1 ITB-CD-CME Bridge Rehabilitation ADDENDUM NO. 2 ISSUE DATE: September27, 2016 DUE DATE: October 27, 2:00pm SUBJECT: Questions & Answers INTENT: This addendum is issued prior to the date bids are due in order to incorporate the following clarifications, additions, omissions, deletions, or changes into the Contract Documents. Except as hereinafter specified, the work shall be in accordance with the drawings and specifications. Bidders are to use the changed quantities of the items listed in this addendum in their proposals, and it shall become a part of the Contract Documents when construction is executed. Bidders are reminded that this addendum must be noted on the Official City Acknowledgment Form when they submit their proposal. A signed copy of the addendum must also be included with the bid submittal. 1. Please Note: Attached is the allowance sheet, additional work sheet, inspection sheet, and the engineers estimate sheet. 2 ALLOWANCES The following Allowances shall be included in the Lump Sum Base Bid. 1.1 ALLOWANCES A. Allowances: Include in the Contract, a stipulated sum/price of the following amounts indicated for use upon the City s instruction. 1. ALLOWANCE #1 Additional Landscaping (if needed, above and beyond the expected restoration of the disturbed construction areas): $5, ALLOWANCE #2 Bridge Repairs (see exhibits 1 through 3): $30, B. Contractor's costs for products, delivery, installation, labor, insurance, payroll, taxes, equipment rental, overhead and profit will be included in Change Orders authorizing expenditure of funds from this allowance. The cost of the bond for the amount of Allowance shall be included as part of the lump sum base bid. C. Funds will be drawn from an Allowance only by Change Order. Contractor can proceed with Change Order Work against Allowance with direction from the Contract Administrator. The Contractor shall not proceed with any work that will exceed the amount of Allowance remaining. Submit proposals for purchase of products or systems included in allowances, in the form specified for Change Orders. D. Submit invoices or delivery slips to show the actual quantities of materials delivered to the Site for use in the fulfillment of each allowance. E. Credits can only be added to an Allowance by Change Order. The Contractor may not use a credit until a Change Order is fully executed. F. Notwithstanding the Contractor s objection, the City may at any time reduce the funds remaining in the Allowance by Change Order. G. ELEMENT INSPECTION NOTES: CS3 = The underwater inspectors noted the following: Pile 3-2, southeast corner at the top of the marine growth, delamination 4ft. H x 7in. W. CS2 = The piles have small construction related surface spalls. CS1 = At several piles, the ends of the cut and patched pickup points are visible. The northeast corner of Pile 2-1 has a 12in. long x 1/64in. wide vertical crack with efflorescence, adjacent to west bulkhead. ELEMENT/ENV: 215/4 R/Conc Abutment 84 lf. REPAIR CS1 = The inboard faces of the abutment caps typically have water and mildew staining. At the south end of Abutment 1, there is a 12in. section of protruding rebar which is a tripping concern.
