The plan quantities for the various concrete barrier items have not deducted linear feet to be paid with the end anchors (15 ft/ea), light foundations (8 ft/ea), ITS junction boxes (4 ft/ea), lighting junction boxes (4 ft/ea) and median overhead sign foundations (10 ft/ea). Please revise the quantities for the various concrete barrier items accordingly. A1: Barrier quantities will be updated accordingly. Q2: Submitted January 30, 1:36 PM. 1. Will the removals of the temporary drainage items be paid for with the rest of the removals or is the cost to be included in the temporary drainage items? 2. Bid item 101 is for 6" reinforced concrete pipe. That size of concrete pipe is not made. Please revise the material spec. A2: 1. The removal of temporary drainage items will be included in the bid item for the temporary drainage items. The note on Sheet 63 will be clarified. 2. The material specification will be revised.

5 Q3: Submitted January 30, 2:38 PM. We see end anchor requirements change from project to project. On this project, how many end anchors are required per barrier median inlet? How many end anchors are required per barrier light pole? Are there any other locations at which end anchors are to be installed? A3: Reinforced end anchorages will be required at the location specified in Standard Drawings RM-4.3 through RM-4.5. Reinforced end anchorages will be required at median barrier inlets where there are expansion joints. Q4: Submitted February 2, 2:00 PM. The subsummary on page 1,331 of the landscaping plans lists 1,060 sy of item nonreinforced concrete pavement, app & 118 cy of item 304 aggregate base. There are no bid items set up for either of these items. Please provide bid items. A4: Item 452 and Item 304 are not required. These items will be removed from sheet The Item concrete walks are detailed on sheet 1326, and the quantity is specified on sheet 1328 (and on the Landscape Sub-summary on sheet 1321). Q5: Submitted February 2, 2:05 PM. Under the current standards the curb ramps are paid by the square foot and include the cost of the walk through the ramp areas. It appears that this project is setup per the old standards where the ramps were paid by the each and the walk was paid through the curb ramps. Please clarify how the Curb ramps and the walk will be paid for. A5: The curb ramp pay item will be updated to be paid for in SF. Q6: Submitted February 2, 3:03 PM. Reference the prebid question answer to the prebid question asked on 1/28/15 at 2:44:01 PM. Since the contract for the structural steel for these bridges is between the fabricator and ODOT it would probably be in ODOT'S best interest to provide the storage fees ( $/unit of time) for the structural steel for bridges 271L & 257 L&R to the contractors so that every bidder can incorporate the appropriate costs if they deem necessary. A6: Plan sheets will be updated to include storage costs. Q7: Submitted February 2, 3:12 PM. Will a Concrete Barrier End Section, Type B1, be required on US 33 at the west end of the project (50+80)? If so, please add a biditem for this. A7: A Type B1 concrete barrier end section will be required. The plans will be updated accordingly. Q8: Submitted February 2, 3:21 PM. Sheet 87 describes that impact attenuator installation, repair, and removal associated with construction access points should be paid for under lump sum Item 614 Maintaining Traffic. However, MOT sub-summary sheets show all impact attenuators associated with construction access points are to be paid for under the typical bid item 454 Work Zone Impact Attenuator, 60 EA. Please clarify where impact attenuators associated with construction access points are to be paid for.

6 A8: The work zone impact attenuators shown at construction access points will be removed from the quantities. The installation, removals and relocations of work zone impact attenuators required to complete construction and change access points are included in Item 614 Maintaining Traffic per the plan insert sheet. Q9: Submitted February 2, 3:23 PM. The quantities for bid Items 605 & 623 Mechanically Stabilized Earth Walls are understated. It appears the 3 embedment was not accounted for. A9: The quantities for Mechanically Stabilized Earth Walls (Bid Items 605 & 623) have been revised. Q10: Submitted February 2, 3:25 PM. Will Glass Fiber Reinforced Polymer (GFRP) stiffening reinforcement be required for RW5 moment slab parapet joints? A10: No, GFRP will not be required for the moment slab parapet joints. Q11: Submitted February 2, 3:27 PM. Bid Item 598 Epoxy Coated Reinforcing Steel qty of 40,849 LB does not match the reinforcing list on sheet 1087/1763. Please revise. A11: Quantity has been revised. Q12: Submitted February 2, 3:58 PM. It appears that the quantity for bid item 582 is substantially understated. A12: The quantity for Item 518 Structure Drainage, Misc.: Miradrain 5000 drainage system or equivalent has been revised. Q13: Submitted February 2, 4:14 PM. Sheet 6 of the Specifications and Special Provisions for Aesthetic Treatments references approved shop drawings for the form liners. There is no other mention of shop drawings for the form liners in this specification or the plans. Are shop drawings required to be submitted for the form liners on this project? If so, please provide all information that is to be included in these drawings. Additionally, please provide the applicable Department review times for approval of these drawings. A13: Shop drawings shall be provided for verification for all Project form liner types. The information shall include, but not be limited to, fully dimensioned drawings, sections, manufacturing conventions, special fabrication instructions, contract document references, manufacturing material specifications, catalog cut sheets, and information pertaining to affixing the form liner to the form. Review times for these shop drawings will be 10 working days. Q14: Submitted February 2, 4:14 PM. In speaking with potential form liner suppliers, it is evident that the City has had several previous discussions with only one supplier. This supplier has had the opportunity to get a head-start on this project. Due to the intricacy of the form liners and the quantity required to build the job, the remaining suppliers are requesting more time to bid this project. If additional time is not granted, these suppliers are suggesting that their prices will be significantly inflated and/or that they might not be able to bid the project. As a means of leveling the playing field for all bidders, and thus ensuring the most economical form liner price to the Department, please consider delaying the sale date of this project. A14: The sale date remains unchanged.

