Engneers (Corps) was asked by Congress to buld a taller dke. It would protect the areas from Port Mayaca to Moore Haven and from the Kssmmee Rver to Nubbn Slough. In 1947 and 1948, two more hurrcanes passed drectly over Lake Okeechobee, causng massve floodng. Although the levees had done ther job n that no lves were lost, the need for better flood protecton was evdent. To ncrease protecton of peoples lves, ther property and the prosperous agrcultural ndustry located south of the lake, the Corps enhanced the exstng levees. Completely encrclng the lake and rasng the exstng levees, one of the levee s prmary purposes was to provde protecton durng severe hurrcanes. In 1960, Congress named the levee Herbert Hoover Dke (HHD). THE DIKE The Corps bult the dke wth gravel, rock, lmestone, sand and shell usng state-of-the-art engneerng n the 1930s. These natural materals allow water to trckle through at tmes. Ths s a normal process called seepage. When the water level n the lake s too hgh, however, the water pressure causes extra seepage that can lead to eroson, or ppng. When the water level s too low, the sol dres out and that changes the makeup wthn the structure and foundaton. Nether extreme s good, so engneers closely montor the lake levels and the dke. The Corps wll remove or replace all federal culverts n the dke system.
19 LAKE OKEECHOBEE Herbert Hoover Dke MAKING IT BETTER AND STRONGER Managng and Montorng Lake Okeechobee and Herbert Hoover Dke Together, the Corps and the South Florda Water Management Dstrct manage the lake accordng to a regulaton schedule. The schedule allows the Corps to manage the lake at a safe level at the begnnng of the wet, hurrcane season to allow for more capacty or a hgher lake level by the end of the wet season. The safe lake level helps provde storage capacty for wet season ranfall, whch n turn provdes water supples to the envronment and the people. The schedule has been developed over tme to meet the many demands for the lake s water. It also provdes water managers wth the authorty to determne when and n what amounts water s to be released from the lake. For the optmum health of the lake and use of ts water, water managers strve to mantan the levels between 12.5 and 15.5 feet, not too hgh and not too low. Past hurrcane seasons, however, created hgher than desrable water levels, rasng concerns about the stablty of the HHD n extreme weather condtons. Water levels and the condton of the dke are montored on a regular bass. Pror to a tropcal storm, the dke s nspected and outlets are closed untl after the storm. Once weather condtons permt, typcally wthn 24 to 48 hours after a storm, Corps personnel nspect the dke agan. Stored n dfferent areas around the lake are equpment, sandbags, stone, rock and other materals to expedte makng repars or strengthenng areas that may become eroded by the force of hurrcane-drven waves. Rgorous analyss has shown that a major rehabltaton of HHD s requred to prevent harmful seepage, whch could undermne the dke structure. The frst part of the rehabltaton currently underway, focuses on a secton of the dke that spans 22.4 mles from Port Mayaca to Belle Glade. The U.S. Army Corps of Engneers and the South Florda Water Management Dstrct are commtted to mantanng Florda s fragle balance between the needs of people and the conservaton of the state s unque wldlfe and water envronments. Herbert Hoover Dke plays an mportant role n helpng us to keep that commtment. FOR MORE INFORMATION EMERGENCY ACTION If a weakened dke condton s observed, the Corps wll actvate ts Emergency Acton Plan. The plan ncludes a formal notfcaton process that moves n rapd order from the federal government, to the state, to the county Emergency Operatons centers. 20 HERBERT HOOVER DIKE REHABILIATION PROJECT UPDATE SPRING 2012 Herbert Hoover Dke (HHD) s lsted as one of the naton s dams and ts rehabltaton s a top prorty of the U.S. Army Corps of Engneers. In the last fve years, the HHD Rehabltaton Project receved $56 mllon n 2008, $74 mllon n 2009, $124 mllon n 2010, and $107.8 mllon n 2011 ths represents a sgnfcant porton of Dam Safety fundng naton-wde. There are currently 10 dams n the naton recevng addtonal funds for constructon projects to reduce the rsk of falure; HHD s one of them. Jacksonvlle Dstrct s workng to rehabltate the 143-mle dke system. Actons taken nclude nstallng a cutoff wall, removng and replacng water control structures (culverts), testng landsde seepage management features and conductng a varety of studes and techncal revews to help ensure the safety of south Florda resdents. Corps teams work daly on the dke, provdng contractor oversght, qualty assurance, nspectons and dke operatons and mantenance. Much progress s also beng made behnd the scenes at the Dstrct, where a team of engneers, hydrologsts, geologsts, scentsts, contract and real estate specalsts, budget analysts and many others, work to ensure the very best rehabltaton strateges are appled to the dke today and n the future. Here s a progress update on the most sgnfcant components of the rehabltaton. OVERALL SYSTEMS APPROACH The Corps s approachng ths project utlzng a dam safety process that prortzes what can be done to lower the rsk across the entre HHD system. Work s underway to produce a Dam Safety Modfcaton (DSM) Report for HHD, whch wll address the entre dke as a system. It wll nclude a rsk reducton approach to mplementng features based on prorty and reducng rsk as quckly as possble. All features planned and under constructon support the goal of ths report. CUTOFF WALL CONSTRUCTION Constructon of the cutoff wall s provdng the soluton by elmnatng exstng ppng and preventng addtonal nternal eroson through the dke and foundaton. The Corps awarded multple task order contracts for cutoff wall constructon n the dke between Port Mayaca and Belle Glade to three contractors.to date, over 21 mles of cutoff wall n the southeast secton are under constructon or have been completed, totalng over $225 mllon n cutoff wall contracts.
21 HERBERT HOOVER DIKE REHABILIATION HHD CULVERT REMOVALS OR REPLACEMENTS Bult n the 1930s, most of these old culverts along the HHD are stll n use today. From a structural ntegrty perspectve, culverts pose a rsk of falure due to the loss of embankment materal nto and along the culverts. As part of the federal culvert replacement program, the Corps wll replace or remove 32 culverts wthn the HHD system. Work began n late 2011, wth the removal of Culvert 14 north of Canal Pont. Addtonal work began n 2012 to replace Culverts 11 and 16 south of Port Mayaca, and Culverts 1 and 1A east of Moore Haven. Contracts for the replacement of Culverts 3 and 4A near South Bay are expected to be awarded n the fall. The Corps antcpates removng or replacng all the culverts wth constructon contnung through A temporary dam s constructed n advance of the removal of Culvert #14 near Port Mayaca. Ths plot test wll be n two locatons on the southern embankment and demonstrate whether ths alternatve approach result n a more economcal rehabltaton plan wth less mpact on adjacent lands whle stll addressng the embankment and foundaton seepage and ppng concerns.