7 Q15: Submitted February 2, 4:16 PM. Sheet 2 of the Special Provisions for Permanent Soil Nails states that the Department will pay for each abandoned soil nail at 50 percent of the production soil nail unit price. Plan sheet 1003 states that the Department will pay for each abandoned soil nail at 25 percent of the soil nail unit price. Please clarify which note is correct in the next addendum. A15: The Department will pay for each abandoned soil nail at 25 percent of the soil nail unit price. Q16: Submitted February 2, 4:18 PM. In an answer to a previous question regarding form liners needed for the project ODOT stated that, The City of Dublin contacted a possible fabricator and verified that the time frames are feasible so long as the contractor is timely with permission to proceed with fabrication. The FRA L/R bridges will be constructed in 2015 per the sequence of construction. We have reached out to several suppliers and none of them have been able to meet our schedule needs so that the FRA L/R bridges can be built in 2015 per the sequence of construction. Will the Department please provide the contact information for the fabricator the City spoke with? A16: The City of Dublin is aware of the following potential fabricators. This is not an all-inclusive list, nor is it intended to imply prequalification: Creative Formliners: (Maryland) Scott System: (Colorado) Spec Formliners: (California) Q17: Submitted February 2, 4:26 PM. The Special Provisions for Aesthetic Treatments states that the work shall be in accordance with American Concrete Institute ACI 303R-91, Guide to Cast-In-Place Architectural Concrete and other appropriate ACI guidelines. This publication was updated in 2012 and is now ACI 303R- 12. Sheet 1 of this ACI standard states that reference to the standard shall not be made in contract documents. Please clarify this discrepancy. A17: This reference has been removed from the Special Provisions for Aesthetic Treatments. Q18: Submitted February 2, 4:27 PM. In an answer to a previous question, ODOT stated that, One complete set of form liners for each wall type shall be furnished to the City at project close-out. To remove all ambiguity and to ensure that bidders are interpreting this response the same, please identify the number of each type of panels required to be furnished to the City for each wall type. A18: The Contractor is responsible for turning over 55 unique form liners at the end of the project. They are as follows: CIP Walls and Abutments: 31 unique form liners MSE Walls: 17 unique form liners Parapets and Piers: 7 unique form liners More detail about the form liners can be found on Exhibit A of the Special Provisions for Aesthetic Treatments.

8 Q19: Submitted February 2, 4:31 PM. Per the sequence of construction requirements, the FRA L/R bridges shall be constructed during the 133 day maximum window duration for Phases 2A and 2B. To meet the 133 day window, the abutments of these bridges will need to be built simultaneously. This will not allow for the re-use of form liner panels between abutments. As such, the Contractor will be required to purchase 100% of the liner for each of these abutments. At the very least, this equates to purchasing 260 form liner panels. The purchase of 260 form liner panels will result in a significant cost to the project. Please consider increasing the maximum duration for the Phase 2A and 2B window to allow for the re-use of form liner panels between the abutments of the FRA L/R bridges. A19: See updated Special Provisions for form liners. The contractor will have to purchase as many as they require to meet the schedule. The time frames will not be modified. Q20: Submitted February 2, 4:32 PM. The Special Provisions for Aesthetic Treatments states, Original bas-relief patterns of the proposed river scene for the highway walls, Celtic design and Martel Font text for the parapet fascia, piers, and bullnose shall be designed and fabricated by the form liner supplier, from clay, to assist in making the molds for the urethane rubber form liners Under no circumstance can these patterns be created on the computer and enlarged by a CNC machine. We have spoken to several potential suppliers who cannot bid this project due to these requirements. Additionally, and to this point, we have only received confirmation from a single supplier who can comply with these requirements. While this single supplier can comply with the requirements of the special provisions, they cannot meet the schedule required to ensure construction of the FRA L/R bridges per the sequence of construction. Please revise the Special Provisions for Aesthetic Treatments to remove the requirements that original patterns be designed and fabricated from clay. A20: The schedule and specification requiring clay molds will not be adjusted. Q21: Submitted February 2, 5:06 PM. The MOT note requires a switch in to 2A no sooner than 6/8/15, and the steel delivery for 271L is scheduled on 6/15/15 per note on the bridge plans. There can be no preconstruction of the substructure of 271, as it is under where traffic is maintained. There will be considerable extra cost due to the error in delivery date in the plans. We need to know what storage costs you negotiated so we can analyze if it will be cheaper to mobilize a crane in to double handle the beams and set up contracts with delivery trucks to move from a temporary storage facility to the bridge placement and all associated costs, or if it will be cheaper to pay the storage costs as indicated on the bridge plans. The same will apply to the 257L and R structures as the same error exists in those dates as well. There is also a high probability the steel for 1763 will need additional storage fees analyzed as well. Please provide those costs for analysis as well. If ODOT would reveal the storage fees, and make it available for all, it will even the playing field and ensure the lowest taxpayer cost. A21: Plan sheets will be updated to include storage costs.

9 Q22: Submitted February 2, 5:35 PM. This question is regarding the field office note on plan page 59. First, the City of Dublin will not allow construction office trailers in their city unless it is located ON the construction project. The level of landscaping and the difficulty getting utilities eliminate the possible use of the project ramp infields as field office location. The 25 x 25 conference room requirement practically eliminates the option of trailers. The City of Dublin also does not allow the installation of fence without a permit which could take up to a year to get. It is also not likely that commercial real estate owners will allow such installation of chain link fencing if the contractor were to provide leased commercial office space. Securing 25 parking spaces also eliminates the ability to limit the rented square footage because generally the parking space ratio is generally 3.5 to 1000sf. This makes the 25 parking spaces and the chain link fence difficult to provide in the city of Dublin. When we look at the next interchange west of the project (Post Road) the availability of commercial real estate is extremely limited. Weighing in all these factors makes the current note unpractical. Has the department coordinated these requirements with the City of Dublin? Will the department consider changing the note to eliminate either the conference room requirement or the fencing requirement? If not, how far from the project will the department accept for a field office location? A22: The City is open to a field office site (a) within the project limits, (b) on City-owned property, or (c) on private property adjacent to the project limits. The 6-foot temporary chain link fence will be allowed. A temporary sign, 32 square feet maximaum size, can be coordinated through Dublin s Code Enforcement. The note on sheet 59 will be replaced by the following: TEM 619 FIELD OFFICE, TYPE C, AS PER PLAN IN ADDITION TO THE REQUIREMENTS OF THE CMS, PROVIDE AN ADDITIONAL UNOBSTRUCTED 625 SQ FT (MINIMUM) AREA OF FLOOR SPACE TO BE USED AS A CONFERENCE ROOM FOR PROJECT RELATED MEETINGS, INCLUDING SIX 30 X72 WORK TABLES AND 36 PADDED, FOLDABLE OR STACKABLE CHAIRS. INCREASE THE BROADBAND INTERNET CONNECTION SPEED TO A MINIMUM OF 30 MBPS. INCREASE THE NUMBER OF ALL-WEATHER PARKING SPACES TO 25 SPACES TOTAL. Q23: Submitted February 2, 5:41 PM. Temp Ramp WN2 typical section on sheet 247 shows bid item 482 Portable Concrete Barrier, 32 (unanchored) on the outside of the temporary ramp. This is next to a 5 to 20 vertical temporary MSE wall. Should this be bid item 484 Portable Concrete Barrier, 32, Bridge Mounted (anchored)? Will anchoring to flexible (if chosen) 615 Temp Pavement be sufficient? A23: The plans will be clarified to specify Item 615 Pavement for Maintaining Traffic, Class A, shall be rigid within the limits of the temporary wall. The portable barrier shall be specified as Item 622 Portable Barrier, 32, Bridge Mounted.

10 Q24: Submitted February 3, 3:02 AM. On plan sheet 225 the Exit 15 Tuttle Crossing Blvd 1 Mile Sign near the bottom of the page appears as though it is intended to be mounted a little to the left of the center of what would have to be a truss of over 100 feet. There is a permanent aluminum sign truss of 50 ft. going in at this location that will support the other two signs detailed on this page, however this location would require the truss to be at least 100' that would likely require the larger truss and foundation. Is this really the intent of the Department, or are we misinterpreting the need to mount this sign on a large overhead truss in this phase of MOT? On page 231 and 242 of the plans at Station there is a proposed sign detailed that does not match the permanent sign that is not detailed on plan sheet What will be required at this location? Ref. Nos. 262 thru 265 are tabulated on plan sheet 1129B. The new trusses that are being painted have 3 pay items per sign Overhead Sign Support Truss. Since the box truss is aluminum those have not been painted in the past. If that is the case then there should only be 2 sections on all of the sign structures listed. The quantity should be reduced for all four painting bid items should be reduced accordingly. A24: The large overhead truss is not required for MOT. The Tuttle Crossing Blvd 1 Mile sign will be located at its permanent location along I-270 SB. The plan sheet will be updated to clarify. The sign at the exit ramp will be temporary. The plans will be updated to clarify. The box truss is to be painted. The quantity of 3 sections per sign structure is correct. Q25: Submitted February 3, 5:00 PM. We request the Department to re-evaluate the 10/19/15 late completion date and 133 calendar day maximum duration to complete Bridges #0257L, #0257R, #0271L and #0271R in Phases 2A and 2B. Did the Department include the following work items when contemplating this work duration: 1. Temporary shoring walls T-02 and T-03 which include 78 each drilled-in soldier piles and 136 each tiebacks. 2. Approximately 50,000 cy of roadway and structure excavation and associating lagging for the temporary shoring walls. 3. The challenging requirements to create clay molds and manufacture substantial formliner quantities to build both abutments at both the #0271L and #0271R bridges concurrently, with the formliner delivery occurring no later than mid-june Including weather days required per CMS C. 5. The abutment widening on the #0257L and #0257R bridges cannot begin until after the closure of US The architectural relief on the parapets on all four bridges requires the parapets to be formed and poured instead of slipformed. 7. Structure water cure requirements per CMS Table A25: The schedule will not be changed. Q26: Submitted February 4, 8:36 AM. There are two items for Work Zone Raised Pavement Markers, bid items 462 and 463. Is it the department s intent for all 614 RPMs to fall under bid item 462 Work Zone Raised Pavement

11 Marker and for all RPM conforming to CMS 621 to be paid for under bid item 463 Work Zone Raised Pavement Marker, APP? A26: Yes, it was the intent to identify the different WZRPM based upon the MOT phasing. Q27: Submitted February 4, 9:56 AM. We have contacted a number of form liner suppliers regarding the liner for this project. There are several suppliers who cannot quote this job due to the strict requirements set forth in the Special Provisions for Aesthetic Treatments. Of the few suppliers who have the capability to follow these requirements, none are able to meet our schedule needs for the construction of the FRA L/R bridges within in the 133 calendar day window. As stated previously, the four abutments at these bridges must be built simultaneously to meet this window duration. Because of this, all form liner will need to be on-site at the beginning of June. The uncertainty of shop drawing requirements (see previously submitted question) and the City s approval process further compound this issue. Please revise the contract documents and make revisions to the requirements so that construction of the FRA L/R bridges is achievable. A27: The schedule will not be changed. Q28: Submitted February 4, 9:58 AM. According to sheet 1332 it shows Everlow Yews and Buffalo Junipers. It only has everlow yews on the subsummary list and only buffalo junipers on the bid sheet. Please clarify. A28: The Buffalo Juniper (Juniperus Sabina Buffalo ) should be bid and installed as Everlow Yew (Taxus x media Everlow ). Any reference to Buffalo Juniper in sections A-A and B-B on sheet 1332 shall be replaced with Everlow Yew. Q29: Submitted February 4, 10:00 AM. On the landscape sheets it states that we can use 8-10' trees. On the bid form, it only states 10' that can be used. Please clarify. A29: Evergreen Trees, both Taxodium distichum and Picea glauca shall have a minimum height of 10 at the time of acceptance as indicated in the general summary. The landscape sheets will be clarified. Q30: Submitted January 21, 11:02 AM. On page 1057, Plan Note #2 reads that the soldier piles shall be pre-fabricated with shear studs attached prior to installation into the drilled shaft. To keep the shear studs undamaged during excavation and lagging, would it be acceptiable to attach the studs as the walls are excavated down? A30: ODOT will allow field welding of the shear studs. A revised sheet will be issued. Q31: Submitted January 23, 10:18 AM. The text on 67/1763 says the 257 L and R substructure is critical to 1B completion. The bridge plans show substructure removal and piling beneath where 1B has traffic maintained. We also could not locate the shoring design to accommodate this abutment work. If the work on abutment substructure cannot begin until Phase 2A, and 2A cannot begin until 6/8/15 then the beams will not be ready 6/15/15 as noted in the bridge plans. This very likely will push off beam delivery at 257 R and the completion of 2B as well. What should these new dates be? A31: The following note will be clarified on Sht. 67, Phase 1B: US33 over Substructure (FRA L&R), as noted on sheets 1566/1763 and 1596/1763. The shading on sheets 143 and

12 153 will be clarified to reflect the direction per sheets 1566/1763 and 1596/1763. Shoring design shall be performed by the contractor. The steel girders will be available to the contractor on 6/15/15; the contractor can erect girders once necessary work is completed. Q32: Submitted January 28, 2:44 PM. The MOT note requires a switch in to 2A no sooner than 6/8/15, and the steel delivery for 271L is scheduled on 6/15/15 per note on the bridge plans. There can be no preconstruction of the substructure of 271, as it is under where traffic is maintained. There will be considerable extra cost due to the error in delivery date in the plans. We need to know what storage costs you negotiated so we can analyze if it will be cheaper to mobilize a crane in to double handle the beams and set up contracts with delivery trucks to move from a temporary storage facility to the bridge placement and all associated costs, or if it will be cheaper to pay the storage costs as indicated on the bridge plans. The same will apply to the 257L and R structures as the same error exists in those dates as well. There is also a high probability the steel for 1763 will need additional storage fees analyzed as well. Please provide those costs for analysis as well. If ODOT would reveal the storage fees, and make it available for all, it will even the playing field and ensure the lowest taxpayer cost. A32: The intent of having the structural steel available at the dates provided in the plans is to try and ensure that the steel fabricator will not be responsible for any delays to the contractor. The steel fabrication bids do include a unit price for additional storage cost per day. If the contractor needs additional storage beyond the stated delivery date, then the additional storage costs which are owed to the steel fabricator will be the responsibility of the contractor. ODOT will collect these costs from the contractor by decreasing the Lump Sum payment for pay item: STRUCTURAL STEEL MISC: ERECTION OF STATE SUPPLIED STEEL. The General Note for this pay item will be updated to detail the potential decrease in Lump Sum payment. Q33: Submitted February 4, 4:03 PM. The intermediate crossframe detail for Br on Sht 1462/1763 shows a field weld at the intersection of the diagonals while all of the other welds on the crossframe are shown as shop welds. Please clarify if it is truly the department's intent to have this weld performed in the field rather than in the shop by the fabricator. A33: A shop weld is acceptable for the weld at the intersection of the crossframe diagonals. Q34: Submitted January 22, 5:08 PM. Will the Department allow the use of Stay-in-Place metal deck forms on this project? A34: No, stay-in-place forms cannot be used. Q35: Submitted January 22, 5:08 PM. Sheets 202 and 203 show proposed Ramp EN open in Phase 2B. This will require the completion of Soil Nail Wall RW-2A, Cast-in-Place Walls RW-2B and RW-2D, SPL Wall RW-2C and Bridge BR#1763 along with the associated roadway work on Ramp EN and I- 270NB. Phase 2B occurs during the 133 day maximum shutdown of US33 through traffic during the summer of With the focus of the project in the summer of 2015 being the completion of four US-33 mainline bridges and approach roadway during the 133 day shutdown, it will be incredibly difficult to also complete all the elements associated with Ramp EN and open proposed Ramp EN in Phase 2B. We request the Department to modify the MOT

13 plan and maintain the 33WB to 270NB traffic on the existing ramp EN and allow proposed Ramp EN construction to continue into the second construction season. A35: The contractor is permitted to modify the MOT scheme as noted on sheet 60: If the contractor so elects, he may submit alternate methods for the maintenance of traffic, provided the intent and provision herein are followed, and no additional inconvenience to the traveling public results therefrom. No alternate plan shall be placed into effect until approval has been granted in writing by the engineer. At the end of the first construction season, the number of lanes and movements as indicated in the MOT plans will be open to traffic. At the end of the second construction season, all proposed lanes shall be open to traffic in their final alignment. The contractor shall set the steel girders for FRA (Ramp EN over South Fork Indian Run) in the first construction season as required by the notes in the bridge plans.
